迄今为止,多种处于不同发育阶段的干细胞已成为治疗退行性疾病的研究焦点。然而,某些因素(如移植后初期的大规模细胞死亡和治疗效果较低)限制了其在临床上的广泛应用。移植前对干细胞进行基因工程改造已成为优化干细胞治疗效果的一种有前景的方法。但目前仍缺乏安全且高效的基因递送系统。因此,开发合适的方法可能为解决基于干细胞治疗的现有挑战提供有效途径。

人牙囊是一种疏松的、来源于外间充质的结缔组织,包绕在发育中的牙齿周围1,2。除了在牙齿萌出过程中协调破骨细胞生成和成骨作用的功能外,该组织还含有干细胞和前体细胞,尤其参与牙周组织的发育3,4,5。因此,牙囊被视为获取人成体干细胞的替代来源之一6,7

多项研究表明,人牙囊干细胞(hDFSCs)能够分化为牙周组织谱系细胞,包括成骨细胞、牙周膜成纤维细胞和成牙骨质细胞。8,9,10此外,这些细胞被证实具有间充质基质细胞(MSCs)的全部特征,包括自我更新能力、贴壁生长特性以及特定表面标志物的表达(如, CD73、CD90、CD105)以及成骨、成脂和成软骨分化潜能11,12,13其他研究也揭示了人牙囊干细胞(hDFSCs)具有神经分化潜能2,14,15,16,17,18.

由于其具有广阔的应用前景且易于获取,人牙 follicle 间充质干细胞(hDFSCs)近年来在组织工程领域受到关注19,20,21早期研究主要集中在DFSCs再生骨组织、牙周组织和牙根的潜力19,22,23,24,25,26,27,28,29,30自从发现人牙 follicle 干细胞(hDFSCs)具有神经发生潜能以来,其作为神经退行性疾病潜在治疗方法的应用已受到广泛研究31,32,33。HDFSCs 在其他组织的再生方面也日益受到重视(例如, 角膜上皮34,35人牙 follicle 间充质干细胞(hDFSC)的治疗潜力不仅源于其直接分化能力,还与其旁分泌活性密切相关。近期研究表明,hDFSC 能够分泌多种生物活性因子,如基质金属蛋白酶(MMPs)、胰岛素样生长因子(IGF)、血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)和肝细胞生长因子(HGF),这些因子在血管生成、免疫调节、细胞外基质重塑及修复过程中发挥关键作用。36.

然而,干细胞治疗的广泛临床转化仍受到多种挑战的限制,例如大量初始细胞死亡以及干细胞有益效应较低37,38。基因工程为此提供了有前景的应对策略,因而可显著增强干细胞的治疗效果38,39,40。对于短暂的细胞调控,微小RNA(miRs)是合适的候选分子,因为这些小的转录后调控因子能够在不引起基因组稳定整合风险的情况下调控干细胞的命运和行为41,42,43。迄今为止,已鉴定出多种具有益作用的miRs,可促进干细胞的增殖、存活、归巢、旁分泌活性以及向多个谱系的分化44。例如,经miR-133a基因工程修饰的间充质干细胞在梗死大鼠心脏中表现出更高的存活率和植入率,与未修饰的间充质干细胞相比,可改善心脏功能45。同样,过表达miR-146a的间充质干细胞被证实可分泌更高水平的VEGF,从而在缺血组织中实现更强的治疗效果46

本文提供了一种人牙囊来源干细胞(hDFSCs)的选择性提取与基因工程改造的详细实验方案。为此,我们描述了人牙囊组织的获取与酶消化过程,以及后续hDFSCs的分离方法。为表征所分离的细胞,本文依据国际细胞治疗学会(International Society for Cellular Therapy)的指南13,包含了验证间充质干细胞(MSC)特性的关键操作步骤。此外,我们还详细介绍了采用阳离子脂质介导的转染策略生成miR修饰的hDFSCs的方法,并对转染效率与细胞毒性进行了评估。

方案

HDFSCs 从罗斯托克大学医学中心口腔颌面整形外科提供的拔除智齿的牙囊中分离获得。所有患者均签署了知情同意书并获得书面批准。本研究已获得罗斯托克大学当地伦理委员会的批准(许可编号:A 2017-0158)。

