该研究方案经广州医科大学附属第二医院临床研究与应用伦理委员会审查并批准。研究内容和方法符合医学伦理规范和要求。术前已向患者告知本研究的目的、背景、流程、风险及获益情况。患者知悉参与本研究为自愿行为，并签署了知情同意书。
1. 患者体位、器械及穿刺孔位置
<ol>
	<li>将患者置于手术台上，取仰卧位并调整为30°反向特伦德伦堡位，在手术过程中再向右侧倾斜30°，术者位于患者右侧进行操作。</li>
	<li>术中采用五孔法，使用三个10 mm套管、一个5 mm套管以及一个12 mm套管用于牵拉和标本取出。随后装配以下止血设备：30°腹腔镜、腹腔镜超声设备以及基本腹腔镜器械，包括单腔导管、超声刀、单极电凝器、血管夹和电动腔内切割吻合器（见材料表）。</li>
	<li>通过静脉给予丙泊酚150 mg、舒芬太尼15 µg和罗库溴铵50 mg，联合使用静脉与吸入麻醉，90秒后行7.5 F气管插管。 
	注意：麻醉药物的选择及静脉与吸入麻醉的实施由麻醉医师根据具体病例决定并执行。
	<ol>
		<li>在进行肝实质离断时，将中心静脉压降低3–5 cm H2O以减少肝静脉出血，同时尽可能限制液体补充。在膀胱和胃部分别置入导管和鼻胃管（见材料表），用于记录尿量和进行减压。</li>
	</ol>
	</li>
	<li>常规放置术前动脉导管和中心静脉（颈内静脉）导管。在脐下2 cm处使用10 mm套管作为观察孔。通过注入二氧化碳建立气腹，维持腹腔内压力在12–14 mmHg（1 mmHg = 0.133 kPa的1/4）。</li>
	<li>其余四个套管按以下位置放置：5 mm套管置于右侧腋前线，12 mm套管置于右侧锁骨中线肋缘下，10 mm套管置于左侧腋前线，另一个10 mm套管置于左侧锁骨中线肋缘下（图2）。</li>
</ol>
2. 手术技术
<ol>
	<li>将胆囊底部向上牵拉，并使用超声刀（参见 材料表用于胆囊三角（Calot's triangle）18&#160;分离。使用中号止血夹妥善结扎胆囊管和胆囊动脉，不要切断胆囊，并保留胆囊 原位 主要地（图3).</li>
	<li>使用超声刀分割圆韧带和镰状韧带。仔细分离左侧冠状韧带和三角韧带，避免损伤邻近的膈静脉分支。图4然后，沿肝胃韧带向小网膜腔内侧切开10 mm。</li>
	<li>通过从附着的14 F单腔导管上取下的吊带，从左至右穿过肝十二指肠韧带，以备阻断肝门入肝血流图5).</li>
	<li>用中号血管夹结扎左肝动脉后，需确保第一肝门已被单腔导管阻断，以防止在游离左肝蒂过程中发生意外出血。
	<ol>
		<li>轻轻抬起肝脏下缘，通过Glisson鞘的Laennec膜间隙游离左侧Glisson蒂19 和肝门板20 (图6），然后准备止血带系统，通过左肝Glissonean蒂插入8 F单腔导管以阻断左肝入肝血流21 (图7).</li>
	</ol>
	</li>
	<li>在松开第一肝门后，使用止血夹夹闭单腔导管以阻断左肝蒂图8），在肝实质离断后进行缝合。通过观察左肝叶缺血情况，确定左、右肝叶的分界。
	<ol>
		<li>沿分界线，使用腹腔镜超声探查并标记肝中静脉的投影位置。注意描绘重要肝内血管的位置和走行轨迹，尤其是位于预期肝实质横切平面的血管图9). 
		注意：本研究中，超声设置为彩色多普勒血流成像（CDFI）模式，将沿超声探头长轴方向流入下腔静脉的薄壁血管判定为肝中静脉（MHV）；此外，该血管也位于分界线附近。</li>
	</ol>
	</li>
	<li>沿肝脏表面的分界线，使用电钩标记实质切开线（图10）。请助手牵拉胆囊底部，沿肝中静脉从足侧向头侧使用超声刀离断肝实质图11).
	<ol>
		<li>早期启动超声刀以有效减少肝实质出血，操作时避免钳夹刀头，以防损伤血管。若在肝实质切除过程中发现较大的肝内血管或胆管，必要时使用2-0缝线进行结扎或缝合固定。</li>
	</ol>
	</li>
	<li>暴露流入4a/4b段及左肝静脉更上方断面处的血管蒂图12）。然后，充分暴露MHV及其属支，并在后续步骤中解剖LHV和MHV的根部图13最后，当暴露时，使用电动加强型钉合器钉合流入道和流出道血管。</li>
	<li>肝脏标本从右肝叶残余部分分离后，在缝合前沿切面进行止血及胆汁渗漏检查。
	<ol>
		<li>使用单极电凝（参见 材料表发现切面有出血点时用于止血（图14）。包裹切除的左肝叶（图15) 放入塑料袋中，通过下腹部4厘米长的切口取出，并放置两根引流管。</li>

