Bile Acid-binding Resins

Bile acid-binding resins are nonabsorbed anion-exchange drugs that reduce circulating bile acids and low-density lipoprotein (LDL) cholesterol, making them important tools in pharmacology. In the intestine, these resins bind bile acids and prevent their reabsorption, interrupting enterohepatic circulation; the liver then converts more cholesterol into bile acids and increases hepatic LDL-receptor activity. Clinically, they help manage hypercholesterolemia and bile acid-induced diarrhea and may relieve pruritus associated with cholestasis. Because they can bind other substances in the gut, bile acid-binding resins may reduce the absorption of coadministered medicines and fat-soluble vitamins, requiring careful dosing schedules.

Tools to diagnose bile acid malabsorption and measure bile acid transport in vivo are limited. An innovative approach in live animals is described that utilizes combined proton (1H) plus fluorine (19F) magnetic resonance imaging; this novel methodology has translational potential to screen for bile acid malabsorption in clinical practice.
