Clinical Encounter

A clinical encounter is an interaction between a patient and a healthcare professional to address health concerns, assess symptoms, provide care, or support prevention. During the encounter, the clinician gathers a medical history, performs a physical examination when appropriate, interprets diagnostic information, and uses clinical reasoning to guide recommendations or treatment. Effective communication and shared decision-making help align care with the patient’s needs, preferences, and risks. Clinical encounters generate essential information for diagnosis, care planning, follow-up, and medical documentation, making them central to patient care, clinical education, quality improvement, and health outcomes research.
