HIV-1感染人源化小鼠的口服联合抗逆转录病毒治疗

本方案描述了一种新型的口服联合抗逆转录病毒药物递送方法,该方法在人源化小鼠中成功抑制了HIV-1 RNA的复制。

摘要

人类免疫缺陷病毒(HIV-1)大流行仍在全球范围内持续蔓延,目前尚无针对 HIV 的有效疫苗。尽管联合抗逆转录病毒疗法(cART)在抑制病毒复制方面已取得成功,但仍无法彻底清除 HIV 感染者体内的病毒储存库。实现对 HIV 感染安全且有效的治愈策略需要采用多种手段,因此建立适用于 HIV-1 感染的动物模型对于推动 HIV 治愈研究至关重要。人源化小鼠能够重现 HIV-1 感染的关键特征。该人源化小鼠模型可被 HIV-1 感染,并可通过 cART 方案控制病毒复制。此外,在停止 cART 治疗后,人源化小鼠会出现迅速的病毒反弹。然而,向动物施用 cART 可能效果不佳、操作困难或具有毒性,许多临床上重要的 cART 方案难以得到最佳应用。此外,研究人员常用每日多次注射的方式给药,这种高强度注射过程不仅可能对研究人员造成安全风险,还会因对动物进行物理束缚而引发应激反应。本文所述的新型口服 cART 方法用于治疗 HIV-1 感染的人源化小鼠,可将病毒血症抑制至检测水平以下,提高 CD4+ 细胞恢复速率,并改善 HIV-1 感染人源化小鼠的整体健康状况。

引言

随着联合抗逆转录病毒治疗(cART)的应用,慢性人类免疫缺陷病毒(HIV)感染者的预期寿命已显著改善1,2。在大多数慢性HIV-1感染者中,cART能有效抑制HIV-1的复制,并使CD4+ T细胞计数恢复至正常水平3,从而改善整体健康状况,并显著减缓疾病进展4。然而,即使在急性感染期即开始抗逆转录病毒治疗(ART),仍会形成潜伏的HIV-1储存库5,6,7。这些储存库可在ART期间持续存在多年,且在ART中断后病毒迅速反弹的现象已有充分记录8,9。接受ART治疗的HIV感染者还面临更高的共病风险,如心血管疾病、癌症和神经功能障碍10,11,12。因此,实现HIV的功能性治愈至关重要。HIV-1感染的动物模型在开发和验证新型HIV治愈策略方面具有明显优势13,14,15。人源化小鼠作为一种小型动物模型,可在不同组织中重建多谱系的人类免疫细胞,从而便于深入研究HIV感染过程16,17,18,19。在各类人源化模型中,人源化骨髓-肝脏-胸腺(BLT)模型能够成功模拟慢性HIV-1感染以及针对HIV-1感染的功能性人类免疫应答20,21,22,23,24。因此,人源化BLT小鼠模型已被广泛应用于HIV研究领域的多个方面。人源化BLT小鼠不仅是模拟持续性HIV-1感染和致病机制的成熟模型,也是评估基于细胞治疗的干预策略的重要工具。本研究作者及其他研究者已证实,人源化BLT小鼠模型能够再现持续性HIV-1感染和致病过程25,26,27,并为评估基于细胞治疗的干预策略提供了有效工具28,29,30,31,32,33。

由多种抗逆转录病毒药物组成的每日服用cART方案可抑制HIV-1复制,使接受成功治疗个体的病毒载量长期保持在检测不到的水平34。在人源化小鼠中使用临床相关cART方案治疗的结果与接受ART治疗的HIV-1感染者中观察到的结果相似22:HIV-1水平被抑制至检测限以下,而中断cART会导致潜伏病毒库中的HIV复制反弹35。皮下(SC)27,36,37或腹腔内(IP)37,38,39注射是目前人源化小鼠cART治疗常用的给药途径。然而,频繁的每日注射因需对动物进行物理固定而引发动物应激反应40。此外,该方法劳动强度高,且由于使用锐器时研究人员暴露于HIV的风险增加,可能存在安全隐患。口服给药方式能够更好地模拟HIV-1感染者服用cART药物时的吸收、分布和排泄过程,因此更为理想。口服给药通常需要将抗逆转录病毒药物加入经灭菌处理的饲料24,37,41或饮水中42,43,44,45,46,这一过程往往需要定制化操作且较为繁琐;同时,灭菌步骤对于免疫缺陷小鼠而言是必要的。然而,许多抗逆转录病毒药物可能与饲料或水在化学上不相容,或导致小鼠不愿进食或饮水,从而影响实际给药剂量及体内药物浓度。本文提出的新型经口cART给药方法克服了以往递送方式的局限性,具有与多种抗逆转录病毒药物兼容、制备和操作安全简便,以及减少因每日注射所致动物应激和焦虑等优势。

