注意：对动物实施的所有外科手术均应遵循《实验动物护理与使用指南》（www.nap.edu/catalog/5140.html）或其他适当的伦理准则。在开展实验前，相关方案须经相应机构的动物福利委员会批准。
1. 动物准备
<ol>
	<li>购买体重为 2.7–3.0 kg 的雄性日本白兔（或体型相当的兔子）。</li>
	<li>在进行实验前，将兔子在 12 小时光照-黑暗循环条件下适应 1 周，并饲喂常规兔粮。</li>
</ol>
2. 麻醉与动物准备
注意：所有后续操作均须在无菌条件下进行。手术区域和器械在使用前应使用10%聚维酮碘溶液、70%乙醇或季铵盐化合物进行消毒。
<ol>
	<li>通过耳缘静脉注射戊巴比妥钠（25 mg/kg）对兔进行静脉麻醉。</li>
	<li>确认兔失去自主活动能力后，将其转移至手术台，并置于仰卧位。用麻醉面罩覆盖其鼻部和口部，开始吸入0.7–1.0%异氟烷-氧气混合气体以实施全身麻醉。 
	注意：若兔出现活动，短暂将异氟烷浓度提高至2%可有效抑制。</li>
	<li>使用电动剪毛器修剪颈部和肩部的毛发。喷洒70%乙醇或其他消毒液对皮肤表面进行消毒后，用剃刀刮除颈部区域残留的毛发。以10%聚维酮碘对术区进行消毒，并皮下注射盐酸利多卡因（3&#160;mg/kg）作为局部麻醉。 
	注意：观察气管的节律性运动，注意颈静脉和颈动脉的搏动情况。当呼吸频率和脉搏减慢时，可考虑暂时减少麻醉剂用量。监测经皮血氧饱和度和脉率亦有帮助。 
	注意：此步骤可作为暂停点。</li>
</ol>
3. 颈静脉的采集
注意：在进行皮肤切口前，应使用局部麻醉剂（如利多卡因）。
<ol>
	<li>在皮肤切开前，皮下注射恩诺沙星（5 mg/kg）进行预防性给药。&#160;镇痛处理采用皮下注射布托啡诺（0.05 mg/kg），每日两次，持续3天。 
	注意：为避免体温下降，可使用手术部位擦洗代替将70%乙醇喷洒在动物体表。</li>
	<li>用手术刀沿颈部纵向切开50–60 mm。&#160;钝性分离皮下组织和筋膜，暴露20–30 mm长的颈静脉节段。使用4-0丝线结扎所暴露静脉的所有分支。</li>
	<li>在颈内静脉和颈外静脉周围放置一根2-0丝线，以便在下一步切开颈静脉后立即进行结扎。</li>
	<li>切开静脉壁远端约1 mm处。将一根2-French球囊导管从切口处向静脉近端插入。用2-0丝线在颈静脉远端位置进行结扎固定。</li>
	<li>向球囊内注入0.2 mL空气，使球囊膨胀。通过导管往返三次，剥离静脉内膜以实现内皮细胞脱落。 
	注意：该操作被认为等同于人类血管重建手术中大隐静脉移植物的扩张过程，后者同样会引起内皮细胞脱落。</li>
	<li>结扎静脉近端，切断静脉以获取移植物。 
	注意：应仔细区分所获取静脉的远端与近端，因为与动脉吻合时需采用反向连接方式（即静脉的远端应与动脉的近端吻合）。例如，从某一侧插入静脉导管可作为标记方法。</li>
	<li>对获取的颈静脉进行治疗性处理时，应根据具体研究问题采用相应方法处理静脉（例如电穿孔26或直接浸入溶液27，以将microRNA转导至静脉壁）。 
	注意：在本方案中，静脉移植物分别浸泡于磷酸盐缓冲液、对照microRNA及microRNA-145溶液中。此步骤可作为暂停点。</li>
</ol>
4. 使用获取的颈外静脉移植搭桥颈动脉
<ol>
	<li>暴露同侧颈动脉长约20–30 mm的节段，小心将其与邻近的静脉和神经分离。用4-0丝线结扎所有暴露静脉的分支。</li>
	<li>静脉注射肝素钠（200 IU/kg），等待3–4分钟。</li>
	<li>使用外科&#160;夹夹闭动脉的近端和远端，在两夹之间剪断动脉。从近端和远端向切开的颈动脉内注射生理盐水，以扩张血管。 
	注意：兔颈动脉容易收缩，应选择充分扩张的部位作为吻合部位。</li>
	<li>将获取的静脉以反向端对端方式与动脉进行吻合。
	<ol>
		<li>在远端吻合过程中，将一根20 G静脉导管从远端向近端插入获取的静脉，以保持静脉管腔开放。</li>
		<li>使用8-0聚丙烯间断缝线将静脉近端与动脉远端吻合。在吻合口处及其对侧位置各放置两针锚定缝线，然后在锚定缝线之间的吻合线上方添加缝线。</li>
