Flat Note

A flat note is a percussion sound produced during clinical examination when tapping an area over dense tissue or fluid, providing clues about underlying anatomy. Because dense materials transmit little air vibration, they generate a very soft, short, high-pitched sound with minimal resonance, unlike the hollow note produced over air-filled spaces. Clinicians use this finding to distinguish solid structures from air-containing organs and to identify changes such as fluid accumulation in the pleural cavity. Interpreting a flat note alongside other examination findings supports bedside assessment of organ boundaries, tissue density, and possible disease.