1. hDFSCs 的分离

注意:为防止细菌污染,智齿在拔除前不应已萌出

  1. 所需溶液的配制
    1. 配制磷酸盐缓冲液(PBS)/青霉素-链霉素-谷氨酰胺(P-S-G)溶液:将495 mL PBS与5 mL P-S-G溶液混合。分装成50 mL等份,于4 °C保存。
    2. 配制hDFSC培养基:将445 mL基础培养基与5 mL抗生素试剂和50 mL胎牛血清(FBS)混合。4 °C保存。
    3. 配制I型胶原酶储备液(30 mg/mL):将300 mg I型胶原酶溶于10 mL含1%抗生素试剂的基础培养基中。剧烈混匀。使用0.2 µm滤器过滤溶液。将I型胶原酶储备液分装为500 µL等份,于-20 °C保存。
    4. 配制Dispase II储备液(40 mg/mL):将40 mg Dispase II溶于10 mL含1%抗生素试剂的基础培养基中。剧烈混匀。使用0.2 µm滤器过滤溶液。将Dispase II储备液分装为500 µL等份,于-20 °C保存。
  2. 手术操作
    1. 注射足量局部麻醉剂(最大剂量:2 mL)。
    2. 制作并翻起三边黏骨膜瓣。无需翻起舌侧瓣。
    3. 使用骨膜剥离器保护舌侧组织。
    4. 使用圆形硬质金属钻头轻柔磨除颊侧和远中骨组织。
    5. 用骨膜剥离器松解滤泡组织,使用后牙(第三磨牙)拔牙钳夹住牙冠(及滤泡)小心拔出完整的牙齿(牙胚)及滤泡。
    6. 用NaCl溶液彻底清创并冲洗拔牙窝。
    7. 使用Vicryl缝线复位黏骨膜瓣(见材料表)。
    8. 将牙滤泡从口腔中取出,并放入50 mL PBS/P-S-G溶液的等份管中。
      注意:样品可在4 °C保存,直至进一步处理。
  3. 牙滤泡的酶消化
    1. 将hDFSC培养基和PBS/P-S-G溶液预热至室温(RT)。
    2. 解冻一份I型胶原酶和Dispase II储备液。取500 µL I型胶原酶储备液和500 µL Dispase II储备液,加入含1%抗生素试剂的4 mL基础培养基中,配制成含3 mg/mL I型胶原酶和4 mg/mL Dispase II的消化液。
    3. 将提取的滤泡置于无菌培养皿中,加入10 mL PBS/P-S-G溶液清洗组织。重复清洗步骤两次。
    4. 在含有10 mL PBS/P-S-G溶液的培养皿中,用无菌手术刀将提取的滤泡切成约1 mm × 1 mm的小块。
    5. 将剪碎的组织及PBS/P-S-G溶液从培养皿转移至50 mL锥形离心管中。用10 mL PBS/P-S-G溶液冲洗培养皿,并将冲洗液转移至同一离心管中。
    6. 在室温下以353 × g离心10分钟,弃去上清液。
    7. 向沉淀组织中加入5 mL消化液,轻轻混匀。将混合物置于37 °C、5% CO2条件下,在摇床培养箱中孵育2小时。
    8. 在室温下以353 × g离心消化后的细胞/组织悬液10分钟,弃去上清液,将沉淀重悬于6 mL hDFSC培养基中。
    9. 将细胞悬液接种至25 cm2细胞培养瓶中,于37 °C、5% CO2和20% O2条件下培养。
      注意:若组织未完全消化,也将残留组织一并转移至细胞培养瓶中。
    10. 细胞接种24小时后小心更换培养基。此后每三天更换一次培养基,直至细胞融合。
      注意:hDFSC应在细胞接种24小时后贴壁生长,可通过更换培养基的方式将其与非贴壁细胞及血液成分分离。
  4. 细胞收获
    1. 将hDFSC培养基、PBS/P-S-G溶液及胰蛋白酶/乙二胺四乙酸(EDTA)溶液预热至室温。
    2. 弃去培养瓶中的上清液,用5 mL PBS/P-S-G溶液洗涤融合的细胞。
    3. 向培养瓶中加入1 mL胰蛋白酶/EDTA溶液,于37 °C孵育3分钟。加入3 mL hDFSC培养基终止消化反应。
    4. 将细胞悬液转移至15 mL锥形离心管中,在室温下以353 × g离心10分钟。弃去上清液,将沉淀重悬于适量hDFSC培养基中。
    5. 细胞计数:将10 µL细胞悬液与10 µL台盼蓝溶液轻轻混匀。取10 µL加入计数板,计算hDFSC细胞数量。

