所有操作均按照以下规定进行 &#34;实验动物护理和使用的指南&#34; （美国实验动物资源研究所，第8版，2011年）制定的标准，并经德国柏林州卫生与社会事务局（Landesamt für Gesundheit und Soziales (LaGeSO)）动物护理与使用委员会批准（方案编号：G0030/18）。首先，通过手术在体重约100克（bw）的幼年Sprague-Dawley大鼠中诱导充血性心力衰竭（见 材料表通过在升主动脉放置内径为0.8 mm的钛夹（主动脉缩窄，AoB）进行操作，具体方法如前所述29,30术后第3周（AoB后）图1），进行去束带（Deb）手术以移除主动脉上的夹子。AoB大鼠中肺动脉高压逆转的手术操作步骤及验证方法如图所示 图1.
1. 手术准备
<ol>
	<li>通过高压灭菌法对所需手术器械（图2）进行灭菌。</li>
	<li>术前30分钟，通过腹腔注射给予大鼠卡洛芬（5 mg/kg 体重）（参见材料表）以实现镇痛。</li>
	<li>通过腹腔注射氯胺酮（87 mg/kg 体重）和赛拉嗪（13 mg/kg 体重）对大鼠进行麻醉。</li>
	<li>使用电动剃刀去除动物颈部和胸部的毛发。</li>
	<li>涂抹一滴眼膏以保护手术期间的眼睛。</li>
	<li>将大鼠仰卧位固定于无菌手术台上，并用胶带小心固定其腹部和四肢。 
	注意：为维持体温，可在手术台下方放置37 °C加热垫。避免头部区域加热，以防眼睛干燥。</li>
	<li>使用聚维酮碘/碘附溶液对动物皮肤进行消毒。注意初次嗅脑手术的瘢痕和缝合线，并铺巾以隔离手术区域。</li>
	<li>通过夹捏脚趾确认麻醉深度。 
	&#8203;注意：手术过程中需定期监测麻醉深度。</li>
</ol>
2. 气管切开术与机械通气
注意：手术过程中，在接触非无菌设备后应更换手套。
<ol>
	<li>使用精细剪刀（图2A），在颈部正中做一条7-10 mm长的切口（图3A）。</li>
	<li>借助一对钝头镊子（图2B'），分离颈部软组织以暴露舌骨下肌群。沿正中切开肌肉以显露气管。剪断并移除初次AoB手术时的缝线。</li>
	<li>使用斜角Noyes弹簧剪刀在两个软骨环之间做约2 mm的气管切口（图2C，3B）。将外径为2 mm的气管插管（图2D）插入气管，并用4-0丝线固定（图2E，3C）。</li>
	<li>将气管插管连接至机械呼吸机（参见材料表），同时尽量减少死腔（图3D-E）。术中维持肺通气，呼吸频率为90次/分钟，潮气量（Vt）为8.5 mL/kg体重。</li>
</ol>
3. 主动脉去束缚
<ol>
	<li>使用精细剪刀在第二和第三肋骨之间做一个约20 mm长的皮肤切口（图3F）。</li>
	<li>借助较小的手术剪刀（图2F），小心地逐层分离并剪开肌肉组织（图3G）。沿第二和第三肋骨之间的肋间隙做一条10 mm的侧向切口。 
	注意：需小心接近胸骨中线，以避免出血。</li>
	<li>使用肋骨牵开器（图2G）扩张第二和第三肋骨之间的肋间隙，以形成手术窗口（图3H）。</li>
	<li>借助钝头镊子（图2B、B'），小心地将胸腺从心脏和动脉导管上分离，以暴露带有夹子的主动脉（图4A）。</li>
	<li>用镊子夹住夹子，并小心清除其周围的结缔组织，以充分暴露夹子。 
	注意：避免用镊子夹持或提起主动脉，以免损伤主动脉导致出血和致命后果。</li>
	<li>借助持针器（图2H），打开夹子（图4B），并将其从胸腔中移除。</li>
	<li>在关闭胸腔前，解除肺不张，确保肺充分复张且无过度膨胀，继续以9.5 mL/kg体重的潮气量进行机械通气10分钟，随后恢复至8.5 mL/kg体重的潮气量，以促进肺复张并消除可能存在的气胸。</li>
	<li>使用4-0丝线行单纯间断缝合关闭深层肌肉组织，然后使用单纯连续缝合连接浅层肌肉和皮肤（图5A、B）。</li>
</ol>
4. 气管拔管
<ol>
	<li>将气管插管从呼吸机上断开。仔细观察大鼠，直至自主呼吸恢复。如果动物在断开连接后未能自主呼吸，请重新连接呼吸机并继续通气5分钟，然后重复该操作。</li>
