1. 干细胞来源的视网膜色素上皮细胞的定向分化
注意：所有孵育步骤均在 37 °C、5% CO2 条件下进行

	<li>使用维持培养基（MM；表1）每日换液，维持hES或hiPS细胞系的培养。当细胞系达到足够的质量控制标准后，将其培养在接种密度为每六孔板孔250,000的鼠胚胎成纤维细胞（MEFs）饲养层上。也可按照Tucker et al.40建立的方案制备无异源成分的培养体系。</li>
	<li>让干细胞集落生长至融合状态。</li>
	<li>更换为含烟酰胺的分化培养基（DM/NIC；表1），每日换液。</li>
	<li>培养三周后，更换为含烟酰胺以及Activin-A或IDE-1的分化培养基（DM/NIC/AA或DM/NIC/IDE1；表1），以促进视网膜色素上皮（RPE）的分化。</li>
	<li>培养五周后，重新换回DM/NIC培养基。当培养基中的pH指示剂在换液后第二天不再变黄时，可停止每日换液。</li>
	<li>等待出现足够大、可用于切割和分离的色素化细胞岛（见图2D-F和图3A）。</li>
</ol>
2. 分离色素性胰岛
<ol style="margin-left: 40px;">
	<li>用一种模拟天然细胞环境的基质包被24孔板的孔（优选无异源成分的基质）。 
	注意：角膜刀和尖细的精细镊子在挑取操作中非常有用。在挑取过程中，整个分化细胞层可能脱落。为避免此情况，请小心操作，并从培养皿中央开始挑取克隆。</li>
	<li>根据所收集的色素化克隆数量，向24孔板中加入适量的分化培养基（DM；表1）。</li>
	<li>在配备解剖显微镜的细胞培养罩内，手动切割出带有色素的细胞岛。使用手术刀在原培养板底部将每个细胞岛切成约2–6小块，并用尖头镊子夹取带色素的部分。</li>
	<li>将1–2个带色素的组织块转移至每个24孔板孔中。</li>
	<li>在细胞贴壁前，3–5天内不更换培养基；贴壁后开始每周更换培养基三次。（若超过该时间仍未能贴壁，则很可能无法贴壁。）</li>
	<li>在分化培养基（DM；表1）中培养3–4周——典型培养状态如图3B所示。此时细胞可能仍未完全铺满孔底，但延长培养时间通常无明显改善。每周喂养细胞三次。</li>
	<li>约3周后，如有必要，可使用尖头镊子手动去除成团的白色细胞簇。除非确有必要，否则不要进行此操作——该步骤容易引入污染。理想情况是在第1.6步中即从原始色素化克隆获得最纯的视网膜色素上皮（RPE）细胞群体。</li>
</ol>
3. 干细胞来源的视网膜色素上皮细胞传代
<ol style="margin-left: 40px;">
	<li>使用200 µl细胞解离液（建议避免使用胰蛋白酶，优先选择可通过稀释失活的试剂）在37 °C条件下处理5–8分钟，通过加入DM培养基稀释以终止反应。用200 µl移液器冲洗并重悬所有细胞（若所选孔中细胞数量较少，可考虑合并两个24孔板中的细胞）。</li>
	<li>离心（800 × g，5分钟），弃去上清液，用3 ml DM培养基重悬细胞。</li>
	<li>将一个（或两个）24孔板中的细胞重新接种至一个经基质包被的6孔板中，每孔加入3 ml DM培养基（按1:5比例扩增）。</li>
	<li>扩增培养1–2周，直至细胞融合度达到约90%；完全分化的纯化培养物图像见图3C。</li>
	<li>使用1 ml细胞解离液在37 °C条件下处理约5–8分钟，通过加入DM培养基稀释以终止反应，用1,000 µl移液器冲洗并收集所有细胞，重悬细胞悬液。</li>
	<li>离心（800 × g，5分钟），弃去上清液，用18 ml DM培养基重悬细胞。</li>
	<li>将一个6孔板中的细胞分别接种至六个经基质包被的6孔板中，每孔加入3 ml DM培养基（按1:6比例扩增），或接种至一个包被的75 cm2培养瓶中（按1:7.5比例扩增）。</li>
	<li>根据需要重复步骤3.4–3.8，直至获得足够数量的细胞。 
	注意：应尽可能多地收集克隆用于扩增，而非依赖传代扩增培养物，因为每次传代均可能诱导视网膜色素上皮（RPE）细胞去分化。</li>