2. hDFSCs 的表征

  1. 免疫表型分析
    1. 流式细胞术分析用细胞的制备
      1. 所需溶液的配制
        1. 配制 PBS/EDTA(2 mM):将 996 mL PBS 与 4 mL EDTA(0.5 M)混合。
        2. 配制染色缓冲液:将 995 mL PBS/EDTA(2 mM)与 5 g BSA 混合,使用 0.22 µm 滤器过滤后,于 4 °C 保存备用。
        3. 配制多聚甲醛(PFA)储备液(4%):将 4 g PFA 溶于 100 mL PBS 中,并将溶液加热至 80 °C,搅拌溶解后调节 pH 值至 7.3。将所得 PFA 溶液分装为适当体积(1.5 mL),于 -20 °C 保存备用。
          警告:PFA 蒸气具有毒性,应在通风橱内配制 PFA 溶液。
      2. 细胞收获后(1.4),将 14 个样本各含 5 × 104 个细胞转移至 1.5 mL 微量离心管中,在 4 °C 条件下以 300 × g 离心 10 分钟,弃去上清液。
        注意:以下操作应在避光环境中进行。除非另有说明,细胞和试剂均需置于冰上保存。
      3. 用适量染色缓冲液(4 °C)重悬细胞,并根据 表 1 所示加入 FcR 封闭试剂(4 °C)。
      4. 根据 表 1 所示,将以下抗体分别加入相应样本管内壁:别藻蓝蛋白(APC)标记的小鼠抗人 CD29 抗体;APC 标记的小鼠 IgG1 κ 同型对照;叶绿素蛋白-花青素 5.5(PerCP-Cyanine5.5)标记的小鼠抗人 CD44 抗体;PerCP-Cyanine5.5 标记的小鼠 IgG2b κ 同型对照;V500 标记的小鼠抗人 CD45 抗体;V500 标记的小鼠 IgG1 κ 同型对照;藻红蛋白(PE)标记的小鼠抗人 CD73 抗体;PE 标记的小鼠 IgG1 κ 同型对照;PerCP-Cyanine5.5 标记的小鼠抗人 CD90 抗体;PerCP-Cyanine5.5 标记的小鼠 IgG1 κ 同型对照;小鼠抗人 CD105 抗体:Alexa Fluor(AF)488 标记;小鼠 IgG1 阴性对照:AF488 标记;PE-Cyanine7 标记的小鼠抗人 CD117 抗体;PE-Cyanine7 标记的小鼠 IgG1, κ 同型对照。每管加完抗体后,短暂离心以收集液滴,轻轻混匀,于 4 °C 孵育 10 分钟。
      5. 加入 1 mL PBS(4 °C),在 4 °C 条件下以 300 × g 离心 10 分钟,弃去上清液。
      6. 用 100 µL PBS(4 °C)重悬细胞,加入 33 µL PFA(4%),混匀后置于冰上或 4 °C 保存,直至进行流式细胞术分析。
    2. 细胞的流式细胞术检测
      注意:以下操作应在避光环境中进行。检测前细胞应始终保持在冰上。
      1. 为检测表面抗原的表达情况,将样本转移至适用于流式细胞术检测的试管中。
      2. 使用流式细胞仪检测至少 2 × 104 个事件,并按照 图 2 所示进行分析。
  2. 人牙龈来源间充质干细胞(hDFSCs)的多向分化潜能
    1. 使用市售的人间充质干细胞功能鉴定试剂盒,验证 hDFSCs 的成脂、成骨和成软骨分化潜能。使用驴抗山羊 AF488 二抗对脂肪酸结合蛋白 4(FABP4)和聚集蛋白聚糖(aggrecan)进行染色,使用驴抗小鼠 AF488 二抗对骨钙素(osteocalcin)进行染色。细胞核染色使用含 4',6-二脒基-2-苯基吲哚(DAPI)的封片剂。
      注意:显微镜分析时,应设置不加一抗的样本作为阴性对照。
    2. 采用激光共聚焦扫描显微镜分析蛋白表达。显微镜设置:40 倍油镜;激发激光波长为 488 nm(用于 AF488)和 405 nm(用于 DAPI)。

