Negative Thoracic Pressure

Negative thoracic pressure is the subatmospheric pressure within the chest that helps drive normal breathing and influences circulation. During inspiration, contraction of the diaphragm and external intercostal muscles enlarges the thoracic cavity, lowering intrathoracic pressure and pulling air into the lungs; the resulting pressure gradient also promotes venous blood return to the heart. In medicine, understanding this mechanism supports assessment of respiratory function, interpretation of cardiopulmonary physiology, and use of negative-pressure ventilators, which assist breathing by generating subatmospheric pressure around the thorax rather than forcing air directly through the airway.

Assessing Cerebral Autoregulation via Oscillatory Lower Body Negative Pressure and Projection Pursuit Regression

Cerebral perfusion is maintained across a range of pressures via cerebral autoregulation. However, characterizing autoregulation requires prominent pressure fluctuations at regulated frequencies. The described protocol will show how oscillatory lower body negative pressure can generate pressure fluctuations to provide data for projection pursuit regression for quantification of the autoregulatory curve.

We describe a "gold standard" for evaluating orthostatic tolerance (OT) using tilt testing with combined lower body negative pressure (LBNP). This can be combined with non-invasive evaluations of cardiovascular reflex control. Normal and abnormal responses are defined.
