引言

人体肠道微生态系统极为复杂,一个健康个体的肠道内含有超过1000种细菌1。肠道菌群参与维持肠道正常生理功能及免疫应答,与人体具有密不可分的关系。越来越多的证据表明,肠道微生物组构成了人体最后一个器官,是人体的一部分,而不仅仅是一群寄生生物2。在生命早期建立的肠道微生物群、其代谢产物与宿主免疫系统之间的“健康”共生关系,对维持肠道稳态至关重要。在某些异常情况下,例如慢性炎症,机体内外环境的变化会严重破坏肠道稳态,导致肠道微生物群落持续失衡,这种状态被称为菌群失调(dysbiosis)3。事实上,暴露于多种环境因素,包括饮食、药物和病原体,均可引起微生物群的变化。

菌群失调与多种肠道疾病的发生机制相关,例如炎症性肠病(IBD)、肠易激综合征(IBS)和假膜性肠炎,同时也与越来越多的肠外疾病相关,包括心血管疾病、肥胖和过敏4。微生物群谱分析显示,IBD 患者的细菌多样性显著降低,并且某些特定菌株的种群数量发生明显改变5,6。这些研究发现,IBD 患者体内Lachnospiraceae和Bacteroidetes减少,而Proteobacteria和Actinobacteria增多。目前认为,IBD 的发病机制与多种致病因素有关,包括肠道菌群异常、免疫应答失调、环境刺激以及遗传变异7。大量证据表明,肠道细菌在 IBD 的启动和进展阶段均发挥作用8,9,提示纠正肠道菌群失调可能为 IBD 的治疗和/或维持治疗提供一种新策略。

粪便微生物群移植(FMT)的原型始于古代中国101958年,Eiseman博士及其同事通过灌肠方式,成功使用健康供体的粪便治疗了四例严重的假膜性肠炎,开启了现代西方医学中以人粪治疗人类疾病的新篇章11. 艰难梭菌 感染(CDI)已被发现是假膜性肠炎的主要病因12 粪菌移植(FMT)在治疗艰难梭菌感染(CDI)方面具有显著疗效。在过去八年中,FMT已成为治疗复发性CDI的标准疗法。13,促使人们进一步研究FMT在其他疾病(如炎症性肠病)中的作用。在过去二十年中,大量病例报告和队列研究记录了FMT在炎症性肠病患者中的应用14一项荟萃分析纳入了12项试验,结果显示,62%的克罗恩病(CD)患者在接受粪便微生物移植(FMT)后达到临床缓解,69%的CD患者出现临床应答。15尽管这些发现令人鼓舞,粪菌移植(FMT)在炎症性肠病(IBD)治疗中的作用仍不明确,且FMT缓解肠道炎症的机制尚未被充分阐明。在FMT能够成为临床IBD治疗常规手段之前,仍需进一步研究。

在本实验方案中,我们对IL-10-/-小鼠进行了粪菌移植(FMT),该小鼠在断奶后会自发性地发展出结肠炎,已被广泛用作模拟炎症性肠病(IBD)多因素特性的金标准模型16,17,18。 IL-10−/−小鼠被广泛用于研究IBD的病因,因其表现出与IBD患者相似的分子和组织病理学特征,并且与患者类似,其疾病可通过抗TNFα治疗得到改善16。与同龄野生型小鼠相比,老龄IL-10−/−小鼠(>9月龄)心脏体积增大且心功能受损19,因此是研究结肠炎诱导性心脏病的理想模型。然而,其他结肠炎小鼠模型,如葡聚糖硫酸钠(DSS)模型和T细胞诱导性结肠炎模型,也可用于类似研究。我们通过口服灌胃方式给予粪便悬液,该途径在人类中已被证实比灌肠更有效且更优20。

方案

所有动物实验程序均经加尔维斯顿德克萨斯大学医学分部机构动物照护与使用委员会批准(方案编号 # 1512071A)。

1. 新鲜粪便样本的采集

  1. 准备无菌纸巾、钝头镊子和 50 mL 离心管。
    1. 将部分纸巾和镊子分别放入独立的高压灭菌袋中,在 180 °C 干热条件下高压灭菌 30 分钟。同时使用无菌的离心管。称量离心管并将其重量标记在管壁上。
  2. 打开动物房内的生物安全柜。
  3. 取一个已高压灭菌且不含垫料的洁净小鼠笼,放入生物安全柜中。取下笼盖和食槽,并将其一同置于柜内。
  4. 在笼底放置一些无菌纸巾,并将金属笼架重新放回笼子上方。
  5. 选择年龄匹配的粪便供体小鼠,将小鼠笼放入生物安全柜中。打开笼子,用镊子或手轻轻抓住供体小鼠(C57BL/6J)的尾巴,将其放置在洁净笼子上方的金属架上。
  6. 将笼盖盖在金属架上方,等待动物排便。
    注意:可同时将几只同窝小鼠放在金属架上。
  7. 收集粪便颗粒,放入无菌的 50 mL 离心管中。按性别分别收集粪便颗粒,不要混合来自雄性和雌性小鼠的粪便颗粒。
  8. 再次称量离心管,计算粪便颗粒的重量。

