摘要

引言

内侧颞叶(MTL)被认为是感觉信息整合的最高级脑区之一1,长期以来一直是靶向分析的常见研究对象。例如,海马及其相关的旁海马区域在记忆研究中已被广泛研究2-5。此外,杏仁核在情绪加工及情绪与认知相互作用研究中的作用也经常被强调6-11。近年来,在新兴的人格神经科学领域中,多个MTL区域也受到关注,该领域将这些脑区及其他脑区的结构与功能与人格特质的个体差异相联系12。评估MTL结构的解剖与功能对于退行性疾病的诊断具有重要意义,因为在这些疾病中,不同的MTL结构可能出现特定的结构和功能异常。例如,在阿尔茨海默病(AD)中,可观察到内嗅皮层和海马的显著萎缩13,14,而海马萎缩可预测由轻度认知障碍向AD的转化15。近年来,自动分割算法在皮层和皮层下结构的分割中日益流行,但与任何工具一样,这些程序在某些情况下不可避免地会出现错误。在这种情况下,研究者应具备识别MTL结构解剖边界的必要知识和指导原则。现有文献中的趋势往往是针对MTL的个别亚区16-21,其中许多方案倾向于聚焦于海马16-19

与大多数现有的内侧颞叶(MTL)追踪指南不同,本方案提供了一套全面的指导原则,可实现所有MTL亚区的清晰定位。文中描述了以下MTL结构的追踪方法:杏仁核(AMY)、海马(HC)、嗅周皮层(PRC)、内嗅皮层(ERC)以及海马旁皮层(PHC)。首先追踪AMY和HC,随后追踪海马旁回(PHG)的各个结构。需注意,此处使用的通用术语HC指代海马结构,包括海马本体、下托以及钩回的后段22-24。此外,PHG可分为前部和后部两个部分。在PHG的前部,可进一步分为外侧前部PHG和内侧前部PHG,其皮层区域分别对应PRC和ERC。PHC即PHG后部的皮层区域,对应于真正的海马旁皮层。为简化表述,本文将使用术语PRC指代外侧前部PHG,ERC指代内侧前部PHG,PHC指代后部PHG。每个结构的分割均始于对其前界和后界以及其他相关解剖标志的粗略定位,随后在冠状面上沿前-后/头-尾方向逐层进行实际的轮廓勾画。在所有情况下,均需密切参考矢状面和轴向图像,以辅助确定解剖边界和标志的位置。

此类追踪指南的必要性还体现在展示自动分割与手动分割方案输出结果可能差异的图示中。当前视觉格式下描述内侧颞叶所有结构的方案优势在于,能够反映解剖结构的变异情况例如, 脑沟(CS)的深度可能会影响边界的定义,可在周围解剖结构的背景下进行描述。例如, PRC和ERC的内侧与外侧边界位置随CS的深度不同而变化25这可能对于缺乏经验的示踪者,或仅对单个或独立结构进行示踪的有经验示踪者而言并不清晰或难以理解;据我们所知,目前尚不存在此类视觉上全面的指导规范。

本方案明确阐述了先前研究中用于内侧颞叶(MTL)示踪的指南,该研究识别了MTL不同亚区在情绪增强记忆效应中的差异化作用26,并根据近年来结构磁共振(MR)成像技术发展所支持的更高分辨率脑图像进行了调整。示踪过程以一名健康志愿者(女性,24岁)使用3T MR扫描仪获取的扫描图像为例进行说明。解剖图像采用三维MPRAGE序列采集(TR = 1,800 msec;TE = 2.26 msec;FOV = 256 x 256 mm;体素大小 = 1 x 0.5 x 0.5 mm),采集角度平行于前连合-后连合(AC-PC)连线。若图像数据以不同角度采集(如斜位方向),应将数据重新网格化为与AC-PC平行或垂直的方向,以确保解剖标志描述的准确对应。随后将图像转换为NIFTI格式,并输入分割软件27进行人工示踪。本方案所用扫描数据来自一项经机构审查委员会批准的研究,志愿者已签署书面知情同意书。

