Parathyroid Allotransplantation

Parathyroid allotransplantation is the transfer of parathyroid tissue between genetically different individuals to restore parathyroid hormone production and regulate calcium homeostasis. After implantation, donor cells must establish a blood supply and release parathyroid hormone in response to changing calcium concentrations, while the recipient’s immune system may recognize the graft as foreign and require immunosuppression. The approach is being investigated for severe or permanent hypoparathyroidism, particularly when conventional hormone replacement is inadequate or impractical. Successful graft function could reduce dependence on lifelong medication and improve control of calcium and phosphate balance, although immune rejection, graft survival, and procedural safety remain important challenges.

Currently any kind of vascularized composite allotransplantation depends on long-term-immunosuppression, difficult to support for non-life-critical indications. We present a new porcine tibial VCA model that can be used to study bone VCA and demonstrate the use of surgical angiogenesis to maintain bone viability without the need of long-term immune-modulation.

Two Techniques to Create Hypoparathyroid Mice: Parathyroidectomy Using GFP Glands and Diphtheria-Toxin-Mediated Parathyroid Ablation

Mice with acquired hypoparathyroidism would be useful for studying novel drug therapies for hypoparathyroidism. Two procedures to create such mice are demonstrated. The GFP-PTX mouse is generated by surgical parathyroidectomy guided by green fluorescing parathyroid glands. A second, non-surgical approach is based on parathyroid-specific expression of the diphtheria toxin receptor.

We present a reproducible mouse model of heterotopic hindlimb-to-neck vascularized composite allotransplantation. This supermicrosurgical protocol, involving 0.3-0.5 mm vessel anastomoses, integrates nylon filament guidance and venous cuffing. It provides a reliable platform for translational studies on tolerance, ischemia-reperfusion injury, and immunological mechanisms in vascularized composite allotransplantation.