3. 术后护理
<ol>
	<li>术后第一天，拔除鼻胃管，给予患者流质饮食。</li>
	<li>术后第二天拔除导尿管，并协助患者下床进行日常活动。</li>
	<li>最后，当每日引流量少于 50 mL 时，分别于术后第四天和第五天拔除两根引流管。嘱患者于 1 个月后返院复诊。</li>

解剖性肝切除术是一种可同时切除病灶及相应肝段及其属支静脉的手术，被认为是治疗肝癌的理想方法23,24,25,26随着技术进步，采用腹腔镜技术的解剖性肝切除术作为传统开腹肝切除术的替代方案已迅速发展，并得到广泛应用27该技术可减少术中出血，最大限度地保留功能性肝体积，降低患者并发症发生率，促进快速康复，并实现更优的长期临床疗效。尤其对于肝肿瘤患者，该技术在延长术后生存时间和无瘤生存时间方面具有积极效果。28,29然而，对于伴有潜在胆囊侵犯的巨大肝细胞癌患者，腹腔镜解剖性肝切除术具有很高的技术难度，尽管已有大量相关报道30,<sup class="xref...

肝细胞癌是一种常见的癌症，在成人中其发病率位居第六，是全球癌症相关死亡的第三大原因，且预计未来其发病率还将上升1。已有报道显示，手术切除、消融性电化学治疗、经动脉化疗栓塞、索拉非尼等全身治疗以及肝移植均为肝癌的有效治疗方式2,3。在这些治疗手段中，肝细胞癌（HCC）的手术切除被认为是主要的根治性治疗方法，因为该方法能够完全切除肿瘤，而非仅作局限性处理4。
腹腔镜手术是一种微创技术，与开腹切除术相比，围手术期并发症更少5，目前已在全球范围内取得显著进展，并逐渐成为肝脏外科的重要手术方式之一6,7,8。然而，在腹腔镜肝切除术中，由于术者无法通过直视明确肿瘤边界，且对难以实现有效腹腔镜下止血存在顾虑，大多数肝胆外科医生对开展此类高难度手术持谨慎态度。1960年，Lin等人报道了一例经肝内门静脉蒂结扎的右半肝切除术病例9。1986年，Takasaki也描述了Glisson蒂离断式肝切除术，称为“肝外鞘外分离法”10。1991年，Reich等人将腹腔镜技术应用于良性肝肿瘤的切除，完成了世界首例腹腔镜肝切除术11。自此以后，解剖性肝切除术逐步受到关注，为腹腔镜肝切除术的发展提供了重要的技术支撑。然而，在本研究所述病例中，肿瘤下缘已达胆囊板水平，单纯采用传统解剖性切除难以确保实现R0切除，而此类情况的处理策略在既往文献中鲜有详细报道。1999年，Neuhaus等人提出了门静脉主干整块切除的原则，证实其可作为良好预后的指标，并提高实现R0切除的可能性12。基于对肝脏解剖结构的进一步认识，我们提出了一种新的手术理念，即&#34;整块切除理念联合解剖性肝切除&#34;，本视频方案对此进行了详细展示。
本研究中，患者为一名67岁女性，因持续1个月的轻度上腹部疼痛于2021年8月入院。既往病史包括高血压和糖尿病。腹部增强CT显示肝S4段存在一大小为247 mm × 54 mm × 50 mm的肿块，呈不均匀强化，肿块下缘已达胆囊板，不能排除侵犯胆囊的可能（图1）。Child-Pugh肝功能13分级为A级，ICG清除率14,15 R15为5.1%（&#60;10%）。根据BCLC分期标准16，患者分期为A期；根据CNLC分期标准17，分期为IB期。经多学科讨论后，决定行腹腔镜左半肝切除术联合胆囊切除术。采用腹腔镜下整块切除联合解剖性肝切除的理念，以实现对巨大肝肿块的完整切除。

腹腔镜肝切除术中整块切除理念与解剖性切除的联合应用

Application of the En Bloc Concept Combined with Anatomic Resection in Laparoscopic Hepatectomy

