Pharmacist Led Management

Pharmacist-led management is a collaborative approach in which pharmacists assess, optimize, and monitor medication therapy as part of patient care. It typically involves reviewing prescriptions, reconciling medicines across care transitions, identifying drug interactions or adherence barriers, and adjusting treatment in coordination with prescribers while tracking therapeutic response and adverse effects. In medicine, this model supports safer and more effective care for patients with complex regimens, chronic conditions, or multiple healthcare providers. By integrating medication expertise with patient education and clinical follow-up, pharmacist-led management can improve treatment continuity, reduce preventable medication-related problems, and strengthen team-based decision-making.