		<li>将动脉和静脉移植物翻转至背面。在吻合线剩余部分继续添加缝线。</li>
		<li>从静脉中移除静脉导管，夹闭静脉移植物的近端，松开颈动脉远端的血管夹。确认静脉移植物逐渐扩张。</li>
		<li>使用8-0聚丙烯间断缝线将静脉远端与动脉近端吻合。松开动脉夹以检查吻合口是否出血，必要时加缝缝线止血。 
		注意：用纱布轻轻压迫出血部位并等待片刻，可能足以实现止血。近端松夹后应观察静脉移植物是否立即扩张并出现强烈搏动。若未观察到此现象，应考虑重复步骤4.4.2–4.4.3。</li>
	</ol>
	</li>
	<li>用4-0丝线结扎颈内动脉，以模拟低血流状态，并促进内膜增生。</li>
	<li>用生理盐水清洗伤口，逐层使用3-0聚乳酸羟基乙酸共聚物（polyglactin 910）缝合伤口。</li>
</ol>
5. 术后操作
<ol>
	<li>检查动物自主呼吸恢复后，停止麻醉并移除麻醉面罩。在动物从麻醉中恢复期间，需频繁检查其状况。</li>
	<li>在呼吸功能完全恢复前，将动物与其他动物隔离。必要时可进行人工辅助呼吸。&#160;在动物完全恢复前，不得将其放回群体饲养。</li>
	<li>麻醉恢复后，观察动物的进食和饮水情况，并提供适当的营养支持。给予镇痛药物（例如，皮下注射布托啡诺 0.05–0.2 mg/kg，每日2次，持续3天），并观察是否存在不适或疼痛的迹象。</li>

由于动脉粥样硬化导致缺血性疾病的患者数量在全球范围内不断增加1。尽管目前心血管疾病的药物和外科治疗已取得进展，但心肌梗死等缺血性心脏病仍是发病率和死亡率的主要原因2。此外，以外周肢体血流减少为特征的外周动脉疾病可引发严重肢体缺血，其中约40%的患者在确诊后6个月内失去肢体，死亡率高达20%3。
血管重建手术，如冠状动脉旁路移植术（CABG）和外周动脉旁路手术，是缺血性疾病的主要治疗手段。这些手术的目的是建立新的血流通道，以向动脉粥样硬化动脉发生狭窄或闭塞病变的远端部位提供充足的血流。尽管原位动脉移植物（如用于CABG的胸内动脉）因预期具有更长的通畅率而被优先选作旁路移植物，但由于静脉移植物（如自体大隐静脉）更容易获取和获得，因此在临床上更为常用4。静脉移植物的薄弱之处在于其通畅率较动脉移植物差5，这是由于在承受动脉压力时发生加速的内膜增生，从而导致静脉移植物病变6。
静脉移植物病变的发展经历以下三个阶段：1）血栓形成；2）内膜增生；3）动脉粥样硬化7。为了应对静脉移植物病变，已开展了大量基础研究8。迄今为止，根据最新指南，在冠状动脉或外周血管重建手术后的二级预防中，除抗血小板治疗和降脂治疗外，尚无其他推荐的药物治疗策略9,10,11,12。因此，为了克服静脉移植物病变，特别是内膜增生，有必要建立一个相关的实验平台以支持进一步的研究。
内膜增生是一种适应性现象，发生在血管周围环境改变时，血管平滑肌细胞（VSMCs）在内膜中增殖、积聚并产生细胞外基质。因此，它构成了移植物粥样硬化的基础7。在增生的内膜中，VSMCs 表现出增殖和基质生成能力，而非收缩功能，这一现象被称为“表型转换”8。调控静脉移植物中 VSMCs 的表型以预防静脉移植物病变，是当前研究的关键目标，已有大量基础研究围绕这一主题展开8。然而，一项旨在通过药物手段调控 VSMC 表型的随机对照临床试验仅显示出有限的效果13。此外，目前尚无标准化的治疗方法用于预防内膜增生。未来仍需开展更多包括动物模型研究在内的基础研究。
为了推动该领域的研究，建立一种能够模拟在动脉血压条件下静脉移植物的动物模型至关重要，以便进行长期的术后观察。Carrel 等人建立了将犬的颈外静脉植入颈总动脉的动物模型14。此后，多种静脉移植物模型被用于研究动脉血压改变对生理和病理过程的影响，包括将下腔静脉移植至胸主动脉或腹主动脉，或将大隐静脉移植至股动脉15,16,17。这些模型通常在猪或犬等较大型动物中建立，适用于模拟临床情况下的静脉移植物病变。然而，对于希望开发通过体内调控血管平滑肌细胞（VSMC）表型来减轻内膜增生新治疗策略的研究人员而言，若能建立一种无需特殊外科技术且成本较低的动物模型则更为理想。