大鼠I型复杂性区域疼痛综合征的慢性缺血后疼痛模型

摘要

本文提供了一种建立慢性缺血后疼痛(CPIP)动物模型的实验方案。该模型是模拟人类Ⅰ型复杂性区域疼痛综合征的公认模型。进一步评估CPIP大鼠模型中的机械性和热性痛觉过敏,以及辣椒素诱导的伤害性行为。

摘要

复杂性区域疼痛综合征Ⅰ型(CRPS-I)是一种神经系统疾病,可导致患者出现剧烈疼痛,目前仍属于尚未解决的医学难题。然而,CRPS-I的潜在发病机制尚未被阐明。已知缺血/再灌注是导致CRPS-I的主要因素之一。通过延长后肢缺血及再灌注过程,已建立大鼠慢性缺血后疼痛(CPIP)模型以模拟CRPS-I。CPIP模型已成为研究CRPS-I机制的公认动物模型。本实验方案详细描述了建立大鼠CPIP模型的具体操作步骤,包括麻醉以及后肢缺血/再灌注的实施过程。通过检测后肢的机械性与热性痛觉过敏程度,以及对急性辣椒素注射产生的伤害性反应,进一步评估该CPIP模型的特征表现。大鼠CPIP模型表现出多种类似CRPS-I的症状,包括建模后早期出现的后肢水肿和充血、持续存在的热痛与机械痛觉过敏,以及对急性辣椒素注射引发的伤害性反应增强。这些特征使其成为深入研究CRPS-I相关机制的适宜动物模型。

引言

复杂性区域疼痛综合征(CRPS)是由骨折、创伤、手术、缺血或神经损伤引起的复杂且慢性的疼痛症状1,2,3。CRPS分为两个亚型:I型和II型(CRPS-I和CRPS-II)4。流行病学研究表明,CRPS的患病率约为1:20005。CRPS-I无明显神经损伤表现,但可导致慢性疼痛,并显著影响患者的生活质量。目前可用的治疗方法疗效有限,因此CRPS-I仍然是一个亟待解决的重要且具有挑战性的临床问题。

建立模拟复杂性区域疼痛综合征-I(CRPS-I)的临床前动物模型,对于探索CRPS-I的潜在机制至关重要。为解决这一问题,Coderre等人设计了一种大鼠模型,通过对后肢施加 prolonged ischemia(缺血)和再灌注来模拟CRPS-I6。已知缺血/再灌注损伤是导致CRPS-I的主要原因之一7。该大鼠CPIP模型表现出多种类似CRPS-I的症状,包括建模后早期阶段出现的后肢水肿和充血,随后出现持续性的热痛和机械性痛觉过敏6。借助此模型,研究提出中枢痛觉敏化、外周TRPA1通道激活以及活性氧物质的生成等因素均参与了CRPS-I的发生发展8,9,10。我们近期成功建立了CPIP大鼠模型,并对支配患侧后足的背根神经节(DRGs)进行了RNA测序分析11。研究发现了一些可能参与介导CRPS-I痛觉过敏的潜在机制11。我们进一步鉴定出背根神经节神经元中的瞬时受体电位香草酸1(TRPV1)通道在CRPS-I的机械性和热性痛觉过敏中发挥重要作用12

在本研究中,我们描述了建立CPIP大鼠模型所涉及的详细步骤。我们通过检测机械性及热痛觉过敏程度,以及对急性辣椒素刺激的反应性,进一步评估了该CPIP大鼠模型的有效性。我们提出,CPIP大鼠模型可作为研究CRPS-I相关机制的一种可靠动物模型。

方案

动物实验方案已获浙江中医药大学动物伦理委员会批准。

1. 动物

  1. 从上海实验动物中心获取雄性Sprague-Dawley(SD)大鼠(体重280–320 g,8–10周龄),饲养于浙江中医药大学实验动物中心。饲养条件应保持12 h/12 h明暗循环,温度恒定在24 °C。自由供应水和饲料 随意摄取注意,本研究共使用48只大鼠。需要说明的是,在该模型中,需在整个过程中观察疼痛反应。若使用镇痛药物,则无法成功建立疼痛模型。实验结束后,处死大鼠。

