Heterotopic Heart Model

The heterotopic heart model is an experimental transplantation system in which a donor heart is implanted alongside the recipient’s native heart, allowing cardiac graft function to be studied without replacing the original organ. Surgeons connect the donor heart to the recipient’s major blood vessels so that it is perfused and contracts independently, while its activity can be monitored through palpation, electrocardiography, or imaging. This model supports research on transplant immunology, graft rejection, ischemia-reperfusion injury, organ preservation, and immunosuppressive therapies. Because recipient survival does not depend immediately on graft performance, investigators can assess cardiac transplantation mechanisms and treatments under controlled conditions.

The mouse heterotopic heart transplantation model has been proven by many investigators to be an important method for studying mechanisms of rejection and immune response. However, the techniques involved are still challenging. By modifying standard techniques we have had success with more than 1000 transplants.

This study presents a pre-clinical heterotopic abdominal heart xenotransplantation model using α-Gal knockout (GTKO) pigs and macaques. The model incorporates optimized recipient selection, vascular anastomosis techniques, and perioperative management strategies. It is suitable for evaluating novel immunosuppressive agents and provides a convenient platform for advancing heart xenotransplantation.

In order to understand the cellular and molecular mechanisms underlying neotissue formation and stenosis development in tissue engineered heart valves, a murine model of heterotopic heart valve transplantation was developed. A pulmonary heart valve was transplanted to recipient using the heterotopic heart transplantation technique.

The cuff technique shortens and facilitates the model of heterotopic cervical heart transplantation in mice by avoiding technically challenging suture anastomoses of small vessels. The technical details shown in this video paper should allow researches to establish this model in their laboratories.