替诺福韦二吡呋酯(TDF)、艾维雷韦(ELV)和拉替格拉韦(RAL)均为水溶性较差的药物。有趣的是,高脂饮食可提高TDF的生物利用度,提示食物中的脂肪可能通过竞争性抑制脂肪酶对TDF起到一定保护作用47。因此,选择DietGel Boost营养凝胶杯替代常规啮齿类动物饲料作为给药方式,因其脂肪含量适中(每100克含20.3克),相较于常规啮齿类饲料(每100克含10克)和典型的小鼠高脂饮食(每100克含40–60克)更为适宜48。每个营养杯总重为75克,因此每杯所提供的食物量(即药物量)足以满足五只小鼠3天的需求。

方案

匿名的人类胎儿组织通过商业途径获得。动物实验根据加州大学洛杉矶分校(UCLA)动物研究委员会(ARC)批准的方案进行,符合所有联邦、州和地方的指导原则。具体而言,所有实验均遵循美国国立卫生研究院(NIH)和国际实验动物护理评估与认证协会(AALAC)关于实验动物饲养和护理的建议与指南,实验方案编号为UCLA ARC 2010-038-02B。所有手术均在氯胺酮(100 mg/kg)/赛拉嗪(5 mg/kg)和异氟烷麻醉(2-3 vol%)下进行,并尽一切努力减轻动物的疼痛与不适。

1. 感染 HIV-1 的人源化小鼠

注意:人源化小鼠的构建方法如先前文献30,31,49所述。以下简要描述该方案。

  1. 从人胎肝中纯化CD34+造血祖细胞 通过 根据制造商方案使用抗CD34磁珠。
  2. 麻醉6至8周大的NOD/SCID/IL2Rγ−/−(NSG)雄性和雌性小鼠,并在手术前进行亚致死剂量照射(2.7 Gy)。
  3. 将来自与胎肝相同供体的胸腺植入肾包膜下,与肝脏一同移植。
  4. 植入后,通过静脉注射向小鼠注入50万至100万个CD34+细胞。
  5. 8–10 周后,采集 100 µL 小鼠血液 通过 眼后静脉丛采血50 加入含有5 µL EDTA的微量离心管中,于350 x g 3 分钟。
  6. 将血浆储存于 -80 °C,用于监测小鼠感染 HIV-1 后的病毒载量。加入 2 mL 83% NH4Cl 溶液,室温孵育 5 分钟以裂解红细胞。
  7. 加入10 mL含10%胎牛血清(FBS)的RPMI培养基以终止裂解。300 x g离心 g 5 分钟。
  8. 吸弃上清液。用抗体组合对细胞进行染色(参见 材料表并通过流式细胞术分析以检测人免疫细胞的植入情况。
  9. 通过眶后静脉注射感染外周血中CD45+细胞比例超过50%的小鼠51,52 含有至少 200 ng p24 的 HIV-1 毒株(例如 NFNSXSL9)30,53,54使用胰岛素注射器采集血液,每两周一次,用于流式细胞术分析及病毒载量测定。