3. hDFSCs 的转染

  1. 细胞收获后(1.4),在转染前24小时将hDFSCs接种于24孔细胞培养板中。
    注意:起始细胞密度约为每孔4 × 104个细胞。转染当天细胞应达到约80%融合度。
    注意:额外接种一份细胞样本作为流式细胞术分析的对照。
  2. 转染复合物的制备
    注意:为防止RNase污染,使用RNase去污染溶液在制备转染复合物前彻底清洁操作区域。仅使用无RNase的材料和溶液。
    注意:以下操作应在避光环境中进行。
    1. 配制miR储存液(50 µM):将Cy3标记的miR前体(5 nmol)溶于100 µL无核酸酶水中。将所得miR储存液分装成若干份(每份5 µL),避光保存于-20 °C,待用。
    2. 将40 pmol miR(50 µM miR储存液0.8 µL)溶于66.7 µL低血清培养基中,轻轻混匀。
    3. 将0.67 µL阳离子脂质类转染试剂溶于66.7 µL低血清培养基中,轻轻混匀,室温孵育5分钟。
    4. 孵育后,将预稀释的转染试剂加入预稀释的miR溶液中,轻轻混匀,室温孵育15分钟。
  3. 将制备好的转染复合物逐滴加入细胞培养孔中的培养基内,通过前后轻轻摇动24孔板使溶液混匀。
  4. 将细胞置于37 °C、5% CO2、20% O2条件下孵育24小时。

4. 转染效果分析

注意:请在光线较暗的室内进行以下操作。

  1. 转染后细胞收集
    注意:将同一样本的所有细胞溶液收集至同一个15 mL锥形离心管中。
    1. 转染24小时后,将各样本的上清液收集至相应的离心管中。
    2. 用1 mL PBS洗涤细胞,将PBS转移至相应的离心管中,并向细胞中加入500 µL胰蛋白酶/EDTA溶液(室温)。在37 °C孵育3分钟。
    3. 向细胞中加入1 mL培养基(室温)以终止消化,将溶液转移至相应的离心管中。在4 °C、300 × g条件下离心10分钟。
      注意:从此时起,除非另有说明,始终将细胞和试剂置于冰上。
  2. 流式细胞术分析前的细胞准备
    1. 弃去上清液。将细胞重悬于100 µL染色缓冲液(4 °C)中,并将细胞悬液转移至1.5 mL微量离心管中。
    2. 向样本中加入0.5 µL胺反应性染料,以区分活细胞与死细胞。轻轻混匀,于4 °C孵育10分钟。
    3. 向细胞中加入1 mL PBS(4 °C),在4 °C、300 × g条件下离心10分钟。弃去上清液。
    4. 将细胞重悬于100 µL PBS(4 °C)中,加入33 µL 4%多聚甲醛(PFA)。混匀后置于冰上或4 °C保存,直至进行流式细胞术分析。
  3. 细胞的流式细胞术检测
    注意:以下操作应在避光环境中进行。检测前始终保持细胞处于冰上。
    1. 将样本转移至适用于流式细胞术检测的试管中。
    2. 使用流式细胞仪检测细胞活力及miR摄取效率。至少检测2 × 104个事件。采用图4所示的设门策略。
      注意:以未转染细胞作为阴性对照,用于设定Cy3阳性细胞的设门范围,并计算转染导致的细胞死亡率。
