Orthotopic Position

Orthotopic position refers to the normal anatomical location of an organ, tissue, implant, or transplanted structure, and it matters because location can determine how that structure functions and interacts with surrounding tissues. In medicine, an orthotopic procedure places a graft or device at the site typically occupied by the corresponding native structure, allowing local blood supply, tissue architecture, and anatomical connections to guide integration; this contrasts with ectopic or heterotopic placement. Orthotopic transplantation and disease models can therefore reproduce clinically relevant anatomy, physiology, and patterns of disease progression, supporting surgical training, treatment evaluation, and more accurate assessment of therapeutic outcomes.

Two techniques can be used to establish this model: injection of a cancer cell suspension into the cecal wall or transplantation of a piece of subcutaneous tumor onto the cecum. This model is useful for studying the natural progression of colorectal cancer and testing new therapeutic agents against colorectal cancer.

Small bowel transplantation has become an accepted treatment option for patients with irreversible intestinal failure. Our experimental model of orthotopic small bowel transplantation in rats serves as a reliable tool to address underlying immunologic and inflammatory processes that complicate intestinal transplantation.
