Summary

Abstract

Introduction
人体肠道微生态系统极其复杂,一个健康人的肠道中有超过1000种细菌1。肠道菌群参与维持肠道的正常生理功能和免疫反应,与人体有着密不可分的关系。越来越多的证据表明,肠道微生物组构成了人体的最后一个器官,它是人体的一部分,而不仅仅是一组寄生虫2。肠道微生物群、其代谢物和生命早期建立的宿主免疫系统之间的“健康”共生关系对于维持肠道稳态至关重要。在一些异常情况下,如慢性炎症,身体内部和外部环境的变化严重破坏肠道稳态,导致肠道微生物群落持续失衡,称为生态失调3。事实上,暴露于多种环境因素,包括饮食、药物和病原体,会导致微生物群的变化。
生态失调与多种肠道疾病的发病机制有关,例如炎症性肠病 (IBD)、肠易激综合征 (IBS) 和伪膜性肠炎,以及越来越多的肠外疾病,包括心血管疾病、肥胖和过敏4。微生物群分析显示,IBD患者的细菌多样性急剧减少,并且某些特定细菌菌株的种群显着改变5,6。这些研究表明,IBD患者中的乳腺螺旋体科和拟杆菌较少,但变形杆菌和放线菌更多。据信,IBD的发病机制与多种致病因素有关,包括肠道菌群异常、免疫反应失调、环境挑战和遗传变异7。大量证据表明,肠道细菌在IBD8,9的起始和应用阶段发挥作用,表明纠正肠道生态失调可能代表IBD治疗和/或维持治疗的新方法。
粪便菌群移植(FMT)的雏形始于中国古代10.1958年,艾斯曼博士和他的同事们通过灌肠成功用健康供体的粪便治疗了四例严重的伪膜性肠炎,开启了现代西医利用人类粪便治疗人类疾病的新篇章11。 已发现艰难梭菌 感染 (CDI) 是伪膜性肠炎12 的主要原因,FMT 在治疗 CDI 方面非常有效。在过去的八年中,FMT已成为治疗复发性CDI13的标准疗法,促使进一步研究FMT在其他疾病(如IBD)中的作用。在过去的二十年中,许多病例报告和队列研究记录了FMT在IBD14患者中的使用。一项包括 12 项试验的荟萃分析显示,62% 的克罗恩病 (CD) 患者在 FMT 后达到临床缓解,69% 的克罗恩病患者出现临床缓解15。尽管有这些令人鼓舞的发现,但FMT在IBD管理中的作用仍然不确定,FMT改善肠道炎症的机制知之甚少。在FMT加入目前临床IBD治疗方案的武器库之前,需要进一步调查。

Protocol
对动物进行的所有程序均已获得加尔维斯顿德克萨斯大学医学分院机构动物护理和使用委员会的批准(协议#1512071A)。
1. 收集新鲜粪便颗粒
- 准备无菌纸巾、钝端镊子和 50 mL 锥形管。
- 将一些纸巾和镊子放在单独的高压灭菌袋中,并在180°C下干热高压灭菌30分钟。也使用无菌锥形管。称量锥形管并在管子上写下它们的重量。
- 打开动物房的生物安全柜。
- 取一个没有任何垫料的高压灭菌清洁小鼠笼,并将其放入生物安全柜中。取下盖子和食品架,然后将它们放在柜子内。
- 将一些无菌纸巾放在笼子的底部,然后将金属架放回笼子的顶部。
- 确定年龄匹配的粪便供体,并将小鼠笼放入生物安全柜中。打开笼子,轻轻抓住供体鼠标(C57BL / 6J)的尾巴,并将其放在干净笼子顶部的金属架上。
- 将笼盖放在架子的顶部,等待动物排便。
注意:将几只猫砂矿同时放在机架上。 - 收集粪便颗粒并将其放入无菌 50 mL 锥形管中。按性别汇集颗粒。不要混合从男性和女性收集的粪便颗粒。
- 再次称量管子并计算粪便颗粒的重量。
2.粪便悬浮液的制备
- 准备无菌溶液(10%甘油在生理盐水中)。
- 每克粪便颗粒向锥形管中加入 10 mL 10% 甘油/生理盐水。
注意:如有必要,将溶液体积增加到 20 mL。这项研究也为每个沉淀(5-10 mg)使用了 1 mL 溶液。 - 使用通风橱内的台式均质机或搅拌机以低速均质混合物以重悬粪便(3 X 30 s)。
- 通过 2 层无菌棉纱布 (10.2 cm x 10.2 cm) 过滤粪便悬浮液。将滤液暂时存放在冰箱中长达6小时或将其包装到无菌低温小瓶中并将其储存在-80°C冰箱中。
- 按照标准程序彻底清洁均质机或搅拌机。
3.口服强饲法给予粪便悬浮液
- 如果使用冷冻样品,请在冰上解冻冷冻粪便悬浮液。通过涡旋混合解冻的粪便悬浮液。
- 将新鲜或解冻的粪便悬浮液转移到 1 mL 注射器中。
注意:每只小鼠将总共接受200μL粪便悬浮液,对照组中的每只IL-10-/- 小鼠将获得200μL的10%甘油/生理盐水17。 - 称量小鼠并选择正确的管饲针尺寸和最大剂量体积。
注意:对于体重在20-25克之间的小鼠,使用带有2.25毫米球的20克3.81厘米弯曲管饲针。请查看 gavageneedle.com 以获取更多信息。 - 通过测量从小鼠鼻尖到剑突(胸骨底部)的长度来测试管饲针。用10%甘油/盐水或粪便悬浮液填充注射器,并去除注射器和针头内的气泡。
注意:如果针头长于长度,请在鼻子水平的针轴/管上做标记。不要将针头/管子穿过动物超过该点,以防止胃穿孔。 - 将一个小鼠笼放在生物安全柜中,取下塑料笼盖,并将金属架留在原位。
- 抓住一只鼠标的尾巴,把它放在金属架上。用一只手抓住老鼠的尾巴,用另一只手的拇指和中指抓住肩膀上的皮肤来约束动物。这样,前腿向侧面伸展,这将防止前脚将针推出。轻轻地将动物的头部向后伸出,并用一只手将头部固定到位。
注意:练习鼠标操作,直到实验者完全有信心,然后再进行实验。 - 将管饲针放在口腔内的舌头顶部。沿着上腭轻轻前进,直到针到达食道。通过一个动作平稳地通过针头。如果感觉到任何阻力,请勿用力针头。取出针头,再试一次。
- 正确放置并验证针头后,通过推动连接到针头上的注射器来缓慢施用材料。不要旋转针头或向前推动针头,这可能会使食道破裂。给药后,轻轻拉出针头。
- 将鼠标放回其主笼。通过寻找呼吸困难或痛苦的迹象来监测动物 5-10 分钟。FMT后12-24小时再次监测小鼠。
4. 疾病监测和安乐死
- 通过潜血和/或结肠镜检查纵向监测小鼠的IBD发作21。通过经胸超声心动图评估心脏功能22,23.
- 通过在用异氟醚(1%-4%)的深层麻醉下斩首对动物实施安乐死。
- 用抗凝剂在微量离心管中收集血液,并在冷冻离心机(4°C)中以1000-2000× g 离心10分钟。将指定血浆上清液保存在-80°C冰箱中。
- 安乐死后,使用瑞士卷技术24 制备小鼠结肠以进行组织病理学分析(H&E染色)25。
- 使用酶免疫测定 (EIA) 试剂盒测量血浆中的 B 型利钠肽 (BNP)23。

