通过16S rRNA基因测序与基质辅助激光解吸电离飞行时间质谱鉴定罕见细菌病原体

摘要

基质辅助激光解吸/电离飞行时间质谱技术(MALDI-TOF MS)与分子技术(16S rRNA 基因测序)可实现对常规诊断中罕见细菌病原体的鉴定。本方案的目标在于结合这两种技术,从而获得更准确、更可靠的数据。

摘要

存在多种罕见且因此描述尚不充分的细菌病原体,据报道可导致免疫功能低下患者发生严重感染。大多数情况下,仅有一些少量数据(多以病例报告形式发表)用于研究这些病原体作为感染因子的作用。因此,为了明确此类微生物的致病特性,有必要开展包含大量此类细菌的流行病学研究。此类监测研究中所采用的方法必须满足以下标准:菌株鉴定需符合现行命名法规且准确可靠,操作简便(具有稳健性),适用于常规诊断并具有经济性,且能够在不同实验室之间产生可比结果。通常,在常规检测中鉴定细菌菌株有三种策略:1)表型鉴定,通过表征细菌的生化与代谢特性进行鉴定;2)分子技术,如16S rRNA基因测序;3)基于蛋白质组学的新型方法——质谱分析。由于质谱分析和分子技术是鉴定多种细菌物种最具前景的工具,本文将重点描述这两种方法,并讨论其应用中的进展、局限性及潜在问题。

引言

在常规诊断中,罕见病原体的准确鉴定受到限制,原因是传统的培养和生化方法操作繁琐且有时结果不可靠。此外,临床微生物学实验室每天需要处理大量病原体,数量从数百到数千不等,因此必须依赖自动化系统。除了实现高通量日常检测外,还需精确鉴定细菌种类,因为不同菌种的抗菌药物敏感性模式存在差异,准确鉴定可为临床医生选择合适的抗生素提供关键信息(例如,Enterococcus spp.、Acinetobacter spp.)12,43

自动化微生物鉴定系统(aMIS)采用标准化的酶促反应组来表征细菌分离株的代谢特性 13,15,16,26,27尽管这些系统中使用的试剂盒采用了大量不同的生化反应, 例如, 本研究中所用aMIS的GN卡中的47 52,该策略仅能对有限种类的细菌实现可靠鉴定。此外,该数据库作为一个高级专家系统,明显侧重于检测具有医学重要性的相关及高度相关细菌 13,15,16,36另外两种在实验室中广泛使用的系统也采用这种生化方法进行细菌鉴定。近期研究表明,本研究中使用的aMIS与其中一种竞争系统在鉴定准确性方面相当(分别为93.7%和93.0%),而3rd aMIS在物种水平上的鉴定准确率仅为82.4% 35这些差异可能源于基础鉴定数据参考的质量、试剂盒和软件的版本、代谢差异以及技术人员的操作熟练程度 35,36.

目前主要使用两种自动化的基质辅助激光解吸电离飞行时间质谱系统(MALDI-TOF微生物鉴定系统,mMIS)。这些系统可根据微生物的蛋白质指纹图谱质谱实现大量细菌种类的检测。例如,本研究使用的mMIS数据库包含6,000个参考质谱。基于质谱的鉴定系统可快速、可靠地检测多种微生物,包括罕见病原体11,48,51。迄今为止,本研究中所用mMIS与其竞争产品之间的直接比较研究仍较少19,33。根据Daek et al. 的研究,两种系统均具有较高的鉴定准确率,但本研究中使用的mMIS在物种水平的鉴定上似乎更为可靠19

