Donor-specific Tolerance

Donor-specific tolerance is a state in which a recipient’s immune system selectively accepts cells, tissues, or organs from a particular donor while preserving responses to other foreign antigens. It can develop when donor antigens are presented under conditions that promote immune regulation, leading to mechanisms such as T-cell deletion, functional anergy, regulatory T-cell activity, or mixed hematopoietic chimerism. In transplantation medicine, this approach aims to reduce graft rejection and limit the need for lifelong, broadly acting immunosuppressive drugs. Understanding donor-specific tolerance may improve graft survival, decrease treatment-related complications, and support more precise therapies for organ and cell transplantation.

Single port laparoscopic surgery is changing the standard of care in surgical care like nothing since the laparoscopic technique was introduced 20 years ago. We present out technique of single port donor nephrectomy using the Gelpoint device. We have successfully performed this surgery in 100 patients.

We describe a valuable diagnostic assay that could potentially be used to decide the withdrawal of immunosuppression after transplant without elevated risk of graft rejection. The assay uses the principles of Delayed Type Hypersensitivity and provides accurate assessment of both donor specific effector and regulatory immune responses mounted by recipients.