通过整合针对这些结构的多种独立示踪方案18-22,25,28-31,以及解剖学分析和图谱资料23,32-37,本方案提出了一套全面的指导准则,以解决现有文献中的不一致问题。结合配套的可视化材料,本研究有望促进对内侧颞叶(MTL)结构的更清晰理解,并激发未来研究对人工分割方法的兴趣,无论将其作为MTL示踪的主要方法,还是作为自动分割的补充手段。本方案提供了准确、直观且便捷的MTL解剖学指导,帮助研究人员确定所有MTL亚区相对于邻近结构的位置,即使分析中仅特别针对部分MTL结构。这不仅将提高定位的准确性,还可在MTL高度可能出现的形态学变异情况下,帮助示踪人员做出合理判断。这些指南可应用于涉及MTL结构和/或功能磁共振成像(MRI)的研究,包括容积分析、脑异常检测,以及在健康人群中的功能、解剖和纤维束成像分析的定位操作。若主要解剖标志物相对保存完好,本方案也可用于指导患者(例如,萎缩患者)MTL结构的分割。对临床受试者数据进行示踪可能需要额外的时间和精力,具体取决于萎缩和/或解剖结构改变的严重程度。

在定义感兴趣区域(ROI)时,必须考虑脑回与皮层之间的区别。从解剖学上讲,脑回包含白质和灰质,而皮层仅指灰质。根据ROI的预期用途,分割可能包含或排除白质。

我们建议依次逐个亚结构、每次一个半脑地进行示踪。某些软件包27 可将一个切片上勾画的边界复制粘贴到后续切片上,从而加快操作速度。建议根据需要参考对侧半球,以确保两侧结构的一致性。例如在检测解剖标志时)。此外,也可对双侧半球内的相同结构进行平行追踪。无论采用顺序追踪还是平行追踪,一旦完成追踪过程,操作者应仔细核对最终结果,并根据需要进行调整,同时参考双侧半球及多个切面视图。根据操作者的经验水平及影像数据的分辨率,对健康受试者的内侧颞叶(MTL)进行手动分割所需时间可能从新手操作者的8–10小时或更长,到经验丰富的操作者仅需3–4小时不等。

方案

1. 杏仁核

  1. 杏仁核的前部切片
    1. 确定杏仁核(AMY)中岛阈首次出现的第一个切片,此处额叶与颞叶之间的白质连接连续且可见30。在冠状面视图中,以角束作为杏仁核的下外侧边界。
    2. 以视交叉作为杏仁核出现的解剖标志。利用轴向和矢状面视图区分早期切片中杏仁核与周围钩回的结构。在轴向视图中沿杏仁核周围的白质纤维束走行,以排除内嗅区域32
    3. 向后移动,确定前连合在双侧半球均连续完整的第一个切片20,此时杏仁核呈现其典型形态。以内嗅沟作为上内侧边界,从半环状沟底部沿白质至杏仁核下端画一条假想线作为下内侧边界,以颞叶茎部作为外侧边界,并返回至内嗅沟,逆时针追踪完成杏仁核轮廓绘制31
  2. 杏仁核的后部切片
    1. 注意在此层面,杏仁核与海马(HC)均在同一张(或几张)切片中可见。
    2. 仍在冠状面视图中,确定杏仁核最后一个切片,该切片中结构位于侧脑室颞角内侧延伸部分38(若脑室不存在,则为海马伞)的上方,且位于钩回(即海马头部隆起部分)的外侧。通过矢状面和轴向视图检查,确保追踪准确且一致。
    3. 从岛叶下环状沟底部向视束画一条假想线,作为杏仁核的上界31,该线也可将其与苍白球和壳核的灰质区分开来。
    4. 沿半月回走行作为上内侧边界,并排除钩回32。利用侧脑室下角和颞叶茎部进行外侧边界的界定。
  3. 沿前后方向连续的杏仁核切片
    1. 系统性地逐一切片追踪杏仁核,参考上述相关指导原则。在杏仁核前部区域,采用与最前切片相同的边界;相反,在杏仁核后部区域,则采用与最后切片相同的边界。
    2. 继续使用轴向和矢状面视图辅助定义并进一步优化杏仁核的边界。