Representative Results
我们对2个月大的C57BL / 6J野生型(WT)和IL-10敲除小鼠进行了3次健康供体FMT(每月一次,持续3个月)。年龄匹配的C57BL / 6J小鼠(年龄差异应为<2个月)作为粪便供体,每次使用新鲜粪便颗粒。EIA测定显示,IL-10缺乏小鼠血浆中的BNP显着升高,健康供体FMT显着减轻了BNP水平的增加(图1A,n = 5, p <0.05)。与WT小鼠相比,超声心动图检测到IL-10-/- 小鼠的左心室射血分数(LVEF)显着降低;FMT显着消除了减少(图1B,n = 5, p <0.05)。这些发现表明,健康的供体FMT减轻了结肠炎引起的心脏损伤。

Discussion
FMT作为一种创新的研究性治疗方法,近年来已成为各种疾病治疗的热门话题,因为共生微生物群的生态失调与多种人类疾病的发病机制有关,包括IBD,肥胖,糖尿病,自闭症,心脏病和癌症26。虽然机制尚未确定,但据信FMT通过建立新的生物菌群并防止残留细菌的损失而起作用。本文提出的方法采用口服强饲法作为给药途径,其已被证明有效27.我们之所以选择口服途径,是因为口服给药是最方便、最经济的,也是大多数患者的首选。此外,在临床试验中,已发现口服胶囊FMT是治疗复发性CDI的有效方法28。其他常见的上消化道给药途径是鼻胃管、鼻空肠管、空肠造口管和食管胃十二指肠镜检查。常见的下消化道递送途径包括结肠镜检查、结肠经内镜肠内导管 (TET) 和直肠灌肠。然而,FMT的最佳途径仍不确定20。目前,上消化道给药被认为是最合适的29,但没有适合所有患者的理想途径。
粪便颗粒收集中使用的工具和容器应无菌,以防止任何交叉污染。由于免疫力的性别差异,从男性和女性收集的粪便颗粒不应混合。在动物模型和人类中,两性之间的微生物群存在差异30。这些性别差异通常会导致局部胃肠道炎症、宿主免疫和对一系列炎症性疾病的易感性的性别依赖性改变31。粪便颗粒可以在无菌磷酸盐缓冲盐水或10-50%甘油/生理盐水中匀浆,10%甘油已被广泛采用29。在这项研究中,使用了10%,20%和50%的甘油/盐水,它们在冷冻条件下都提供了良好的细菌保存。均质化应在通风橱内低速进行,以尽量减少暴露于在此过程中产生的可吸入气溶胶。粪便悬浮液应通过两层无菌纱布或20μm尼龙过滤器过滤,以去除可能阻塞管饲针头的大颗粒。过滤后的粪便悬浮液应立即放入冰箱中,其余的可以等分并储存在-80°C冰箱中。如果粪便是从人类供体那里获得的,冷冻尤其必要。荟萃分析发现,冷冻 FMT 在复发性 CDI 患者中与新鲜 FMT 一样有效32,33,34。然而,Cui等人也观察到,在FMT后6个月,新鲜粪便细菌组的CD患者的反应率比冷冻粪便细菌组35高26.7%,这表明在某些情况下,新鲜FMT比冷冻FMT更好。
在此阶段,FMT输注的最佳剂量和频率仍然未知。研究发现,对单一FMT治疗的临床反应持续时间是短暂的,不足以诱导受者肠道菌群的根本性改变,需要序贯FMT治疗来维持CD缓解36,37。我们连续3个月每月进行一次FMT,我们的数据显示出良好的治疗效果。在一项正在进行的研究中,我们将在一组IL-10缺陷小鼠中进行12个月的每月FMT,并将在研究结束时评估肠道微生物群,肠道炎症和心脏功能。我们预计序贯FMT将比单一FMT显示出更好的治疗效果。
