Guiding Sheath

A guiding sheath is a flexible, hollow medical device that provides a stable pathway for introducing and directing catheters, guidewires, or other instruments through the vasculature. In neurointerventional procedures, it is inserted into a blood vessel and advanced toward the cerebral circulation, where its lumen supports device passage while its outer structure helps maintain access and reduce repeated vessel entry. Guiding sheaths enable procedures such as cerebral angiography, endovascular treatment, and delivery of devices for managing vascular disorders. Their design and placement influence navigation, procedural control, and the safe access required for minimally invasive neuroscience research and clinical care.

Point-of-care ultrasound (POCUS) of the optic nerve sheath diameter (ONSD) has been shown to be useful in identifying patients with increased intracranial pressure (ICP). However, the non-standardized technique for this POCUS has hampered its use. We present a standardized image acquisition protocol for use in the acute care setting.
