Epicardial Spasm

Epicardial spasm is a transient, intense constriction of a major coronary artery that reduces myocardial blood flow and can cause chest pain, ischemic electrocardiographic changes, or, in severe cases, arrhythmias. The spasm arises when vascular smooth muscle contracts abnormally, often in association with impaired endothelial regulation, producing temporary narrowing that may occur at rest and then resolve. Clinicians evaluate suspected cases through symptom patterns, electrocardiography, coronary imaging, and, when appropriate, pharmacologic provocation testing. Recognizing this mechanism distinguishes vasospastic angina from fixed coronary obstruction and supports treatment strategies aimed at preventing recurrent episodes and protecting cardiac function.

Here, we describe methods for isolating primary mouse epicardial cells by an outgrowth culture assay and assessing the functional migration of epicardial-derived cells (EPDC) using an ex vivo heart culture system. These protocols are suitable for identifying genetic and chemical modulators of epicardial epithelial-to-mesenchymal transition (EMT) and motility.

Vascular Spasm

The vascular phase, also known as vasospasm, is the initial stage of hemostasis, crucial for preventing excessive bleeding when a blood vessel is injured. After a vessel is cut, nerves in the damaged area trigger pain and other sensory impulses. Simultaneously, the smooth muscles in the vessel wall contract, resulting in a vascular spasm. This contraction reduces the vessel's diameter at the injury site, slowing or stopping blood loss through the vessel wall. Vascular spasms typically last for...

Myocardial lactate production (coronary arterial-venous difference in serum lactate level) during coronary spasm provocation testing is considered as a highly sensitive marker that reflects acetylcholine-induced myocardial ischemia due to microvascular spasm. This article presents the procedures to assess myocardial lactate production for the diagnosis of coronary microvascular spasm.
