大学生作为具有高教育潜力的青年核心群体,在推动全球社会进步和可持续发展中发挥着关键作用1。他们正处于身心发育的关键阶段,同时面临学业压力、社会适应和独立生活等多重挑战。因此,其健康状况日益受到生活方式和校园环境的影响2。例如,繁重的学习任务、不规律的作息、缺乏锻炼以及自我管理能力不足,常常导致身心健康问题的发生3。越来越多的证据表明,大学生正面临显著的心理健康挑战。一项大规模的荟萃分析显示,该人群抑郁症状的合并患病率为34%,焦虑症状为32%4。这些比率显著高于一般人群,凸显出这一人生阶段所特有的压力,这一现象在东亚地区同样明显5。这一过渡时期常常打破原有的健康习惯,因此在此期间培养积极的健康行为,对于塑造终身健康至关重要。

世界卫生组织指出,健康行为能够提高健康意识、自我管理能力以及生活质量6。健康行为是指个体在适应环境过程中与身心健康相关的各种行动7。这些行为在健康生活方式的形成过程中发挥着至关重要的作用。为了理解与改善行为相关的各种因素,并有效促进在短期和长期内以健康习惯替代不健康习惯的潜在机制,有必要采用有效的理论模型。

在各种健康行为干预模型中,由 Manoj Sharma 教授于 2015 年首次提出的健康行为改变多理论模型(Multi-Theory Model, MTM)8 具有突出地位。与其他行为改变理论(如跨理论模型(Transtheoretical Model, TTM)、计划行为理论(Theory of Planned Behavior, TPB)和社会认知理论(Social Cognitive Theory, SCT))相比,MTM 在满足大学生特定需求方面具有独特优势。尽管 TTM 能有效描述行为改变的各个阶段,但在维持新行为方面提供的可操作策略有限,而这一点对于面临不断变化的学业与社交压力的学生而言尤为困难9。TPB 强调行为意向是关键预测因素,但未明确区分行为的启动与维持,而这一区分在学生动态的生活环境中至关重要10。SCT 尽管强调了自我效能和观察学习,但在设计针对特定阶段的干预措施时缺乏系统性的框架支持11。相比之下,MTM 不仅将行为改变过程划分为启动和维持两个阶段,还为每个阶段引入了具体且可测量的构念,使其特别适用于高校环境中靶向性强且可持续的健康行为干预方案的开发12。该理论将健康行为改变分为启动和维持两个阶段,从而能够应对健康行为干预中复杂多样的需求13

MTM 的发展经历了四个阶段。早期理论侧重于通过提供信息来传播知识和提高意识,从而改变行为14。第二代理论加入了技能训练和意识提升的内容15。第三代理论出现于20世纪90年代,引入了基于证据的技术,并强调以实证为基础的理论框架,如社会认知理论11。在第四阶段,即最新阶段,MTM 整合了多个经典理论的核心要素,以全面分析和预测健康行为的启动与维持16。MTM 融合了多种理论的关键组成部分,以更准确地预测和解释个体如何开始并持续保持健康行为。

MTM 将行为改变分为两个阶段:启动和维持。启动阶段受参与式对话(PD)、行为信心(BC)和物理环境变化(CPE)的影响。维持阶段则受到情感转化(ET)、行为改变实践(PC)和社会环境变化(CSE)的影响17。MTM 已成功应用于饮食管理18、洗手行为19、戒烟20、戒酒12以及戒赌21等领域。然而,现有大多数研究主要集中于该模型适用性的定性描述或单因素相关性分析17,22,23,缺乏对 MTM 各构念与中国大学生健康行为之间关联强度的定量综合分析。

鉴于大学生面临的独特挑战,探讨动机动机转变模型(MTM)在其健康行为中的作用十分必要。本研究从行为改变的两个阶段——启动阶段和维持阶段入手,聚焦该模型的六个关键构念,旨在量化各MTM构念与大学生健康行为改变之间的横断面关联强度。通过定量整合现有数据,本研究提供了关于不同MTM构念与大学生健康行为之间横断面关系强度的可比较证据。通过综合相关性数据,本研究旨在提出假设,明确在各个阶段中哪些构念最为关键,从而为未来开展纵向和实验性研究提供检验依据。本研究弥补了以往系统综述的局限性,这些综述大多停留在定性描述层面,缺乏对MTM构念与大学生健康行为之间横断面关联证据的定量综合。此外,本研究填补了针对大学生群体在MTM定量研究方面的空白,为面向该群体的健康干预措施提供了更为精确的理论基础。进一步地,本研究通过定量分析识别出针对不同阶段的干预重点,为制定分阶段的大学生健康行为干预策略提供了直接的初步证据。