构成视网膜的各种细胞类型以特定的功能结构排列。位于视网膜后部的光感受器负责通过光转导过程将光转化为电信号。然而，若无邻近其他细胞类型（包括穆勒胶质细胞和视网膜色素上皮（RPE）细胞）的协同作用，光转导无法完成。单层RPE细胞将感光视网膜与脉络膜毛细血管（光感受器的主要血液供应来源）分隔开，并处于理想位置以调控多种对光感受器稳态至关重要的功能。事实上，RPE细胞与光感受器之间高度相互依赖，通常被认为共同构成一个单一的功能单位。（有关RPE多种功能的综述，参见Strauss, 20051。）视网膜色素上皮细胞的死亡或功能障碍可引发年龄相关性黄斑变性（AMD），这是工业化国家永久性视力丧失的首要原因2-4。

由干细胞诱导视网膜色素上皮（RPE）细胞是一个相对简单的过程，且可在完全无异源成分的条件下完成。最简单的方法是自发诱导形成RPE细胞单层，但通过定向分化技术可显著提高产量。然而，这些技术需要使用外源性蛋白质因子，这类因子通常价格昂贵，且常在细菌或其他非人类来源系统中生产10,12,37。在我们的研究中，我们采用了一种已建立的方案10，该方案使用烟酰胺和激活素A（Activin A）——一种已被证明足以诱导RPE特化的信号分子38。本文将证明，小分子IDE-1可有效替代Activin A，从而降低成本，并减轻使用重组蛋白所带来的相关顾虑39。此外，我们使用无异源成分的血清替代物，并将正在分化的RPE细胞培养在一种合成的无异源成分基质上。先前研究已表明，采用该方法可非常有效地诱导RPE细胞分化40。
当以单层形式分化时，最早在五周后即可观察到含有视网膜色素上皮（RPE）细胞的色素化克隆12。一旦克隆达到足够大小，可手动切除并转移至另一培养皿中进行扩增。RPE细胞在传代过程中极易发生去分化，因此应避免使用超过五代的细胞（我们发现，经过两到三代传代后，即可轻松获得足够数量的细胞，用于表征及在动物模型中的移植）。细胞完全分化后，我们采用多种技术从解剖学和功能学层面对其进行表征，以确保其能够有效替代病变的RPE细胞。这些表征技术的具体描述以及将iPS来源的RPE细胞植入啮齿类动物视网膜下腔的实验方案，已在此前发表的研究中详细阐述12,32,41，不再赘述于本方法学论文中。
尽管开发用于高效诱导iPS-RPE的标准化方案对于临床应用显然至关重要，但在动物模型中仍需开展大量临床前研究。目前对iPS来源细胞的免疫原性仍存在担忧，多种不同的移植技术（包括在人工基质上移植细胞）正在被探索42,43。基于这些原因，我们认为发表标准化操作方案将有助于推动临床及临床前研究的开展，特别是当需要对不同实验室、不同研究团队所获得的iPS-RPE细胞进行直接比较时。

本文所述步骤如图1所示，可便捷地从干细胞诱导生成视网膜色素上皮（RPE），方法如先前报道10,12。在维持iPS细胞系数周后，5至7周后细胞集落中开始出现色素化集落（7周龄培养物见图2A-C）。随着培养的持续，这些集落可继续生长数周。当集落达到足够大小时（8周龄培养物见图2D-F），可按图3A所示进行手动切除。仔细切除以避免非RPE细胞污染，将极大有助于获得纯度足够的RPE细胞培养物（图3B-C）。