2. 粪便悬液的制备

  1. 制备无菌溶液(10% 甘油生理盐水)。
  2. 每克粪便颗粒加入 10 mL 的 10% 甘油/生理盐水至锥形管中。
    注意:如有需要,可将溶液体积增加至 20 mL。本研究中也采用每粒粪便颗粒(5–10 mg)加入 1 mL 溶液。
  3. 在生物安全柜中使用台式匀浆器或搅拌器以低速匀浆混合物,使粪便充分重悬(3 次,每次 30 秒)。
  4. 将粪便悬液通过两层无菌棉纱布(10.2 cm × 10.2 cm)过滤。滤液可暂时储存于冰箱中,最长不超过 6 小时;或分装至无菌冻存管中,保存于 -80 °C 超低温冰箱。
  5. 按照标准操作程序彻底清洗匀浆器或搅拌器。

3. 通过口服灌胃给予粪便悬液

  1. 若使用冷冻样本,需将冷冻的粪便悬液置于冰上解冻。解冻后通过涡旋振荡充分混匀粪便悬液。
  2. 将新鲜或解冻的粪便悬液转移至1 mL注射器中。
    注意:每只小鼠将接受总共200 µL的粪便悬液;对照组中的每只IL-10-/-小鼠将接受200 µL的10%甘油/生理盐水17。
  3. 称量小鼠体重,并选择合适的灌胃针规格及最大给药体积。
    注意:对于体重在20–25克之间的小鼠,应使用20 G、3.81 cm长的弯头灌胃针,末端带2.25 mm球状头。更多详情请访问gavageneedle.com。
  4. 通过测量小鼠鼻尖至剑突(胸骨下端)的距离来测试灌胃针长度。用10%甘油/生理盐水或粪便悬液填充注射器,并排除注射器及针头内的气泡。
    注意:若针长超过该距离,应在针管/导管上与鼻尖齐平处做标记。操作时切勿将针管/导管插入超过该标记位置,以防胃穿孔。
  5. 将一个鼠笼放入生物安全柜中,取下塑料笼盖,保留金属笼架。
  6. 抓住小鼠尾巴将其放置于金属笼架上。一手握住尾巴,另一只手用拇指和中指捏住肩部皮肤以固定动物,使前肢向两侧伸展,防止前爪将针推出。轻柔地向后伸展动物头部,并用手固定头部位置。
    注意:操作人员应在熟练掌握小鼠操作技术并具备充分信心后,再开始正式实验。
  7. 将灌胃针置于舌面上方并插入口腔内,沿上颚轻柔推进,直至针头进入食管。应平稳地一次性完成插入动作。如遇阻力,切勿强行推进,应退出针头后重新尝试。
  8. 确认针头正确到位后,缓慢推动连接的注射器,以注入样品。切勿旋转针头或继续向前推进,以免导致食管破裂。给药完成后,轻柔地将针头拔出。
  9. 将小鼠放回原笼。观察动物5–10分钟,注意是否存在呼吸困难或不适表现。在粪便微生物移植(FMT)后12–24小时内再次进行监测。

4. 疾病监测与安乐死

  1. 通过粪便潜血试验和/或结肠镜检查对小鼠进行IBD发病的纵向监测21。采用经胸超声心动图评估心脏功能22,23。
  2. 在异氟烷(1%–4%)深度麻醉下,通过断头法处死动物。
  3. 使用含抗凝剂的微量离心管收集血液,并在4 °C冷藏离心机中以1000–2000 × g 离心10分钟。将上清液(定义为血浆)保存于-80 °C冰箱中。
  4. 处死后,采用瑞士卷技术24制备小鼠结肠,用于组织病理学分析(H&E染色)25。
  5. 使用酶联免疫分析(EIA)试剂盒检测血浆中B型利钠肽(BNP)水平23。

结果

作为一种创新性的实验性治疗方法,粪便微生物移植(FMT)近年来已成为治疗多种疾病的研究热点,因为共生微生物群的失调被认为参与了包括炎症性肠病(IBD)、肥胖、糖尿病、自闭症、心脏病和癌症在内的多种人类疾病的发病机制26。尽管其作用机制尚未明确,但普遍认为FMT通过重建新的生物菌群并防止残余细菌的丢失而发挥作用。本文所采用的方法以口服灌胃作为递送途径,该方式已被证实有效27。我们选择口服途径,是因为其最为方便、经济,且大多数患者更易于接受。此外,临床试验已发现,口服FMT胶囊是治疗复发性艰难梭菌感染(CDI)的有效方法28。其他常见的上消化道递送方式包括鼻胃管、鼻空肠管、空肠造瘘管和经内镜食管胃十二指肠检查。常见的下消化道递送方式则包括结肠镜、结肠经内镜肠道置管(TET)和直肠灌肠。然而,FMT的最佳递送途径仍不明确20。目前认为上消化道给药最为合适29,但尚无一种理想途径适用于所有患者。
