Dorsal Midline Approach

The dorsal midline approach is a neurosurgical technique that accesses the posterior aspect of the spinal cord through the midline dorsal surface, providing a direct route to selected intramedullary lesions and spinal structures. After posterior exposure, surgeons identify the dorsal median sulcus, a natural anatomical groove that separates the spinal cord’s dorsal columns, and use it to guide a carefully centered myelotomy while minimizing disruption of surrounding neural tissue. This approach supports biopsy or removal of spinal cord tumors, vascular malformations, and other lesions, while anatomical landmarks and microsurgical methods help preserve sensory pathways and neurological function.
