Milky Spot Analysis

Milky Spot Analysis is the study of milky spots, specialized immune-cell aggregates in the omentum, a fatty membrane that helps coordinate abdominal immune surveillance. These structures contain macrophages, lymphocytes, and other immune cells associated with small, permeable blood vessels, allowing fluid, cells, and particulate material from the peritoneal cavity to enter and interact with local defenses. In cancer research, microscopy, staining, and quantitative analysis of milky spot number, size, distribution, and cellular composition can clarify how tumor cells reach and colonize the omentum. The approach supports investigation of peritoneal metastasis, tumor–immune interactions, and therapies targeting abdominal dissemination.

We outline a protocol that implements both in vivo and ex vivo approaches to study ovarian cancer colonization of peritoneal adipose tissues, particularly the omentum. Furthermore, we present a protocol to quantitate and analyze immune cell-structures in the omentum known as milky spots, which promote metastases of peritoneal adipose.
