研究在方案部分开始时遵循了机构指南。所有实验方案和操作均按照《赫尔辛基宣言》进行，并经过长庚纪念医院机构审查委员会的审查和批准。所有志愿者均被告知本研究的性质，并在参与前签署了知情同意书。整个实验过程所需的耗材见图1和图2，以及材料表中。
1. 制备mACS滴眼液所需材料
<ol>
	<li>准备 250 mL 的 10% 甘油溶液，并备好一个 21 G 蝴蝶翼型输液套管、一个不带针头的 3 mL 注射器，以及六支含有 158 USP 单位肝素的 10 mL 真空采血管（图 1）。</li>
	<li>将 21 G 血液采集针连接至 3 mL 注射器，并将 3 mL 的 10% 甘油溶液抽入已准备好的注射器中。 
	注意：在插入针头前，所有材料必须经过灭菌处理。</li>
	<li>按顺序将 10% 甘油溶液分装至各真空采血管中，每管约 0.5 mL（图 3A）。 
	&#8203;注意：由于试管内存在负压，针头插入后必须立即拔出，以确保将 3 mL 甘油溶液均匀分配至六个试管中。</li>
	<li>用 75% 酒精无菌棉签对患者的皮肤进行消毒。使用 18 G 血液采集针穿刺患者上肢浅表静脉，总共采集 60–70 mL 静脉血。</li>
</ol>
2. MACS滴眼液的制备
<ol>
	<li>将抽取的静脉血依次注入六个真空采血管中，每管10 mL（图3B）。 
	注意：此步骤依赖真空管内的负压完成充填。为避免血细胞破裂和溶血，切勿施加正压。</li>
	<li>将六个真空采血管置于37 °C恒温培养箱中孵育4小时（图3C）。 
	注意：密封管内残留的负压可维持并稳定缺氧状态，该管此前已加入甘油。</li>
	<li>4小时后从培养箱中取出试管，在室温下以3,500 × g 离心10分钟。</li>
	<li>此时准备mACS提取所需材料，包括无菌滴眼液瓶、带针头的3 mL注射器、0.22 µm滤器、18 G针头及一副无菌手套（图2）。 
	注意：操作台面应使用75%酒精擦拭，以确保无菌环境。离心后的上清液即为mACS的半成品。</li>
	<li>将六个试管放置于试管架上，待完全离心结束后打开管盖（图3D）。 
	注意：此步骤无需无菌操作。</li>
	<li>佩戴无菌手套，使用带18 G针头的3 mL注射器逐管抽取mACS。 
	注意：此步骤中应小心避免吸取下层的血细胞层（图3E）。</li>
	<li>拔出针头，将其连接至0.22 µm滤器，并将23 G、1.5英寸的采血针连接至原3 mL注射器的下方出口（图3F）。</li>
	<li>轻柔推动针头，使液体通过0.22 µm滤器，注入已准备好的无菌滴眼液瓶中（图3G）。</li>
	<li>重复上述步骤，直至所有mACS均经过滤并储存于滴眼液瓶中（图3H）。</li>
	<li>制备好的mACS滴眼液短期使用可储存于4 °C；如需长期保存，则应置于-20 °C。 
	&#8203;注意：在4 °C下保存不得超过2周，在-20 °C下保存不得超过3个月9,19。</li>
</ol>
3. 离体 小鼠角膜上皮伤口愈合模型
注意：以下 离体 动物模型基于 Hung 等人关于角膜上皮机械损伤的既往经验20以下步骤应在显微镜下进行，以制作边界清晰且一致的角膜上皮损伤。
<ol>
	<li>用 3%–4% 异氟烷麻醉 C57BL/6 小鼠。将皮肤活检打孔器压在小鼠角膜中央，使上皮层留下一个浅表的圆形印迹，以形成均匀的伤口边缘。 
	注意：操作需轻柔，避免眼球破裂。</li>
	<li>在已确定的区域范围内，使用带有0.5 mm钻头的角膜锈环清除器，将角膜上皮层刮除至鲍曼层。</li>
	<li>建立 离体 机械性角膜损伤动物模型，摘取眼球并制备培养孔。
	<ol>
		<li>首先处死小鼠；然后轻压小鼠上下眼眶边缘，将镊子尖端伸入眼球后腔，取出眼球并用镊子夹住。</li>
		<li>用角膜剪剪断视神经及眶周软组织，以完整分离眼球。</li>
		<li>在96孔板中加入熔化的石蜡。迅速用镊子尖端在石蜡中打一个圆形孔，随后等待其凝固。</li>
	</ol>
	</li>
	<li>准备待测培养基：0.5% mACS、0.5% AS 作为对照，生理盐水作为阴性对照，以及杜尔贝科改良伊格尔培养基（DMEM）。 
	注意：mACS 是使用上述提到的方案获得的。</li>
	<li>对于 离体 培养时，将收获的眼球置于已准备好的96孔板中，每孔加入200 µL培养基。</li>
	<li>将96孔培养板放入37 °C、含5% CO₂的培养箱中2确保每24小时更换一次培养基。</li>
	<li>为确认依次的伤口愈合效果，每隔8小时通过荧光素染色在显微镜下监测上皮伤口面积。
	<ol>
		<li>用生理盐水溶解荧光素纸上的荧光素。</li>
		<li>将荧光素染料滴加至小鼠角膜中央，随后在显微镜下观察并记录。典型结果如图所示 图4.

