在啮齿类动物中进行视网膜下注射以递送悬浮的视网膜色素上皮细胞

摘要

本文以多媒体形式展示了一种被学术界广泛认可的在大鼠和小鼠中进行视网膜下腔注射RPE细胞悬液的实验方案。该方法可用于评估移植的RPE细胞在视网膜变性动物模型中的挽救潜力、安全性以及存活能力。

摘要

将光转化为电冲动的过程发生在视网膜外层,主要由视杆细胞、视锥细胞光感受器以及视网膜色素上皮(RPE)细胞完成。RPE细胞为光感受器提供关键支持,RPE细胞的死亡或功能障碍是年龄相关性黄斑变性(AMD)的典型特征,而AMD是55岁及以上人群永久性视力丧失的首要原因。尽管目前尚未发现治愈AMD的方法,但在患病眼中植入健康的RPE细胞可能成为一种有效的治疗手段,且可从多能干细胞中大量生成RPE细胞。关于RPE细胞移植的安全性和有效性的若干重要问题仍可在动物模型中进行研究,目前已有多个被广泛接受的RPE注射方案。本文所述技术已被多个研究团队在不同研究中采用,首先使用锐利针头在眼球上制造一个小孔,然后将装有细胞的钝头针头注射器经此孔插入,并穿过玻璃体,直至轻轻接触RPE层。采用这种相对简单且所需设备最少的注射方法,我们实现了干细胞来源的RPE细胞在宿主RPE细胞间的稳定且高效的整合,从而在动物模型中显著防止了光感受器的退化。虽然不属于实际操作步骤,我们还描述了如何评估注射所引起的创伤程度,以及如何验证细胞已被准确注入视网膜下腔的方法。 体内 成像方式。最后,该方案的应用不仅限于RPE细胞;也可用于向视网膜下腔注射任何化合物或细胞。

引言

感觉视网膜由神经元、胶质细胞和内皮细胞构成的功能性层次结构组成。位于视网膜后部的光感受器受光激活,通过光转导将光子转化为电信号,这些信号经中间神经元进一步处理后传递至大脑的视觉皮层。光转导过程离不开穆勒胶质细胞和视网膜色素上皮(RPE)细胞的协同作用。RPE细胞以单层结构排列在光感受器后方,执行多种多样且对光感受器功能和稳态至关重要的功能。事实上,RPE细胞与光感受器之间具有高度的相互依赖性,通常被视为一个功能单位。RPE细胞的死亡或功能障碍会对光感受器造成严重的继发性损害,并与老年性黄斑变性(AMD)密切相关,后者是老年人致盲的首要原因1,2。

RPE 可通过多种分化和注射技术,从人胚胎干细胞(hES)和/或人诱导多能干细胞(hiPS)中高效获得,并植入啮齿类动物的视网膜下腔32,33。(参见 Westenskow et al. 发表的一篇多媒体形式的方法学论文,其中展示了我们所采用的定向分化方案34。)关于外源性递送的 RPE 细胞在植入后的安全性、存活率及功能能力,目前仍存在若干关键问题,因此掌握在啮齿类动物中进行视网膜下注射的技术至关重要16,18,21,29,36,37。RPE 细胞的递送并非易事,且学界对于最有效的注射技术尚存争议。本文所描述的方案是一种简单而有效的视网膜下腔团注式递送 RPE 细胞的方法,并已被应用于首个干细胞来源 RPE 移植的临床试验31。(读者也可参考 Eberle et al. 在 JoVE 上发表的另一篇文章,其中展示了啮齿类动物视网膜下注射的另一种操作方式38。)

本文所述技术无法实现可视化,且不可避免地会造成创伤(与其他所有视网膜下注射技术一样)。该操作通过在角膜缘血管下方制造一个小孔,沿巩膜路径插入钝针,将细胞悬液注射至对侧视网膜下。注射者在钝针接触视网膜时会感受到阻力。尽管如此,注射后可直接观察到细胞,且可通过使用短暂性荧光标记物标记视网膜色素上皮(RPE)细胞,并利用共聚焦扫描检眼镜(cSLO)进行检测,从而确定所诱发的视网膜脱离程度。此外,还可使用光学相干断层扫描(OCT)系统监测创伤情况,并轻松识别注射部位。

方案

注意:所有动物均按照斯克里普斯研究所制定的伦理准则进行处理。

1. 注射材料的准备(约20分钟)

  1. 预热细胞解离溶液(最好选择可通过稀释失活而非通过血清失活的溶液)、无菌 PBS 和培养基(表 1)。
  2. 通过拆卸带有钝针的注射器,并将其各部件在水中煮沸 15 分钟,进行灭菌。