最初，兔颈静脉移植至颈动脉的模型被引入神经外科领域18,19。随后，该模型被应用于内膜增生的研究20,21。最初的模型仅包含静脉移植步骤，因而节省了操作时间。此外，后续研究显示，静脉移植物的制备方式本身也会影响内膜增生的程度22。Davies 等人评估了在兔静脉移植模型中球囊导管损伤对内膜增生的影响23,24。尽管在静脉移植基础上增加球囊损伤更接近临床实际情况，但仍需要更可重复的模型。因此，Jiang 等人研究了不同血流环境对内膜增生的影响，并建立了远端分支结扎术作为可重复性更高的模型25。然而，他们在进行静脉移植时采用了套管技术，这与临床中手工缝合吻合的方式有所不同。在本实验方案中，我们报道了一种可重复、具有临床相关性且易于推广的兔静脉移植模型制备方法，用于评估在动脉血压条件下内膜增生的情况。

A Rabbit Venous Interposition Model Mimicking Revascularization Surgery using Vein Grafts to Assess Intimal Hyperplasia under Arterial Blood Pressure

本方案为静脉内膜增生领域的研究人员提供了一种简便、经济、可重复且具有临床相关性的模型。本方案在经济性、临床适用性和技术可行性方面均具有良好的平衡，因此强烈推荐给希望将体外实验结果应用于体内环境的研究人员。我们认为，我们的方案可应用于更大的动物模型，以开发缺血性心脏病和下肢疾病的新治疗策略。本方案主要适用于心血管外科领域，但也可能用于解决终末期肾病患者动静脉瘘的退行性改变问题。局部麻醉并预防性给予抗生素镇痛药后，用10%聚维酮碘消毒手术部位。在麻醉后的日本大耳白兔颈部区域，使用手术刀做一条50至60毫米的切口。钝性分离皮下组织和筋膜，暴露20至30毫米长的颈静脉节段。使用4-0丝线结扎暴露静脉的所有分支。用2-0丝线环绕颈内静脉和颈外静脉，并在静脉远端侧的静脉壁上做一个1毫米的切口。将一根2-French球囊导管从切口处向静脉近端方向插入。在颈静脉远端位置结扎2-0丝线缝合线。用200 µL空气充盈球囊，并利用球囊反复剥离静脉内膜3次，以实现内皮细胞脱落。第三次传代后，结扎静脉近端，并在远端位置切断静脉。然后将20号静脉导管从远端向近端插入所获取的静脉，并在近端位置切断静脉。为了将颈动脉与获取的颈静脉进行移植连接，需暴露同侧颈动脉20至30毫米的血管段。仔细将动脉与附近的静脉和神经分离。用4-0丝线结扎暴露的静脉的所有分支。静脉注射肝素钠，剂量为每千克体重200国际单位。用外科橡胶夹夹闭动脉的近端和远端，然后在两个夹子之间切断动脉。分别从近端和远端向切开的颈动脉内注入生理盐水，以扩张动脉。将获取的静脉置于动脉附近，以反向端对端方式吻合血管。将静脉近端与动脉远端吻合时，在对应位置及对侧位置放置两针8-0聚丙烯锚定缝线。在锚定缝线之间的吻合线上方添加缝线。将动脉和静脉移植物翻转倒置，然后在吻合线的剩余部分添加缝线。接下来，从静脉中移除静脉导管，并在近端夹闭静脉移植物。远端松开颈动脉。观察静脉移植物是否逐渐扩张。使用8-0聚丙烯间断缝线，将静脉的远端与动脉的近端进行吻合。然后松开动脉夹，检查吻合口部位是否有出血。最后，用4-0丝线结扎颈内动脉，以模拟血流远端流出道不良的情况，并促进内膜增生。随后用生理盐水清洗伤口。用 3-0 聚乙醇酸乳酸缝线逐层缝合切口，并在将兔子放回其饲养笼之前，监测其完全恢复。在此代表性图像中，可在静脉移植术后 2 周观察到成功的内膜增生。在此，可以观察到载有 microRNA-145 的聚乳酸-羟基乙酸共聚物（poly(lactic-co-glycolic acid)）纳米颗粒对内膜增生形态的治疗效果。事实上，使用载有 microRNA 的聚乳酸-羟基乙酸共聚物纳米颗粒进行治疗，可显著减轻内膜增生。此外，对细胞增殖标志物 Ki-67 的免疫染色显示，经 microRNA-145 处理组中的 Ki-67 阳性细胞数量减少，表明血管平滑肌细胞的表型由未成熟的增殖状态转变为成熟的收缩状态。本方案的主要目标是确保移植移植物的通畅性。此外，在颈动脉去夹闭后确认移植物能够充分扩张也至关重要。