同样,针对高度保守但又具有差异性的基因(例如,16S rDNA 或 rpoB)的分子技术可实现明确的物种鉴定3,22,61。其中,16S rDNA 是最广泛应用的看家基因,因为它存在于所有细菌中34。其功能保持不变,且最终长度约为 1,500 bp,足够长,适合进行生物信息学分析14,34。许多研究人员将 16S rRNA 基因分析视为细菌鉴定的"金标准"21。这主要是因为目前仅有少数实验室仍采用 DNA-DNA 杂交技术用于鉴定罕见或新发细菌14,34。此外,越来越多可用于 16S rRNA 基因分析的数据库正在陆续建立50。然而,必须注意到,基于 16S rDNA 的检测系统在灵敏度上相较于标准 PCR 方法存在一定局限性。此外,该分子方法技术要求高、耗时较长,且需要经过高度培训的技术人员以及专用的实验设施,因此难以在常规诊断中广泛应用55。此外,已有研究表明,联合至少两种不同的细菌鉴定方法可显著提高菌株鉴定的准确性。结合 MALDI-TOF MS 与 16S rDNA 测序可高准确度地鉴定大量不同细菌物种。近期已有研究报道,将 MALDI-TOF MS 与 16S rRNA 基因分析相结合,用于流行病学问题及罕见病原体的研究与细菌鉴定56

方案

1. 细菌DNA的提取

  1. 磷酸盐缓冲液(PBS)溶液的配制
    1. 称取1.65 g Na2HPO4 x 2H2O、0.22 g NaH2PO4 x 2H2O 和 8.80 g NaCl 置于烧瓶中,加入蒸馏水定容至 1,000 ml,调节 pH 至 7.4。使用细菌过滤器(0.22 µm)过滤溶液,以供最终使用。
  2. 革兰氏阴性菌的DNA提取
    1. 将患者样本划线接种于合适的培养基(例如,哥伦比亚血琼脂),鉴定并分离潜在病原体。
    2. 制备纯培养物,并进行革兰氏染色,以确定菌体形态并确认培养物的纯度49
    3. 用1 µl一次性接种环挑取单个细菌菌落,转移至含有1 ml PBS溶液的无菌2.0 ml离心管中。
    4. 将该悬浮液在恒温混匀仪中95 °C孵育10分钟。孵育后,使细菌裂解液(含溶解的DNA)冷却至室温,可直接用于扩增,或于-20 °C保存以备后续使用(图1a)。
  3. 革兰氏阳性菌的DNA提取
    1. 将患者样本划线接种于合适的培养基(例如,哥伦比亚血琼脂),鉴定并分离潜在病原体。进行革兰氏染色以确认培养物的形态类型。
    2. 制备纯培养物,并进行革兰氏染色,以确定菌体形态并确认培养物的纯度。
    3. 用无菌的1 µl接种环挑取单个细菌菌落,悬浮于2.0 ml离心管中的500 µl PBS溶液中。
    4. 加入500 µl玻璃珠,将离心管置于振荡式匀浆器中,在最大频率和振幅(50 Hz)下处理5分钟。使用直径为1.0 mm的玻璃珠以破坏细菌细胞壁。
    5. 将该离心管在95 °C孵育10分钟,随后冷却至室温。提取物可直接用于PCR反应,或于-20 °C保存以备后续使用。

2. 16S rDNA PCR

  1. 扩增引物的制备
    1. 使用引物TPU1(5´-AGA GTT TGA TCM TGG CTC AG-3' [M = A/C])作为正向引物,RTU4(5´-TAC CAG GGT ATC TAA TCC TGT T-3´)作为反向引物25。用无DNA酶和RNA酶的水配制引物储备液,浓度为100 pmol/µl(100 µM),再稀释成10 pmol/µl的工作液。引物储备液和工作液可储存于-20 °C,或直接使用。
  2. PCR反应体系的配制
    1. 在无菌的200 µl反应管中,加入1 µl每种引物、1 µl dNTP混合液(100 mM)、2.5 µl DNA提取物、5 µl 10x PCR缓冲液(含25 mM MgCl2)、0.25 µl Taq DNA聚合酶(5单位/µl)以及39.25 µl无DNA酶和RNA酶的PCR级水。
  3. PCR程序
    1. 启动如下设置的PCR程序:首先,在95 °C预变性5分钟,以激活Taq DNA聚合酶;其次,进行35个扩增循环,每个循环包括95 °C变性1分钟、50 °C退火1分钟和72 °C延伸1.5分钟;第三,在72 °C进行最终延伸10分钟;最后,将PCR反应降温至4 °C。
      注意:Staphylococcus aureus 和 H2O 分别作为阳性和阴性对照(图1b)。
    2. 将含有PCR反应体系的反应管放入热循环仪中,启动程序。
  4. PCR产物的纯化
    注意:为纯化PCR产物,可采用多种方法,包括基于酶的处理或硅胶吸附基质法。此处使用的单步PCR纯化方法依赖于两种水解酶促反应:外切酶I(Exo I)可降解单链DNA,而虾碱性磷酸酶(SAP)则水解未掺入的dNTPs。另一种方法基于硅胶膜吸附技术,采用高盐快速结合—洗涤—低盐洗脱的流程。
    1. 向25 µl PCR产物中加入1.5 µl含有Exo I和SAP的酶混合液,充分混匀后转移至热循环仪,运行一个简单程序:先在37 °C孵育15分钟,再在80 °C孵育15分钟。纯化后的PCR产物可储存于-20 °C,或直接用于后续标记PCR反应。