2. CPIP 模型建立

  1. 使用戊巴比妥钠(50 mg/kg,腹腔注射 [i.p.])对所有大鼠(包括假手术组和CPIP模型组)进行麻醉。必要时以戊巴比妥钠(最多20 mg/kg/h,i.p.)维持麻醉。使用镊子夹捏大鼠后肢足垫或尾尖,检查每只动物的反射反应。确保大鼠在建模前无反应。术中在大鼠眼部涂抹兽用眼膏,防止角膜干燥。将麻醉后的大鼠置于维持在37 °C的加热垫上,进行后续操作。
  2. 后肢足部缺血与再灌注
    1. 大鼠麻醉后,用甘油润滑其右后肢足部和踝关节。
    2. 将一个内径为7/32"(5.5 mm)的腈类70邵氏硬度O型环滑入1.5 mL Eppendorf管较大的一端(使用前将管盖切除)。小心地将后肢足部插入中空的Eppendorf管,直至到达管底。
    3. 将O型环从Eppendorf管逐渐滑移到右后肢踝关节附近,并保持3小时。对假手术组大鼠进行相同操作,但使用已切断的O型环(无法造成缺血)套于踝部。
    4. 缺血处理3小时后剪除O型环。密切观察大鼠,直至其恢复足够意识以维持胸卧位。注意接受麻醉的大鼠在完全恢复前不得与其他大鼠合笼饲养。

3. 伤害性行为测试

  1. 将大鼠置于一个透明的聚甲基丙烯酸甲酯(Plexiglas)箱内,箱体底部为网格结构。在进行任何行为学测试前,让大鼠适应环境0.5小时。
  2. 机械性异常性疼痛
    1. 使用von Frey纤维丝(0.4、0.6、1.0、2.0、4.0、6.0、8.0、15.0和26.0 g)进行测试。从中间强度的纤维丝(4.0 g)开始,将其垂直作用于后肢足底中部区域,施加适当力度使纤维丝轻微弯曲,持续时间最长为5秒。若后肢出现突然回缩反应,则视为伤害性行为。在第-3、-2、-1天以及之后每隔一天进行一次机械性异常性疼痛测试,直至第13天。
    2. 采用上下法(up-down testing method)测定疼痛阈值,并使用Dixon法计算50%足底回缩阈值(PWT)13,14,15
  3. 热痛觉过敏
    1. 采用Hargreaves法检测热痛觉过敏。将灯泡(50 W)发出的光束直接对准后肢足底,测量足底回缩潜伏期(PWL)。设定20秒为截断时间,以避免加热造成过度损伤。
    2. 对每侧后肢在5分钟间隔内重复测试3次,取三次结果的平均值作为该大鼠的PWL16。在第-3、-2、-1天以及之后每隔一天进行一次热痛觉过敏测试,直至第13天。
  4. 辣椒素诱导的急性伤害性行为
    1. 使用二甲基亚砜(DMSO)配制辣椒素储备液(200 mM),再用无菌磷酸盐缓冲液(PBS)按1:1000稀释后用于后肢足底注射。PBS中最终DMSO浓度为0.1%(对照溶剂为仅含0.1% DMSO的PBS)。使用连接1 mL注射器的30 G针头,向后肢足底(足垫内注射)注入50 μL辣椒素溶液或对照溶剂。
    2. 注射后立即用摄像机记录10分钟内的伤害性行为(如舔舐、咬噬或抽动注射侧足部),并按照先前描述的方法进行量化分析17,18,19
  5. 后肢足部水肿评估:通过测量足部直径的增加程度来评估后肢足部水肿。使用数显卡尺测量,并计算不同时间点测得值与基础值之间的差值。在造模后15分钟、24小时、48小时和72小时评估足部厚度的变化。

结果

将O形环套在踝部后,同侧后肢足垫皮肤出现发绀,提示组织缺氧(图1A)。剪断O形环后,同侧后肢足垫开始充血并出现明显肿胀,表现出显著的充血征象(图1A)。足垫肿胀逐渐减轻,并在缺血/再灌注操作后48小时恢复至正常水平(双因素方差分析,Sidak事后检验,图1B)。上述所有表现均与既往研究结果一致6,12

随后,采用 von Frey 纤毛试验测定机械性异常性疼痛。与假手术组相比,CPIP 组同侧后肢足底在造模后第 1 天即出现明显的机械性异常性疼痛,并持续至观察期第 13 天(双因素方差分析,Sidak 事后检验,图 1
