Levator Ani Avulsion

Levator ani avulsion is a structural injury in which part of the pelvic floor muscle separates from its attachment to the pubic bone, often in association with difficult vaginal birth. Excessive stretching and compression of the levator ani during fetal descent, particularly with marked muscle distension or instrumental delivery, can disrupt muscle fibers and their bony insertion. The injury may reduce pelvic-floor support and is assessed through clinical examination and imaging, including translabial ultrasound or magnetic resonance imaging. Recognizing avulsion helps clinicians investigate pelvic organ prolapse and related dysfunction, counsel patients about prognosis, and guide follow-up or rehabilitation.

We describe procedures for repeated administration of inhibitors of muscarinic signaling to the levator auris longus (LAL) muscle of young adult mice and for subsequent immunostaining of its neuromuscular junctions (NMJs) in wholemounts. The LAL muscle has unique advantages for revealing in vivo pharmacological effects on NMJs.

The protocol described in this paper uses the mouse levator auris longus (LAL) muscle to record spontaneous and nerve-evoked postsynaptic potentials (current-clamp) and currents (voltage-clamp) at the neuromuscular junction. Use of this technique can provide key insights into mechanisms of synaptic transmission under normal and disease conditions.

Here we present a protocol to standardize the measurement of the levator ani hiatus size by magnetic resonance imaging. The purpose is to extract biomechanical inferences from image analysis by comparing resting and straining values in patients of both sexes with pelvic prolapse, using consistent anatomical bony landmarks.