本实验的总体目标是可重复地从干细胞中获得高质量的视网膜色素上皮（RPE）。首先将可诱导的多能干细胞培养至汇合状态，实现这一目标；第二步，更换培养基以诱导细胞向RPE方向分化。接下来，将RPE细胞从培养皿中的其他已分化细胞中分离出来，然后扩增培养，直至获得足够数量的细胞。最终，可通过观察培养细胞的深色色素沉着和鹅卵石样形态，确认RPE细胞已达到终末分化阶段。与现有的自发分化等方法相比，该技术的主要优势在于添加特定因子能够加速分化过程，缩短时间，并提高RPE细胞的产量。迄今为止，已有数千只啮齿类动物通过使用源自iPS细胞的RPE细胞治愈了多种视网膜退行性疾病。然而，若要将这一方法高效且有效地转化为临床人类疾病的治疗手段，建立完善且可重复的标准化方案至关重要，这些方案必须能够为我们提供高质量、高数量的RPE细胞。该方法的可视化展示极为重要，因为在任何给定时间内，仅能有一人在生物安全柜内的显微镜下操作，这使得分离步骤难以被观察。接下来，我实验室的研究技术员Mitchell Prince将演示与该程序的数字培养方面相关的实验方案：为引导干细胞分化为RPE细胞进行培养，需将目标干细胞系接种于六孔板中每孔含2×10⁶个细胞的鼠胚胎饲养层细胞上，待细胞集落达到汇合状态后，改为每日更换分化培养基。添加烟酰胺的培养基。三周后，开始在饲养液中加入活性物质或IDE，以促进视网膜色素上皮（RPE）的分化。两周后，换回分化培养基，并单独使用烟酰胺，停止每日饲养。一旦培养基中的 pH 指示剂在换液后第二天停止变为黄色，立即将培养物放回培养箱，直至出现足够大、可切割并移除的色素细胞小孔。为分离色素小孔，接下来用一种无异源成分且模拟天然细胞环境的基质对 24 孔板的孔进行包被。接着，根据需要，向 24 孔板的多个孔中加入分化培养基。在细胞培养超净台中，使用解剖显微镜和手术刀片将原始培养板底部每个有色素的眼状结构切成两至六块，用锐利的镊子夹取有色素的部分，然后将一至两块有色素的组织转移至每个孔中。将培养板置于37摄氏度、5%二氧化碳条件下培养三至五天，直至细胞贴壁，随后开始每周更换培养基三次。在分化培养基中培养细胞三到四周，每周三次继续换液并传代。视网状上皮细胞（RPE）情况，加入200 µL预热至37°C的无胰酶细胞解离液，以使细胞脱离。 5 到 8 分钟后，用新鲜的分化培养基终止反应，然后用 200 微升移液器将培养物重悬为单细胞悬液，收集细胞数较少的克隆。接着，将解离后的克隆转移至 15 毫升离心管中，离心沉淀细胞。将每管细胞沉淀重悬于 3 毫升分化培养基中，随后将克隆分别接种至矩阵编码的六孔板的各个孔中，每孔额外添加 3 毫升分化培养基。然后将细胞培养一至两周，直至细胞融合度达到约90%，再用一毫升解离溶液将细胞解离，方法如前所述。此次使用1000微升移液器重悬细胞，将单细胞悬液转移至50毫升锥形管中。然后再次离心细胞，将沉淀重悬于18毫升的分化培养基中，最后将细胞接种至基质包被的培养容器中，以在最少的传代次数内获得最大数量的目标细胞，用于后续下游分析。在诱导性多能干细胞系培养5至7周后，开始出现色素化的克隆。这些克隆在培养维持期间可继续生长数周。 当它们达到足够大小后，可进行手动切除。仔细切除以避免混入非视网膜色素上皮（RPE）细胞，可极大促进获得纯度足够的RPE细胞培养物。若技术操作得当，掌握该技术后可在8至12周内获得大量RPE细胞。在进行该操作时，重要的是要注意在配制培养基和喂养细胞过程中避免污染培养物。观看本视频后，我们希望你们中的许多人能够从诱导多能干细胞（IPS）中大量制备出高质量的视网膜色素上皮（RPE）细胞。希望这将有助于加快从实验室到临床的转化进程，为目前无法治疗的视网膜变性及新生血管性疾病患者提供基于细胞的治疗方案。
