Pancreatic Parenchymal Preservation

Pancreatic parenchymal preservation is a surgical principle that removes or treats diseased pancreatic tissue while retaining as much healthy gland as possible, helping maintain endocrine and exocrine function. Surgeons apply this approach through limited resections, enucleation, or other tissue-sparing procedures that target lesions while protecting the main pancreatic duct, blood supply, and surrounding parenchyma. It is particularly relevant for benign, premalignant, or low-grade lesions and for patients at risk of future pancreatic disease. Preserving functional tissue can reduce postoperative diabetes, pancreatic exocrine insufficiency, and other consequences associated with extensive resection, while supporting individualized treatment planning in pancreatic surgery.

This protocol presents robotic duodenum-preserving total pancreatic head resection (R-DPPHRt) for intraductal papillary mucinous neoplasms (IPMNs). A video demonstrates the technique in a patient with main duct-IPMN, highlighting its feasibility as a minimally invasive, organ-preserving alternative to pancreaticoduodenectomy.
