Clinical Pathway Nursing

Clinical pathway nursing is the structured coordination of evidence-based care for patients with a specific condition, procedure, or stage of illness. It translates clinical guidelines into an organized sequence of assessments, nursing interventions, treatments, education, and discharge planning, with responsibilities and expected milestones defined across the care team. Nurses monitor patient responses, document progress, and identify variances when recovery differs from the anticipated pathway, prompting reassessment or modification of care. Used in hospitals and other healthcare settings, clinical pathways promote consistent, timely, and coordinated practice while supporting communication, resource management, patient safety, and evaluation of outcomes.