本系统综述和荟萃分析方案已在PROSPERO注册,注册编号为CRD 420251012795。本研究的设计与实施均遵循《系统综述和荟萃分析优先报告条目》(PRISMA)24指南(补充文件1)。

1. 文献检索策略

通过检索PubMed、Embase、Web of Science、Scopus、万方和中国知网(CNKI)数据库中自各数据库建库至2024年10月期间发表的研究,获取相关文献。中文检索词包括 "多里路模型" 与 "大学生" 与 "健康行为". 使用的英文检索词为 (("多理论模型")或("MTM")) 并且 (("大学生*")或("大学生*")或("本科生*")或("院士")) 并且 (("健康行为*")或("健康行为*")或("健康相关行为*")).

2. 文献纳入与排除标准

纳入标准如下:(1)研究对象为大学生;(2)采用多理论模型(MTM)作为健康行为的预测框架;(3)提供了足够数据以计算关联性指标(OR值);(4)研究设计为横断面研究;(5)纳入的文章语言限于英文和中文。排除标准如下:(1)重复发表的研究;(2)数据不完整或无法获取的研究;(3)未使用多理论模型(MTM)的研究;(4)综述、病例报告、会议摘要、动物实验等。

3. 文献筛选与数据提取

将检索到的文献导入 EndNote X21 软件进行文献综述和去重。研究筛选过程由两名评审员(X.L.H. 和 K.X.H.)独立完成。首先,他们根据纳入标准对标题和摘要进行筛选;随后,获取并评估可能相关文章的全文。每个阶段出现的任何分歧均通过协商一致解决,必要时由第三位高级评审员(X.X.Z.)进行裁决。数据提取由相同的两名评审员(X.L.H. 和 K.X.H.)独立完成,使用预先测试过的标准化数据提取表格。提取的数据包括以下内容:第一作者、发表年份、研究国家、样本量、人口学特征(如性别和平均年龄)、所研究的具体健康行为、各 MTM 构念的测量工具,以及各构念与行为改变之间关联的效应量数据(如带有 95% 置信区间的比值比)。

4. 方法学质量评估

采用一种专为横断面研究设计的综合评价工具来评估所纳入研究的方法学质量25。该工具包含七个评价标准:(1)科学合理的设计;(2)合理的数据收集策略;(3)报告样本应答率;(4)样本对总体具有良好的代表性;(5)研究目的与方法恰当;(6)报告统计检验效能;(7)统计方法合理。各项指标的回答分为"是"、"否"和"不明确"三类,分别赋值为1分、0分和0.5分。若某项指标不适用,则以"_"表示,并计为1.0分26。该量表总分为7.0分。得分6.0–7.0为A级质量,4.0–5.5为B级质量,低于4.0为C级质量。质量评价由两名评审员(X.L.H. 和 X.Z.)独立完成,评分分歧通过讨论达成共识。

5. 统计分析

数据采用 Review Manager 5.3 软件进行分析。研究间的异质性通过P值和统计量进行评估。若P>0.1 且<50.0%,表明研究结果间的异质性可接受,则采用固定效应模型27。若P≤0.1 且≥50.0%,表明存在显著异质性,则采用随机效应模型28。本研究全程使用比值比(OR)作为定量效应量指标。在必要时,进行敏感性分析或亚组分析以探讨异质性的来源。统计学显著性水准设定为P<0.05。发表偏倚通过漏斗图的视觉检查以及Egger线性回归检验进行评估,后者适用于包含足够数量研究(n ≥ 10)的结局指标。主要的数据分析,包括森林图和漏斗图的生成,使用 Review Manager(RevMan)5.3 版本软件完成。此外,发表偏倚的统计学检验(Egger检验)采用 Stata 18 版本软件进行。对于研究数量充足(n ≥ 10)的结局指标,使用漏斗图评估发表偏倚,显著性水准设定为 α = 0.05。

初步共检索到90篇文献。通过使用EndNote软件并结合人工筛选剔除43篇重复文献后,保留47篇文献。通过阅读标题和摘要,排除19篇明显不相关或不符合本研究主题与设计要求的文献。进一步阅读全文后,因与研究主题无关、数据不完整或无法获取全文,再排除15篇文献。最终共有13篇文献12,22,23,29,30,31,32,33,34,35...
