Liver Perfusion Protocol

A liver perfusion protocol is a controlled ex vivo method for flushing and processing liver tissue, enabling researchers to examine hepatic cells, immune populations, and pathogen-associated responses under defined conditions. The procedure typically circulates a buffered solution through the portal circulation and hepatic vessels to remove blood, maintain tissue viability, and support enzymatic dissociation into hepatocytes and nonparenchymal cells. In immunology and infection research, this approach helps isolate Kupffer cells, liver sinusoidal endothelial cells, and infiltrating leukocytes for phenotyping, functional assays, and analysis of host–pathogen interactions. Standardized perfusion also improves sample reproducibility and supports studies of inflammation, immune cell trafficking, and hepatic disease mechanisms.

Here we present a protocol describing oxygenated ex situ machine perfusion of donor liver grafts. This article contains a step by step protocol to procure and prepare the liver graft for machine perfusion, prepare the perfusion fluid, prime the perfusion machine and perform oxygenated normothermic machine perfusion of the liver graft.

The porcine model of liver normothermic machine perfusion (NMP), described here, can be successfully used to study NMP as a preservation strategy, a tool for viability assessment, and a platform for organ repair. It holds a high translational value, however it is technically challenging and labor-intensive.
