Kocherization

Kocherization is a surgical maneuver that mobilizes the duodenum and pancreatic head to expose the underlying retroperitoneum, making it important in abdominal surgery and trauma assessment. The surgeon incises the peritoneal reflection along the lateral border of the duodenum and carefully dissects these structures medially, creating access to the inferior vena cava, aorta, right renal region, and posterior duodenal tissues. This exposure supports evaluation and control of hemorrhage, assessment of pancreatic or duodenal injury, and operations involving the pancreaticoduodenal region. Its extent can be tailored to the clinical objective, from limited mobilization to broader retroperitoneal access.

The ValsalvaAnalyzer software includes functions to analyze continuous beat-to-beat electrocardiogram and blood pressure (BP) measurements recorded during the Valsalva maneuver (VM). The computed clinical biomarkers and estimated model outputs provide insight into sympathetic and parasympathetic regulation of heart rate and BP during the VM.

This protocol presents robotic duodenum-preserving total pancreatic head resection (R-DPPHRt) for intraductal papillary mucinous neoplasms (IPMNs). A video demonstrates the technique in a patient with main duct-IPMN, highlighting its feasibility as a minimally invasive, organ-preserving alternative to pancreaticoduodenectomy.
