Tuberculosis Diagnosis

Tuberculosis diagnosis is the process of identifying infection with Mycobacterium tuberculosis and determining whether disease is active, enabling timely treatment and limiting transmission. Clinicians combine medical history and examination with tests such as the tuberculin skin test or interferon-gamma release assay, which detect immune sensitization, while chest radiography, sputum microscopy, nucleic acid amplification, and culture provide evidence of pulmonary infection. Culture remains an important reference method, whereas molecular tests can rapidly detect bacterial DNA and, in some platforms, drug-resistance mutations. Accurate diagnosis supports case confirmation, guides antimicrobial selection, distinguishes latent infection from active disease, and strengthens tuberculosis control in clinical and public-health settings.

Mycobacterium tuberculosis forms drug tolerant biofilms when cultured in certain conditions. Here we describe methods for culturing M. tuberculosis biofilms and determining the frequency of drug tolerant persisters. These protocols will be useful for further studies into the mechanisms of drug tolerance in M. tuberculosis.

Separation and Fractionation of Cell Wall and Cell Membrane Proteins from Mycobacterium tuberculosis for Downstream Protein Analysis

This protocol separates insoluble cell wall and membrane proteins into simple fractions (1-5 proteins) using preparative isoelectric focusing (IEF) based on isoelectric point, followed by separation by molecular weight. The resulting fractions can be used directly for immunological and proteomic analysis without further purification.

An In Vitro Caseum Binding Assay that Predicts Drug Penetration in Tuberculosis Lesions

Here we describe a rapid equilibrium dialysis (RED) method to measure drug binding to caseum from pulmonary tuberculosis lesions and cavities. The protocol is also used with a foamy macrophage-derived matrix that is an effective surrogate to caseum.

Diagnosing Pulmonary Tuberculosis with the Xpert MTB/RIF Test

The Xpert MTB/RIF test integrates sample decontamination, hands-free operation, on-board sample processing, and ultra-sensitive hemi-nested PCR for the simultaneous detection of Mycobacterium tuberculosis and rifampicin resistance, either in expectorated sputum or concentrated sputum sediments, in approximately two hours. Testing is standardized and requires only moderate laboratory infrastructure and training.
