Intramedullary Drilling

Intramedullary drilling is a technique that uses a rotating drill to create a controlled channel or defect within the medullary cavity of a long bone, the region containing bone marrow. In bone biology, the procedure typically passes through cortical bone into the marrow space, where drill diameter, depth, and speed influence injury size and the resulting mechanical and cellular responses. This controlled disruption can trigger inflammation, progenitor-cell recruitment, vascular changes, and new bone formation, allowing researchers to examine interactions among marrow, vasculature, and skeletal tissue during repair. Experimental models use intramedullary drilling to standardize bone injury, study regeneration, and evaluate effects on healing.

Intervertebral disc degeneration is a significant contributor to back pain and a leading cause of disability worldwide. Numerous animal models of intervertebral disc degeneration exist. We demonstrate an ovine model of intervertebral disc degeneration, utilizing a drill bit, which achieves a consistent disc injury and reproducible level of disc degeneration.

This protocol describes performing a craniectomy using a high-speed pneumatic drill on a 3-month-old Danish Landrace pig. The access is made through the frontal bone and reveals the ventral dura mater and underlying cerebral hemispheres. This procedure allows for access to a large portion of the pig brain.

Treatment with Locking Intramedullary Nailing for Intertrochanteric Fracture of the Femur Utilizing a New Awl with a Distal Positioner

Here, we present a protocol for accessing the guidewire of intramedullary femoral nailing in obese patients using an in-house designed awl. Using the new awl with a distal positioner for the guidewire insertion and opening the bone can increase efficiency in inserting the guidewire and reduce the difficulty of surgery.
