Neuroma Management

Neuroma management is the assessment and treatment of symptomatic neuromas, abnormal nerve growths that can develop after peripheral nerve injury, surgery, or amputation. When axons attempt to regenerate without an appropriate pathway, disorganized nerve fibers and surrounding scar tissue may form, producing ectopic electrical activity, tenderness, and neuropathic pain. Management begins with clinical evaluation and may include desensitization, physical therapy, medications, or injections; persistent cases can require surgical revision or procedures that redirect nerve regeneration. Understanding neuroma biology helps clinicians distinguish nerve pain from other conditions and select treatments that reduce symptoms while preserving function.

Source: Zimmermann, S., et.al. Stromal Vascular Fraction-enriched Fat Grafting for the Treatment of Symptomatic End-neuromata. J. Vis. Exp. (2017)This video demonstrates the surgical procedure for neurolysis and stromal vascular fraction (SVF)-enriched fat grafting in the management of symptomatic neuroma. Neurolysis removes adhesion tissue to restore nerve gliding, while SVF-enriched fat grafting is applied around the nerve to create a protective barrier, support nerve healing, and enhance...

Source: Brakkee, E. M., et al. Surgery and Behavioral Testing in the Tibial Neuroma Transposition Model in Rats. J. Vis. Exp. (2023).This video demonstrates a procedure to generate a rat tibial neuroma transposition (TNT) model. After exposing the tibial nerve in the hind leg of an anesthetized rat, transect the nerve. Next, create a subcutaneous tunnel, thread the severed nerve through the tunnel, and suture it in place. In the absence of a target for reinnervation, disorganized axonal...

All animal procedures described here must be conducted in accordance with institutional animal ethics guidelines and approved by IACUC. All procedures must follow the principles of the 3Rs—Replacement, Reduction, and Refinement—and must be performed by trained personnel. Monitoring Animals for Signs of Pain and Distress Recognizing early signs of pain is essential for ethical animal research. Animals are routinely monitored using behavioral, physical, and physiological parameters that reflect...

This protocol describes the tibial neuroma transposition model, which entails a lesion of the tibial nerve with subsequent transposition of the proximal nerve end toward a subcutaneous pretibial or lateral position. Behavioral testing of neuroma pain and plantar hyperalgesia is quantified using Von Frey monofilaments.
