Preload Recruitable Stroke Work

Preload recruitable stroke work is a load-adjusted index of ventricular contractility that describes how effectively the heart converts filling into mechanical work. It is determined by changing preload, measuring stroke work from the pressure-volume loop, and relating it to end-diastolic volume; the slope of this relationship reflects myocardial systolic performance. Because the measure is less dependent on a single loading condition than conventional ejection-based indices, it can help assess ventricular function during changes in circulation. In medicine and cardiovascular research, preload recruitable stroke work supports evaluation of heart failure, myocardial dysfunction, cardiac interventions, and experimental treatments affecting contractility.

The video demonstrates a method for investigating the impact of stromal cells on leukocyte recruitment. In a parallel plate flow chamber model, pro-inflammatory cytokines induce the expression of adhesion receptors by endothelial cells, facilitating neutrophil recruitment and migration. The cross-talk between stromal cells and endothelial cells diminishes neutrophil recruitment to the endothelial cells.

The ability of inflamed endothelium to recruit leukocytes from flow is regulated by mesenchymal stromal cells. We describe two in vitro models incorporating primary human cells that can be used to assess neutrophil recruitment from flow and examine the role that mesenchymal stromal cells play in regulating this process.

The Frank-Starling-Sarnoff curve is clinically important and describes the relationship between cardiac preload and output. This report illustrates a novel method of simultaneous jugular venous and carotid arterial Doppler velocimetry as transient surrogates of cardiac preload and output, respectively; this approach is enabled by wireless, wearable Doppler ultrasound.
