Q1: What is a PiCCO catheter and when is it used in patient care?
A PiCCO, or Pulse Contour Cardiac Output catheter, is used for hemodynamic monitoring in cardiovascularly unstable patients. It employs a thermodilution method to analyze a patient's cardiovascular status. The catheter requires a central venous catheter and is inserted into an artery, typically the femoral artery, to measure cardiac output and other critical hemodynamic parameters.
Q2: How is the femoral artery identified before PiCCO catheter insertion?
Ultrasound guidance is used to identify the femoral artery. The probe is positioned in a mediolateral direction to visualize the femoral vein, femoral artery, and femoral nerve. The mnemonic NAVY helps identify anatomical structures: nerve, artery, vein, Y fronts or midline. This sonographic identification ensures accurate and safe puncture site selection.

The puncture site is prepared by disinfecting with spray disinfectant, allowing it to dry completely. Local anesthesia with mepivacaine 1% is infiltrated at the puncture and suture sites. The area is then covered with sterile protective film. A protective sheet is placed under the patient's pelvis, and the site may be razored to remove visible dirt before final disinfection.

A 20-milliliter bolus of cooled sodium chloride 0.9% is administered through the central venous catheter. A temperature probe in the PiCCO catheter measures the temperature difference over time following injection. At least three separate measurements should be performed for reliable results. If variation exceeds 10% between measurements, they should be repeated and removed from overall calculation.
Q6: What equipment connections are required to set up the PiCCO monitoring system?
The PiCCO monitor adapter attaches to the telemetry monitor, followed by the PiCCO cable connected to the red attachment point.
