Gene Therapy

Gentherapie ist die Einf&#xFC;gung eines Gens in die Zellen eines Individuums, um schwere Krankheiten zu verhindern oder behandeln.
 Das eingef&#xFC;gte Gen mag eine gesunde Kopie eines mutierten Gens oder ein anderes Gen sein, das den Wirkungen eines fehlerhaften Gens entgegenwirkt.
 Gene k&#xF6;nnen in vivo verabreicht werden, direkt in eine Person, oder ex vivo, in Zellen, die dem K&#xF6;rper entnommen wurden und dann zur&#xFC;cktransplantiert werden.
 Das neue Gen wird unter Benutzung eines Vektors eingef&#xFC;gt, normalerweise ein modifizierter Virus, der in eine Zelle eindringen und die DNA im Kern abliefern kann.
 Manche Vektoren integrieren die DNA in das Genom, w&#xE4;hrend andere au&#xDF;erhalb des Chromosoms bleiben.

Die Gentherapie ist eine Technik, bei der ein Gen in die Zelle einer Person eingef&#xFC;gt wird, um eine schwere Krankheit zu verhindern oder zu behandeln. Das hinzugef&#xFC;gte Gen kann eine gesunde Version des Gens sein, das im Patienten mutiert ist. Es kann jedoch auch ein anderes Gen sein, welches das krankheitsverursachende Gen des Patienten inaktiviert oder ersetzt. Zum Beispiel kann bei Patienten mit schwerer kombinierter Immundefekt (SCID) aufgrund einer Mutation im Gen f&#xFC;r das Enzym Adenosin-Deaminase, eine funktionierende Version des Gens eingef&#xFC;gt werden. Die Zellen des Patienten k&#xF6;nnen das Enzym dann erneut herstellen und somit die potenziell t&#xF6;dliche Krankheit in einigen F&#xE4;llen wieder heilen.
 
 <h4>Das Einf&#xFC;hren von Genen</h4>
 
 Gene k&#xF6;nnen in die Zellen eines Patienten mit zwei Hauptmethoden eingef&#xFC;hrt werden. Es kann in vivo, also direkt in eine Person durch Injektion in spezifische Gewebe oder in den Blutstrom geschehen oder ex vivo. Dabei werden dem Patienten Zellen entnommen und wieder eingef&#xFC;gt, nachdem ihnen das Gen hinzugef&#xFC;gt wurde.
 
 Das Gen wird normalerweise in einen Vektor eingebaut, um in die Zellen des Patienten und dann in den Zellkern transportiert zu werden. Dabei werden h&#xE4;ufig virale Vektoren verwendet, die so modifiziert worden sind, dass sie keine Krankheiten verursachen, sondern nur das Gen in sich tragen. Wenn beispielsweise retrovirale Vektoren verwendet werden, wird das Gen zuf&#xE4;llig in das Genom der Person eingebaut, was zu einer stabilen Expression des eingef&#xFC;gten Gens f&#xFC;hrt. In anderen F&#xE4;llen werden beispielsweise adenovirale Vektoren verwendet, bei denen sich das Gen nicht in das Genom des Wirtes integriert, sondern nur vor&#xFC;bergehend exprimiert wird. Der Vektor ist auch so konstruiert, dass er eine Promotorregion enth&#xE4;lt, so dass das neue Gen in Boten-RNA (mRNA) transkribiert werden kann. Dann wird die mRNA in der Zelle in ein Protein &#xFC;bersetzt. Das Proteinprodukt, wie z.B die. Adenosin-Deaminase, dient dann zur Behandlung der Krankheit.
 
 <h4>Klinische Studien</h4>
 
 Die Gentherapie befindet sich noch im Anfangsstadium und birgt erhebliche Risiken. Das liegt zum Teil daran, dass die Integration und Aktivit&#xE4;t der eingef&#xFC;gten Gene nicht vollst&#xE4;ndig kontrolliert werden kann. Bei einigen fr&#xFC;hen Studien der Gentherapie f&#xFC;r eine andere Form von SCID beispielsweise entwickelten einige Patienten Krebs, woraufhin die Studien gestoppt wurden. Die Technologie wird jedoch st&#xE4;ndig verbessert. Bei schweren Krankheiten, f&#xFC;r die es keine andere wirksame Behandlung gibt, k&#xF6;nnen die Vorteile die Risiken &#xFC;berwiegen. Neben dem SCID gibt es weitere Krankheiten, bei denen die Gentherapie erfolgreich war, wie zum Beispiel bei einem Typ der vererbten Blindheit und bei behandlungsresistenten B-Zell-Leuk&#xE4;mien .

Gene therapy is a treatment approach that changes gene activity to help address disease. It focuses on the genetic cause of a problem rather than only easing symptoms. In this way, it is used in disease intervention.

One common strategy is to replace a faulty gene with a normal copy. Another strategy is to inactivate a gene that is causing harm. Gene therapy can also introduce a new gene to help cells work more normally.