2. 抗逆转录病毒药物的制备

  1. 称量单个药物;例如,制备10个含cART的食物杯时,在生物安全柜中使用无菌细胞刮刀,分别将250 mg的FTC(恩曲他滨)、375 mg的TDF和500 mg的RAL或ELV称量至各自的无菌15 mL离心管中。
  2. 向250 mg FTC管中加入1 mL DMSO(终浓度为250 mg/mL),向375 mg TDF管中加入1.5 mL DMSO(终浓度为250 mg/mL),向500 mg RAL或ELV管中加入1 mL DMSO(终浓度为500 mg/mL)。搅拌或使用移液器混匀药物混合物,直至完全溶解并获得澄清溶液。
  3. 使用孔径为0.22 µm的亲水性PVDF膜滤器和无菌注射器对溶液进行除菌过滤。单个药物溶液可在-20 °C下保存12周。
  4. 使用前,将每种药物溶液的一份等分试样在37 °C下新鲜解冻至溶液澄清。使用移液器充分混匀。
  5. 混合药物并充分混匀以制备母液:1 mL溶于DMSO的FTC、1.5 mL溶于DMSO的TDF和1 mL溶于DMSO的ELV或RAL。
    注意:此剂量可制备10个食物杯。
  6. 向一个食物杯中加入350 µL cART母液,制成一个DietGel Boost cART食物杯。
  7. 向该食物杯中加入0.75 mL甲氧苄啶-磺胺甲噁唑(终浓度为0.48 mg/mL)。
  8. 使用1 mL无菌移液器吸头充分搅拌。
  9. 根据需要,使用微型刮刀将含cART的食物从原杯分装至60 mm培养皿中。在天平上称量食物重量,根据小鼠数量计算每个笼子所需的含cART食物杯用量。

3. 向 HIV-1 感染小鼠施用抗逆转录病毒药物

  1. 将普通饲料从笼中移除,并替换为含有 cART 的食杯。
    注意:小鼠平均每天最多可摄食 5 g 食物。大约一个食杯可供五只小鼠摄食 2 天。
  2. 每周更换三次 cART 食物。
  3. 称量使用过的食杯以监测摄食量。每周称量小鼠体重以确认摄食情况。

4. 通过实时 PCR 监测病毒载量

  1. 每两周通过眼后静脉采血评估BLT小鼠体内的人类免疫细胞(CD4和CD8 T细胞水平)以及HIV-1的复制情况。按照步骤1.5–1.8的说明收集血浆。
  2. 监测HIV-1感染小鼠在口服cART治疗前及治疗期间共8周的血浆病毒载量。使用病毒RNA提取试剂盒从血浆中提取血浆病毒RNA,并如前所述27,30,31,利用引物和探针(见材料表)通过实时PCR进行定量。采用以下扩增程序:48 °C(15分钟),95 °C(10分钟),然后进行45个循环:95 °C(15秒)、60 °C(1分钟)。

通过流式细胞术评估 CD4/CD8 比值

  1. 按照步骤 1.5–1.8,从双周采血的外周血中制备单细胞悬液。
  2. 使用表面标志物抗体对细胞进行染色,并通过流式细胞术进行分析。流式细胞术中使用以下表面标志物抗体27,30,43,49:CD45(克隆号 HI30)、CD8(克隆号 SK1)、CD3(克隆号 OKT3)、CD4(克隆号 RPA-T4)27,30,42,49。

结果

假设一只平均体重为25 g的小鼠每天消耗4 g食物,则通过口服给药的每日药物剂量相当于2.88 mg/kg的替诺福韦(TFV)、83 mg/kg的恩曲他滨(FTC)以及768 mg/kg的拉替拉韦(RAL)。为了检测优化后的食物给药方案相较于每日注射cART是否具有毒性并影响整体健康状况,我们在cART治疗前及治疗期间每周监测小鼠体重,给药途径包括口服和皮下注射。各组在开始cART治疗前体重无显著差异(图1)。然而,在每日皮下注射cART期间,小鼠体重持续下降。相比之下,通过DietGel口服给予FTC/TDF/ELV或FTC/TDF/RAL的小鼠在5周后体重恢复至ART治疗前水平。此外,拉替拉韦组与艾维雷韦组之间未观察到显著的体重变化差异。

为了检测口服cART给药是否能像每日注射一样有效抑制病毒载量,采用RT-PCR方法每两周评估一次血浆病毒载量。图2显示,FTC/TDF/ELV ART食物给药方案可在4周内以100%的效率将病毒复制抑制至检测不到的水平;FT...

讨论

本研究通过将三种抗逆转录病毒药物与高营养食物混合,建立了一种用于HIV-1感染人源化小鼠的口服cART给药方法。与每日注射给药相比,口服给药使用更简便、给药频率更低、减少了对动物的操作、最大限度地降低了应激反应,并提高了安全性55。迄今为止,仅有少数针对人源化小鼠的研究24,37,41采用含有研碎抗逆转录病毒药物的饲料颗粒来治疗小鼠。然而,由于难以获得特制饲料颗粒的生产途径,该方法难以广泛推广。其他研究42,43,44,45,46则使用饮水作为cART的递送系统。但将药物配制于饮水中...