	<li>自主呼吸恢复后，将插管从气管中取出，并用海绵头清洁气管周围的液体（图2I）（参见材料表）。</li>
	<li>使用6-0 Prolene缝线以单纯缝合法关闭气管（图2E' 和 图5C）。然后使用4-0丝线以单纯间断缝合法缝合甲状舌骨下肌群（图5D），并以单纯连续缝合法缝合皮肤（图5E）。操作过程中使用聚维酮碘/碘伏溶液对肌肉和皮肤进行清洁和消毒。</li>
</ol>
5. 术后护理
<ol>
	<li>手术完成后，小心将动物转移至配备辅助氧气和红外线灯的恢复笼中，以在恢复期间保持动物温暖并提供充足的氧气。将氧气面罩靠近大鼠的鼻部。每次恢复笼中仅放置一只动物。</li>
	<li>动物苏醒后，小心将其转移至配备水和食物的普通笼子中。在接下来的12小时内，每2小时检查一次手术动物的健康状况。</li>
	<li>手术完成后，通过腹腔注射卡洛芬（5 mg/kg 体重）每日给予镇痛处理，持续一周。</li>
	<li>为避免细菌感染，术后一周内在饮水中添加阿莫西林（500 mg/L）。</li>

心力衰竭是发达国家死亡的首要原因，预计在未来十年内将增加25%。肺动脉高压（PH）——肺循环血压的病理性升高——影响约70%的终末期心力衰竭患者；世界卫生组织将其分类为由左心疾病引起的肺动脉高压（PH-LHD）1。PH-LHD起始于左心室（LV）收缩和/或舒张功能受损，导致充盈压升高，血液被动淤积进入肺循环2。尽管初期具有可逆性，但PH-LHD会因肺循环各部分（即动脉、毛细血管和静脉）发生主动的肺血管重构而逐渐变为固定性病变3,4。可逆性和固定性肺动脉高压均会增加右心室后负荷，初期引起代偿性心肌肥厚，但最终导致右心室扩张、运动减弱、纤维化和失代偿，进而逐步发展为右心室功能衰竭1,2,5,6。因此，肺动脉高压会加速心力衰竭患者的疾病进展并增加死亡率，尤其是在接受左心室辅助装置（LVAD）植入和/或心脏移植手术治疗的患者中更为显著7,8,9。目前尚无能够逆转肺血管重构过程的治愈性疗法，因此需要在合适的模型系统中开展基础机制研究。
重要的是，临床研究表明，肺动脉高压-左心疾病（PH-LHD）作为主动脉瓣狭窄患者常见的并发症，在主动脉瓣置换术后早期可迅速改善10。类似地，在一项为期5年的随访研究中发现，尽管术前肺血管阻力（PVR）较高（&#62;3 Wood单位），但若其对硝普钠具有可逆性，则在心脏移植后可实现持续性正常化11。同样，通过使用可植入的脉动性和非脉动性心室辅助装置减轻左心室负荷，可在数月内有效降低左心疾病（LHD）患者可逆性和固定性肺血管阻力，并改善右心室功能12,13,14。目前，驱动肺循环和右心室心肌逆向重构的细胞与分子机制尚不明确。然而，对其机制的理解可能为揭示可用于治疗性干预肺血管和右心室重构的生理通路提供重要线索，不仅适用于PH-LHD，也适用于其他类型的肺动脉高压。
一种合适的、能充分模拟PH-LHD病理生理学和分子特征的临床前模型，可用于研究因大鼠主动脉缩窄术（AoB）引起的压力超负荷型充血性心力衰竭的转化医学研究。4,15,16与小鼠横主动脉缩窄（TAC）模型中由压力超负荷引起的心力衰竭相比17，对AoB大鼠升主动脉在主动脉根部上方进行缩窄，不会导致左颈总动脉出现高血压，因为缩窄部位位于左颈总动脉自主动脉分出处的近端。因此，AoB不会引起大脑皮层左侧的神经元损伤，而后者是TAC的典型特征18，这些因素可能影响研究结果。与其他通过手术诱导PH-LHD的啮齿类动物模型相比，大鼠模型尤其是主动脉缩窄术（AoB）模型，表现出更强的稳定性和可重复性，并能更好地模拟PH-LHD患者肺循环的重构特征。同时，围手术期死亡率较低。19主动脉缩窄大鼠的左心室压力升高和左心室功能障碍可诱导肺动脉高压-左心疾病（PH-LHD）的发生，进而导致右心室压力升高&#160;和 RV&#160;重塑。因此，AoB 大鼠模型已被多个独立研究团队（包括我们自己）在一系列既往研究中证实为识别肺血管重塑的病理机制以及检验 PH-LHD 潜在治疗策略的极为有用的工具。4,15,20,21,22,23,24,25.
