Anastomotic Leak

Anastomotic leak is a postoperative complication in which a surgically created connection between two hollow organs fails to seal, allowing contents to escape into surrounding tissue or body cavities. It develops when anastomotic healing is disrupted by factors such as inadequate blood supply, tension, infection, or impaired tissue repair, and may trigger localized inflammation, abscess formation, or systemic infection. In medicine, understanding this complication supports risk assessment, postoperative monitoring, and timely diagnosis using clinical evaluation and imaging. Prevention and management strategies, including optimized surgical technique and appropriate intervention, can reduce morbidity, prolonged hospitalization, and the need for further surgery.

We describe a novel intrathoracic esophagogastric anastomotic technique designed to create a large diameter anastomosis while simultaneously maintaining conduit blood supply to minimize the incidence of anastomotic leaks and strictures. Construction and orientation of the stomach conduit designed to optimize upper gastrointestinal tract function is also described.
