Venous Bypass Graft

A venous bypass graft is a surgically placed segment of vein that reroutes blood around a narrowed or blocked artery, restoring circulation to tissues beyond the obstruction. Surgeons commonly harvest the patient’s saphenous vein, reverse or prepare it to accommodate arterial flow, and connect it above and below the diseased segment so blood can travel through an alternate pathway. Venous bypass grafts are used in procedures such as coronary artery bypass and lower-limb revascularization to treat ischemia and reduce symptoms or tissue injury. Long-term success depends on graft patency, while thrombosis, atherosclerosis, and intimal hyperplasia can contribute to graft failure.

In this protocol, a model of porcine orthotopic liver transplantation after static cold storage of donor organs for 20 h without the use of a veno-venous bypass during engraftment is described. The approach uses a simplified surgical technique with minimization of the anhepatic phase and sophisticated volume and vasopressor management.

The severe organ shortage has resulted in increased use of marginal kidney grafts for transplantation. This has triggered interest in alternative storage methods, since marginal grafts especially tolerate cold storage poorly. The technique of normothermic ex vivo kidney perfusion (NEVKP) represents a novel preservation method for kidney grafts prior to transplantation.

Neointimal hyperplasia is the primary cause of stenosis in arterialized veins. We propose a new protocol whereby the right external jugular vein is grafted using the cuff technique in the common carotid artery of Sprague Dawley rats. The survival rate was 100 % at the time point of sacrifice.