2. 海马体

  1. 定位海马(HC)
    1. 当侧脑室颞角沿杏仁核(AMY)的下外侧边界出现时,开始追踪海马(HC)。如果在先前的切片中侧脑室颞角已存在,则应注意:海马起始的标志是侧脑室颞角扩大并向上方外侧延伸。
    2. 在海马最后一次于侧脑室三角区腹内侧出现的层面结束追踪31。始终利用辅助视图帮助定位海马及其边界。
  2. 海马(HC)的边界定义
    1. 以侧脑室颞角为界,划定海马的外侧边界。当侧脑室颞角显示不清时,可从分割中排除一行体素以表示该边界。
    2. 在下方,使用钩束(或其假想延伸线)至脑室腔,以区分海马与海马旁回(PHG);上方则以alveus与fimbria共同作为上界。全程采用一致的定义进行追踪。
    3. 此外,在分割中包含下托(subiculum),使其内侧毗邻海马旁回(PHG)的白质束,上方与钩回(uncus)的弯曲轮廓对齐,并主要从海马水平向外延伸39。向后追踪时,持续遵循上述定义,直至距状沟(calcarine sulcus)出现干扰为止。
  3. 注意海马(HC)的分段
    1. 应注意海马可分为三个部分:头部、体部和尾部。
    2. 以钩回尖端(uncal apex)的出现作为海马头部过渡至体部的标志;以结构快速上移并体积增大作为海马尾部出现的标志,这一变化通常与穹窿脚(crus of the fornix)的出现相一致23,30,40
  4. 追踪以下结构时需特别注意
    1. 将后部钩回(posterior uncus)纳入分割范围。
    2. 在冠状切面上,应将alveus上方的脉络丛从分割中剔除,但在低分辨率图像中可能难以实现。
    3. 参考两个辅助视图,避免将尾状核尾部及丘脑枕部误纳入海马尾部的上部区域。
    4. 注意fascicular gyrus的出现位置:在穹窿脚层面,fascicular gyrus最初由fasciola cinerea与海马尾部分隔,更靠后方则成为距状沟上方的灰质结构,应避免将其纳入分割32

3. 海马旁回

  1. 记录 PHG 的分区
    1. 请注意,海马旁回可分为两个主要部分:前部海马旁回(,PRC 和 ERC),以及后部 PHG(,PHC)。

    4. PRC与ERC之间的边界可能因切片而异。

      2. 在以下情况下 常规 CS(深度1–1.5 cm),沿侧副沟内侧壁中点至该沟外侧壁内端的区域追踪PRC25.

    6. 沿ERC追踪至海马旁回内侧回终点或钩回尖端后方1.5 mm处37 .

  3. 海马旁回后部切片
    1. 从PRC末端后方的切片开始追踪PHC,直至HC尾部末端后方4 mm处32. 文献中的其他定义在讨论部分有所描述。同样,根据研究目的决定是否包含白质。
    2. 在ERC消失后,采用与PRC后部相同的方法勾画PHC。一旦出现扣带回的白质,即以其作为PHC的上界。以此方式继续追踪,直至出现距状沟,该沟将PHC的上内侧界限定至其下缘。30.
    3. 若在距状沟出现之前已形成小型沟回,应将其纳入分割范围,但需谨慎区分该沟回与距状沟。

结果

手动分割与自动分割之间可能差异的示意图

AMY、HC、PRC、ERC 和 PHC 手动分割的三维模型如图1所示,分割的矢状面切片如图2所示。为了展示手动与自动勾画之间可能存在的极端差异,下图图3并列显示了某代表性受试者 AMY 的切片图像,其中自动分割存在错误,并与手动勾画结果进行了对比。尽管自动分割软件能够识别出这些结构的主要本体,但其分割结果较为粗糙,与手动分割相比,导致 AMY 体积被低估。

传统上,许多研究人员认为手动分割是金标准。然而,由于内侧颞叶(MTL)结构形态高度可变,且这些结构与周围神经组织及非神经区域之间的磁共振成像(MRI)对比度通常较弱,使得对各个结构进行精确勾画变得复杂。在历史上,文献中对某些MTL结构的描述存在分歧。例如,在对内嗅皮层(PRC)进行分割时,有研究将侧副沟描述为中断的35,36,但此前其他文献则将其描述为较短的侧副沟(CS),未超过岛阈水平30。这种解释上的差异导致了PRC前界定义的不同,这在一定程度上可以解释为何在不同研究中观察到个体结构分割结果存在变异。在分割过程中,手动描记在这方面具有独特优势,因为视觉检查允许根据具体情况作出调整,而这种适应性在自动分割中难以实现。当前方案中用于界定PRC和外嗅皮层(ERC)的标准即可说明这一点。值得注意的是,有研究者建议应始终将PRC的内侧边界定义为侧副沟内侧壁中点,而不考虑沟的深度30。关于后部海马旁回(PHC),由于其后边界有时界定模糊,现有文献中存在多种定义。在本方案中,采用海马尾部终止点之后的边界,以反映在解剖学32和功能研究46中...

A correction was made to A Comprehensive Protocol for Manual Segmentation of the Medial Temporal Lobe Structures. Table 1 and its legend were updated. References 10 and 14 were also updated.