在本研究中，采用AoB大鼠模型建立主动脉去束带手术方法，以研究肺血管和右心室逆向重构的机制。此前，已建立小鼠26和大鼠27的主动脉去束带心肌逆向重构模型，用于探究调控左心室肥厚消退的细胞与分子机制，并评估促进心肌功能恢复的潜在治疗策略。此外，少数早期研究探讨了主动脉去束带对大鼠PH-LHD的影响，结果表明主动脉去束带可能逆转肺小动脉的中膜肥厚，使pre-pro-内皮素1的表达恢复正常，并改善肺血流动力学27,28，为心力衰竭大鼠肺动脉高压可逆性提供了证据。本研究对去束带手术的技术流程进行了优化和标准化，例如采用气管切开术替代经气管插管，以及使用具有确定内径的钛夹而非带钝针的聚丙烯缝线进行主动脉束带26,27，从而实现对手术过程更佳的控制，提高模型的可重复性，并改善动物存活率。
从科学角度来看，PH-LHD去带状模型的意义不仅在于证明了心力衰竭中心血管和肺部表型的可逆性，更重要的是鉴定出触发和/或维持肺动脉逆向重构的分子驱动因素，这些分子可作为未来治疗靶向的有前景候选目标。

A Model of Reverse Vascular Remodeling in Pulmonary Hypertension Due to Left Heart Disease by Aortic Debanding in Rats

由于左心疾病引起的肺动脉高压（PH-LHD）是最常见的肺动脉高压类型，但其病理生理机制相较于肺动脉高压（PAH）尚不明确。因此，目前尚无获批用于治疗或预防 PH-LHD 的有效疗法。用于治疗 PAH 患者肺动脉高压的药物不推荐用于 PH-LHD 的治疗，因为在左侧充盈压升高的情况下，降低肺血管阻力（PVR）并增加肺血流量可能导致左心失代偿和肺水肿。需要开发新的治疗策略以逆转 LHD 患者的肺动脉高压。与 PAH 不同，PH-LHD 是由于左心衰竭时血液在肺循环中淤积所导致的机械负荷增加而引发的。临床上，在主动脉瓣狭窄患者中通过主动脉瓣置换术，或在终末期心力衰竭患者中植入左心室（LV）辅助装置，可实现左心室的机械性减负荷，不仅可使肺动脉压和右心室（RV）压力恢复正常，还可使 PVR 恢复正常，从而为肺血管系统的逆向重构提供了间接证据。本研究利用一种已建立的由压力超负荷诱发左心衰竭进而发展为 PH 的大鼠 PH-LHD 模型，构建了用于研究该生理逆向重构过程分子与细胞机制的模型。具体而言，实施了主动脉去束带手术，导致左心室心肌发生逆向重构并实现减负荷。与此同时，右心室收缩压完全恢复正常，右心室肥厚显著改善但未完全逆转。该模型可能成为研究肺循环及右心室生理逆向重构机制的重要工具，旨在为开发治疗 PH-LHD 及其他类型肺动脉高压的治疗策略提供依据。

首先，通过在AoB动物中进行去束带手术前后的心脏超声检查，确认了主动脉去束带的成功（图6）。为此，在胸骨旁长轴（PLAX）B模式下观察主动脉弓。在AoB动物中可见升主动脉上的夹闭夹位置，而去束带（Deb）手术后该夹闭夹消失（图6A、B）。随后，采用脉冲波多普勒成像评估主动脉血流（图6C-F）。AoB动物在夹闭前后测得的峰值血流速度分别为733.24 ± 17.39 mm/s和5215.08 ± 48.05 mm/s（n = 8只动物）（图6C、E），表明在AoB部位存在显著的压力梯度。去除夹闭夹后，在相应主动脉位置的峰值血流速度分别为1093.79 ± 28.97 mm/s和2578.73 ± 42.27 mm/s，显示压力梯度明显减弱，符合功能性去束带效果（...