3. DNA 琼脂糖凝胶电泳

  1. 电泳缓冲液(TBE缓冲液)的配制
    1. 将54.0 g(445 mM)TRIS碱、27.5 g(445 mM)硼酸和20 ml pH 8.0的0.5 M EDTA(10 mM)溶液置于烧瓶中,用NaOH滴定,并加入蒸馏水定容至1,000 ml。然后将此5倍浓度缓冲液用蒸馏水按1:10稀释,得到0.5x TBE缓冲液,用于电泳。
  2. 上样缓冲液的制备
    1. 将250 mg溴酚蓝、250 mg二甲苯青FF和15 g聚蔗糖混合材料表) 转移至一个烧瓶中,然后加入0.5x TBE缓冲液至总体积为100 ml。将上样染料分装为每份1 ml,于-20℃保存。 °C 以供后续使用。
  3. 琼脂糖凝胶的制备
    1. 称取6 g标准电泳琼脂糖加入300 ml 0.5x TBE缓冲液(见3.1.1节)于锥形瓶中,配制2%(w/v)琼脂糖凝胶。随后将琼脂糖混合液置于微波炉中加热至接近沸点,直至琼脂糖完全溶解,溶液呈透明状。
    2. 让熔化的琼脂糖充分冷却,然后加入10 µ1% (w/v) 溴化乙锭储备溶液。将溶液倒入制胶模具中,并插入一个可形成30个加样孔的梳子。待凝胶完全凝固后,取出梳子,将凝胶放入含有0.5×TBE缓冲液的电泳槽中。
      注意:溴化乙锭具有毒性和致突变性,因此应谨慎操作。建议佩戴腈类手套。存在其他可替代的嵌入性核酸染料,可作为无毒替代品使用。
  4. PCR产物的琼脂糖凝胶电泳
    注意:为了验证PCR扩增片段的正确大小并估算DNA浓度,需将纯化的PCR产物在琼脂糖凝胶中进行分离。
    1. 移液器 2 µ将8微升上样缓冲液加入PCR反应管中,并加入8 µPCR产物的量。将此混合液移入凝胶的加样孔中。加入8 µ在另一孔中加入分子量大小标记物(DNA Ladder),以估计PCR产物的大小。
    2. 施加10 V/cm的恒定电压,当溴酚蓝指示剂迁移至约 ¾ 凝胶总长度的部分。使用紫外透射仪(波长302 nm)观察分离的PCR片段,并用凝胶成像系统记录结果。通过与DNA Marker条带比较,目测估计DNA的量。标记PCR时,以约10 ng为目标用量。