自体血清（AS）在干眼病中的治疗作用最早由Fox等人于1980年代报道1。人们认为，自体血清具有润滑特性，并含有多种重要的上皮营养性生化成分，可模拟天然泪液，促进角膜上皮细胞的增殖。在过去几十年中，基于这一理论已开展了多项研究。其中的营养成分包括表皮生长因子（EGF）、维生素A、转化生长因子β（TGF-β）以及其他细胞因子。有趣的是，血清富含TGF-β和维生素A，这些成分被认为在表皮增殖过程中发挥关键作用2,3,4,5。此外，在治疗眼表疾病患者时，多项研究显示，自体血清滴眼液在患者自述症状改善、其他客观干眼参数6,7以及显微镜下发现（如细胞密度）等方面具有一定优势8。荟萃分析研究表明，使用自体血清滴眼液治疗可能有助于改善患者的症状，但仍缺乏长期疗效和观察数据9,10。
与自体血清（AS）不同，富血小板血浆（PRP）是在制备过程中添加抗凝剂，并通过进一步的差速离心及化学激活血小板而获得。与AS相比，PRP中含有多种化学物质和生长因子，例如转化生长因子-β（TGF-β）、血管内皮生长因子（VEGF）和表皮生长因子（EGF）。PRP也已应用于眼表疾病，并在缓解临床症状方面显示出益处11。
上皮缺损与炎症之间的关联十分复杂。值得注意的是，免疫病理生理学也是眼表疾病中的另一个重要问题。促炎性细胞因子（如 IL-1β 和 IFN-γ）被认为是炎症级联反应中的关键介质12。因此，基于对免疫机制的理解，为治疗开辟了新的途径。阻断这一炎症过程的策略，包括白细胞介素-1受体拮抗剂（IL-1Ra）及其他抗炎性细胞因子的产生，在眼表疾病中也可能发挥重要作用13,14,15。
自1998年以来，Orthokine作为一种已商业化的自体条件化血清（ACS），已临床应用于患有骨关节炎（OA）、类风湿性关节炎（RA）和脊柱疾病患者的治疗13。与AS和PRP相比，ACS的特有技术特征在于使用化学涂层玻璃珠以及低氧孵育来激活单核细胞16。理论上，通过对细胞施加生存应激，可促进其分泌更多的抗炎因子，从而获得更高浓度的关键免疫调节成分，包括IL-1Ra。已有研究报道，与AS相比，ACS在OA治疗中具有更优的疗效17。眼表疾病在某些方面与骨科炎症性疾病的免疫背景相似。因此，基于人源血液疗法在骨科领域取得的成功，ACS可能凭借其上皮趋向性和免疫调节特性，在临床实践中优于传统治疗方法。尽管ACS已在骨科炎症性疾病中广泛应用，但其在眼科领域的临床应用仍需进一步探索，而高昂的成本、文献支持的缺乏以及制备工艺缺乏标准化导致疗效差异，可能限制了其在眼科的推广。
