Dull Note

A dull note is a low-pitched, muffled percussion sound produced when an examination site contains little air and absorbs or dampens vibrational energy. During clinical percussion, tapping the body surface generates sound and tactile feedback that vary with the density and composition of underlying tissues; fluid-filled or solid regions typically produce dullness rather than the resonant note of air-filled spaces. Clinicians use this finding to assess tissue boundaries and identify possible abnormalities, including lung consolidation, pleural fluid, or abdominal fluid accumulation. Interpreted alongside inspection, auscultation, and palpation, a dull note supports bedside diagnosis and guides further evaluation.

Source: Alexander Goldfarb, MD, Assistant Professor of Medicine, Beth Israel Deaconess Medical Center, MA Medical percussion is based on the difference in pitch between the sounds elicited by tapping on the body wall. The auditory response to tapping depends on the ease with which the body wall vibrates, and is influenced by underlying organs, strength of the stroke, and the state of the body wall. There are three main medical percussion sounds: resonance (heard over lungs), tympany (heard over...