4. Sanger 测序

  1. 循环测序 PCR
    注意:在10 µ使用商业试剂盒进行l反应(材料表).
    1. 加入2 µ5x 反应主混合液,2 μl µDNA 制备物的 1.5 微升 µTPU1 或 RTU4 工作溶液(10 pmol/μL)µ升)和4.5 µ无DNA酶和RNA酶的水。
    2. 将此测序反应混合物放入热循环仪中,并进行另一轮PCR。总共进行25个循环,每个循环包括变性步骤(96℃) °C,10秒),退火步骤(45-60 °C,5秒)和一个延伸步骤(60 °C,2 分钟)。最后,将反应冷却至 4 °C.
  2. 测序反应的纯化
    1. 使用市售的PCR纯化离心柱纯化标记反应混合物(材料表).
    2. 加入10 µ将测序反应液加入预水化的凝胶过滤介质中,并在750 × g下离心3分钟。
      注意:通过使用此方法,未掺入的染料终止物将保留在凝胶基质中。
    3. 然后在真空浓缩仪中将水相洗脱液干燥。在40℃下以2,500 × g离心20至30分钟 °C,直至完全干燥。
    4. 加入10 µl 高度去离子甲酰胺,然后在 90 ℃ 变性 °C 反应 2 分钟,然后将混合物置于冰上冷却。用移液器吸取 10 µ变性样品在96孔板上的处理 µl 板。将干燥并纯化的 PCR 产物于 -20 保存 °如果分析需在后续时间进行。
  3. 16S rRNA基因测序及DNA序列分析
    注意:在自动测序仪上进行序列分析(材料表)。此处使用的测序仪是一种基于荧光的自动化毛细管电泳系统,可同时分析4个样本(50 cm 4毛细管阵列),并使用液体聚合物 67 (图1c).
    1. 将微量滴定板放入测序仪中。按照规定格式填写样品表(板记录) 2,保存并按照所提供的步骤开始测序运行/分析 1.
      注意:一次测序运行时长为2小时,可提供800至1,000 bp的数据。
    2. 打开测序分析软件(材料表序列数据以文本文件(.seq)和包含带有质量值(QV)的电泳图谱文件(.ab1)提供。
      注意:碱基识别由测序分析软件自动完成,并通过目视检查其相应的电泳图谱例如峰高、峰分离度)在该序列用于进一步应用之前。
    3. 使用BLAST算法将储存的DNA序列文件与NCBI nr数据库进行比对(http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?db=Nucleotide)。

5. 基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)

注意:质谱仪使用337 nm的N2激光,并由特定控制软件操作(材料表)。谱图以线性模式记录,覆盖2,000至20,000 Da的质量范围。数据的解析及对样品的评分由分析软件实时完成(材料表)(图1d)。