腹腔镜肝切除术已在多项研究中被报道，是目前肝切除的主流方法。在某些特定情况下，例如肿瘤邻近胆囊床时，外科医生无法通过腹腔镜途径触诊确定手术切缘，从而导致对实现R0切除存在不确定性。传统上，手术通常先切除胆囊，再切除肝叶或肝段；然而，在上述情况下可能存在肿瘤组织播散的风险。为解决这一问题，基于对肝门部结构及肝内解剖的充分认识，我们提出一种独特的手术方法——将胆囊与肝脏进行整块解剖性切除，以实现联合肝切除与胆囊切除的同步完成。 原位首先，在解剖胆囊管后，不先行切断胆囊，而是通过单腔输尿管预阻断肝门；其次，沿Laennec膜与肝门板之间的间隙游离左肝蒂；第三，由助手牵拉胆囊底部，沿肝脏表面缺血线及术中B超定位，使用超声刀切除肝实质组织，使整个肝中静脉（MHV）及其属支完全显露；最后，离断左肝静脉（LHV），将标本自腹腔取出。肿瘤、胆囊及其他周围组织被整块切除，符合无瘤原则，实现了足够的切缘宽度和R0切除。因此，腹腔镜肝切除术结合整块切除理念与解剖性切除，是一种安全、有效且根治性的方法，术后复发与转移率低。

手术持续时间为255分钟，术中未观察到并发症，估计失血量少于20 mL。手术未中转为开腹手术，且未出现术后并发症。解剖性切除了肝2段、肝3段和肝4段（包括胆囊），并完全外周暴露了肝中静脉（MHV）及其属支（V5v，肝静脉第五段的腹侧支；V8v，肝静脉第八段的腹侧支）。肿瘤的石蜡病理学22检查结果证实为肝细胞癌，距阴性肝切除切缘2.2 cm（图16）。CT复查结果显示，残余肝实质内无异常不均匀强化（图17，术后6天评估；图18，术后1个月评估）。

这种微创方法更容易获得广泛的阴性切缘，并避免肿瘤组织扩散。该方法有助于最大限度实现R0切除，并避免围手术期并发症。该技术可用于涉及切除四至五个肝段的腹腔镜肝切除术。由于这种微创外科手术方法要求操作者具备开放手术的经验，因此建议接受适当的开放手术培训。完成患者术前准备后，将胆囊底部向上牵拉，使用超声刀进行胆囊三角的分离。需注意用中号止血夹妥善结扎胆囊管和胆囊动脉。避免切开胆囊，主要将胆囊原位保留。仔细分离左侧冠状韧带和三角韧带，避免损伤邻近的膈静脉分支。随后，通过从附着的14F单腔人用导管上取下的吊带，从左至右绕行肝十二指肠韧带后方，为肝门阻断做好准备。使用中等大小的血管夹结扎左肝动脉后，需确保第一肝门已被单腔导管阻断，以防止在游离左肝蒂过程中发生意外出血。轻轻抬起肝脏下缘，通过兰纳斯膜（Laennec膜）与肝门板间隙游离左格利森蒂，随后建立止血带系统，将一根8F单腔人体导管穿过左格利森蒂以阻断左肝入肝血流。在松开第一肝门阻断后，使用止血夹夹闭单腔导管以阻断左肝蒂，该导管将在肝实质离断后予以钉合固定。沿分界线使用腹腔镜超声检查寻找并标记肝中静脉的投影位置。注意描绘重要肝内血管的位置和走行，尤其是预期肝脏实质横切平面内的血管。在助手协助下，使用超声刀牵拉胆囊底部，并沿肝中静脉从足侧向头侧离断肝实质。早期启动超声刀以有效减少肝实质出血，避免夹闭刀头以防损伤血管。必要时，使用2-0缝线结扎肝实质切除过程中发现的较大肝内血管和胆管。现在，暴露位于切割线上的流入段4A、段4B及左肝静脉的血管蒂。然后完全暴露肝中静脉及其属支，并在后续步骤中解剖左肝静脉和肝中静脉的根部。当切面出现出血点时，使用单极电凝进行止血。将切除的左肝叶用塑料袋包裹。通过下腹部长约4厘米的切口将其取出，并放置两根引流管。图中显示了带有附着胆囊的切除肝组织标本。肿瘤的石蜡病理检查证实为肝细胞癌，距阴性肝切除切缘2.2厘米。术后六天及一个月后，剩余肝实质未见异常非均匀性强化。应注意避免切开胆囊，将胆囊原位缝合保留。同时通过兰内膜与肝门板之间的间隙游离左侧格利森蒂。该方法也可用于门静脉结扎，通过缝线阻断肝门部位。然而，阻断时间较难控制。
