Question 1

How do indirect-acting cholinergic agonists increase acetylcholine levels in the body?

Accepted Answer

Indirect-acting cholinergic agonists, also called anticholinesterases, inhibit the breakdown of acetylcholine by blocking cholinesterase enzymes. This allows acetylcholine to remain active longer at synapses, binding repeatedly to receptors and potentiating cholinergic actions throughout the autonomic nervous system, neuromuscular junction, and central nervous system.

Question 2

Why are anticholinesterases effective for treating myasthenia gravis?

Accepted Answer

Anticholinesterases prolong acetylcholine activity at the neuromuscular junction by preventing its enzymatic breakdown. This continuous depolarization extends the end plate potential, allowing muscle fibers to respond more effectively to neural signals. The enhanced cholinergic transmission compensates for deficient neuromuscular transmission in myasthenia gravis patients.

Question 3

What cardiovascular effects result from anticholinesterase administration?

Accepted Answer

Anticholinesterases activate parasympathetic signaling in the cardiovascular system, inducing bradycardia and lowering blood pressure. These effects make them therapeutically useful for treating supraventricular tachycardia, a condition characterized by abnormally rapid heart rates that anticholinesterases help normalize through enhanced cholinergic transmission.

Question 4

How do anticholinesterases help treat glaucoma?

Accepted Answer

Anticholinesterases enhance cholinergic activity in the eye, promoting aqueous humor drainage and reducing intraocular pressure. By facilitating fluid outflow through parasympathetic activation, these agents effectively lower the elevated eye pressure characteristic of glaucoma, helping prevent progressive vision loss and optic nerve damage.

Question 5

What gastrointestinal and urinary effects do anticholinesterases produce?

Accepted Answer

Anticholinesterases promote smooth muscle contractions in the gastrointestinal and urinary tracts by enhancing parasympathetic cholinergic transmission. These effects make them effective for treating post-surgical bowel atony and neurogenic bladder, conditions involving impaired muscle function and reduced contractility in these organ systems.

Question 6

How do anticholinesterases that cross the blood-brain barrier benefit Alzheimer's disease patients?

Accepted Answer

Anticholinesterases that penetrate the blood-brain barrier augment cholinergic transmission in the brain, addressing the disrupted cholinergic signaling associated with Alzheimer's disease. At low doses, they improve cognitive function through an alerting response, though higher doses can cause confusion, tremors, and convulsions.

Question 7

What dose-dependent effects occur when anticholinesterases reach high concentrations?

Accepted Answer

At low doses, anticholinesterases cause initial excitation and improved cognitive function. However, higher doses produce depression, convulsions, tremors, and confusion. Excessive doses can lead to prolonged depolarization at the neuromuscular junction, causing muscle weakness, paralysis, respiratory failure, and potentially fatal coma.