  1. 用于 MALDI-TOF 质谱分析的细菌制备
    1. 培养目标细菌(例如, Myroides odoratimimus) 在含5%马血的哥伦比亚血琼脂上,37℃培养 °C 培养 18 至 24 小时,具体时间取决于细菌菌株(图2a进行革兰氏染色,以验证待研究生物体的形态类型及培养物的纯度。
    2. 使用木质牙签将单个细菌菌落转移至96孔钢制靶板的一个孔中图2b 2c。点样 1 µ在钢靶上干燥的生物样本上加入70%甲酸,静置干燥约2-3分钟。
    3. 然后,向该点覆盖1 µl 矩阵溶液,含有 50 mg/ml CHCA(α-氰基-4-羟基肉桂酸)溶于有机溶剂(50% 乙腈,2.5% 三氟乙酸,47.5% H2O
      注意:可采用酸覆盖法(平板制备法)以提高质谱质量。已有研究表明,对于革兰氏阳性菌,该方法可有效改善检测结果。 "酸性攻击" 提高所测光谱的评分值 45.
      注意:也可使用自动化样品制备系统(材料表) 可用于实现无接触的点样,其中无接触的点样区域为 1 µl 70% 甲酸溶液,经过第一个干燥循环后,1 µ将l基质溶液施加于细菌涂片上。随后,样品在60%相对湿度的标准条件下干燥,即可用于分析。
    4. 将已覆盖基质的涂片在通风橱中再次风干约5分钟。涂布基质后的细菌涂片现已稳定,可保持数小时。
      注意:由于蛋白质降解,不含基质的细菌涂片可能不稳定。
  2. 进行 MALDI-TOF 质谱分析
    1. 样品的引入
      1. 打开我们mMIS的控制软件材料表).
      2. 按下仪器的 IN/OUT 按钮,对质谱仪的样品加载口进行通气。
      3. 听到咔嗒声后打开加样口,插入已带有干燥细菌涂片和基质的MALDI-TOF MS靶板。
      4. 关闭端口并再次抽真空。观察真空度,当达到 4.5 x 10-6 mbar 开始测量。
    2. 样品分析
      1. 打开我们mMIS的分析软件材料表)并点击 "文件" 菜单。选择 "新分类" 和一个新窗口 "MALDI 生物鉴定仪实时分类向导" 打开。输入项目名称,例如 "黏液杆菌测量项目_1,现场进行 "项目名称" 并按下按钮 "新". 在名为的新对话框下 "新项目" 检查项目名称,然后按按钮继续 "好的".
      2. 在 "MALDI 生物鉴定仪实时分类向导" 观察 "分析物放置" 窗口打开。选择目标位置(例如A1, A2, A3…),右键单击任意选定位置(以蓝色方块标示),然后选择 "添加样品"在自动打开的表格视图中输入样品名称、样品编号,以及可选的备注。点击按钮 "下一步" 继续进行。
      3. 在 "MALDI生物鉴定仪实时分类向导" 观察 "MALDI生物鉴定仪方法的选择" 窗口打开。选择 "布鲁克分类法" 从 "分类树中的 MSPs" 并点击 "下一步" 继续。
      4. 在 "MALDI 生物鉴定仪实时分类向导" 观察 "项目概述" 窗口已打开。请核对上述输入的条目以确认当前分类项目。点击按钮开始分类运行 "完成" (图 2d-f)。当达到适当的真空度时,测量将自动开始。
      5. 按照分类运行直至测量成功完成。在结果表中查看结果 "MALDI Biotyper 实时分类项目 Myroides 测量项目_1" 打开菜单 "查看" 并点击 "结果".
      6. 查看 "布鲁克道尔顿 MALDI 生物指纹分类结果" 在默认的网络浏览器中以 HTML 文件形式打开。打印并保存结果。
    3. 每天在样品分析前测量测试标准品以校准仪器,并每周进行一次仪器验证(仪器自检)。
      注意:测量过程中,MALDI-TOF 谱图由分析软件实时分析材料表给出包含十个物种及其相应评分的列表,并输入实验室信息系统(LIS)用于报告。
    4. 批判性地评估 MALDI-TOF MS 结果(参见代表性结果),并根据需要结合其他检测方法,如生化特性分析、抗生素敏感性谱型分析或 16S rRNA 基因测序,必要时进行革兰染色,以确定细菌种属并纳入微生物学报告。

结果

基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)是一种新颖、快速且经济的微生物常规诊断方法。通过MALDI-TOF MS进行细菌种属鉴定所获得的谱图主要由核糖体蛋白组成,同时也包含其他“在功能上高度保守、受环境条件影响极小的管家蛋白”17。该微生物鉴定系统(mMIS)的数据库包含大量参考谱图,甚至能够可靠地鉴定临床上罕见的细菌分离株7,56,57。评分值用于反映鉴定结果的可靠性:评分高于2.300表示极有可能的种属鉴定;评分在2.000至2.300之间表示可靠的种属鉴定;评分在1.700至2.000之间表示可能的种属鉴定;而低于1.700的评分则不可靠。当出现多个种属的评分均高于2.000时,需结合其他附加检测方法,这也是我们联合使用多种技术的原因,例如16S rDNA测序、API系统以及针对细菌形态和/或表型特征的技术,如革兰染色、运动性检测等etc.。当评分低于1.700时,也必须采用上述方法以获得可靠的种属鉴定结果。近期已有研究展示了MALDI-TOF MS与16S rDNA测序相结合的应用<...

讨论

基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)和16S rRNA基因测序均可用于鉴定大量不同种类的细菌。MALDI-TOF MS是一种快速、经济且操作简便的方法,且已有大量细菌质谱数据库可供使用。因此,MALDI-TOF MS是一种快速、经济高效且可靠的筛查方法,适用于以罕见细菌病原体为重点的研究17,20,39,51。在一项比较MALDI-TOF MS与其他表型鉴定方法的前瞻性研究中,Seng et al. 证实了MALDI-TOF MS在成本效益和速度方面的优势59,Tan et al. 报道其在实验室环境中每年可将细菌鉴定所需的试剂和人力成本降低56.9%62。Gaillot et al. 的研究也支持了此类成本的降低28
