Tibial Cortex Transport

Tibial cortex transport is a limb-reconstruction technique that moves a segment of the tibial cortex to restore bone continuity after segmental loss, often in complex nonunions or bone defects. The method uses a controlled corticotomy or osteotomy and gradual mechanical distraction, typically with an external fixation system, to transport a vascularized bone segment while new regenerate bone forms behind it; the segment ultimately docks with the opposing bone end. This approach can address substantial defects while preserving or restoring limb length and alignment. It is particularly relevant in orthopedic trauma and reconstructive medicine, where infection, impaired healing, or extensive tissue damage complicates conventional grafting.

Currently any kind of vascularized composite allotransplantation depends on long-term-immunosuppression, difficult to support for non-life-critical indications. We present a new porcine tibial VCA model that can be used to study bone VCA and demonstrate the use of surgical angiogenesis to maintain bone viability without the need of long-term immune-modulation.

The tibial nerve transection model is a well-tolerated, validated, and reproducible model of skeletal muscle atrophy. The model surgical protocol is described and demonstrated in C57Black6 mice.