2. 注射用视网膜色素上皮细胞的制备(约30分钟至1小时)

  1. 使用预热的细胞解离液在 37 °C 下孵育 5–8 分钟,以解离视网膜色素上皮(RPE)细胞。
  2. 轻轻刮取培养皿表面,使仍附着的细胞脱落。
  3. 用大量培养基(加满 15 ml 离心管)稀释细胞,以中和解离液,并进行细胞计数。
  4. 在 800 × g 条件下离心 5 分钟,收集细胞沉淀。
  5. 将细胞重悬于无菌预热的 PBS 中,浓度为 200,000 个细胞/µl(以便在 0.5 µl 体积中递送 100,000 个细胞),并转移至 1.5 ml 微量离心管中。
  6. 可选:加入活细胞瞬时荧光标记物,在 37 °C 下孵育 30–45 分钟。
  7. 用平头针头将 0.5 µl 细胞悬液吸入注射器,并尽快完成注射。

3. 视网膜下注射(每次注射约5分钟)

注意:如果可能,使用成年白化大鼠学习该技术,因为其角膜缘血管更容易观察。在学习过程中(尝试注射细胞之前)注射快绿溶液,以便更清晰地观察注射部位。

  1. 对啮齿类动物进行麻醉。采用腹腔注射100 mg/ml的氯胺酮和10 mg/ml的赛拉嗪(20 µl/10 g体重),优于使用异氟烷吸入麻醉,因为动物在吸入麻醉状态下口鼻固定于吸入器中,难以操作并完成眼球注射。
    1. 通过轻捏动物的一只脚掌确认其是否已深度麻醉。若动物出现退缩反应,需等待数分钟后再次测试,直至确认深度麻醉后再开始视网膜下注射。
  2. 将动物侧卧放置,使待注射的眼朝向上方。
  3. 在解剖显微镜下,用手指轻轻固定动物头部(一手拇指和食指置于耳部上方,另一手固定下颌),沿眼睑方向平行轻拉皮肤,使眼球轻微突出于眼窝外(暂时性眼球突出),从而更易于操作(见图1C)。注意不要紧握动物咽喉部位。
  4. 使用30 G的一次性无菌锐利针头,在角膜缘正下方穿刺造孔(若刺中血管,将引起明显出血,并影响后续定位孔口);穿刺时保持一定角度,避免针头接触晶状体(见图1D)。切勿用锐利或钝针触碰晶状体,否则将立即导致白内障形成。
    注:注射操作由两人协作效果更佳。一人使用锐利针头完成穿刺后,可由另一人立即接过装有钝针的注射器进行注射,有助于持续聚焦并准确定位穿刺孔。
  5. 在保持头部固定的同时,将一次性锐利针头缓慢退出眼球。务必准确记住穿刺孔的位置。
  6. 将预装细胞悬液、连接钝针的注射器安装至显微操作仪,或手持操作,随后将钝针针尖经原穿刺孔插入眼内,再次注意避免接触晶状体,并轻柔地将针头缓慢推进至眼内,直至感到轻微阻力(见图1D)。
  7. 尽量减少任何移动,缓慢而谨慎地将RPE细胞注入视网膜下腔。
    注:注射将不可避免地引起RPE/视网膜脱离。然而,操作越精确,脱离范围越小,视网膜复位的成功率也显著提高(见图1E)。过度动作可能导致针头重新进入视网膜,侧向移动则可能损伤视网膜。使用注射泵为可选操作,但有助于实现极为精确的给药。
  8. 缓慢退出注射器。滴加眼表保湿液以维持眼球表面湿润。
  9. 持续监测动物状态,直至其恢复胸卧位。在动物未恢复胸卧位前,不得无人看管,也不得将其放回与其他清醒动物共处的笼中。

结果

利用本文所述的方法,我们可以快速且一致地将视网膜色素上皮(RPE)细胞悬液递送至啮齿类动物的视网膜下腔。虽然使用显微操作器并非必需,但采用图1A&B所示装置可显著减少操作创伤。如图1C所示固定啮齿类动物,以实现暂时性眼球突出。无论使用显微操作器还是手动操作,其步骤均相同,如图1D的示意图所示。若操作得当,可通过共聚焦扫描激光检眼镜(cSLO)检测到标记的RPE细胞发出的荧光,并可通过光学相干断层扫描(OCT)系统观察到诱导产生的视网膜脱离(图2)。在图3中,使用cSLO围绕整个注射部位采集了多幅图像(图2和图3的图像均在视网膜下注射后立即获取)。请注意,在中心图像(3-5)中视网膜脱离最为明显,但并不严重。

讨论
