Segmental Liver Resection

Segmental liver resection is a surgical procedure that removes one or more anatomically defined liver segments while preserving as much functioning liver tissue as possible. Guided by the liver’s segmental vascular and biliary anatomy, the surgeon identifies and controls the segment’s portal inflow, hepatic venous drainage, and bile duct branches before transecting the parenchyma along the planned boundary. This limited hepatectomy is used primarily to treat selected primary liver tumors, metastases, and other localized lesions when complete removal is feasible. By combining oncologic clearance with parenchyma preservation, the procedure can support postoperative liver function and may expand surgical options for patients with limited hepatic reserve.

2025

We present a laparoscopic technique for anatomic S7+S8d resection with right hepatic vein transection, preserving the inferior right hepatic vein and segment S6. Relying on meticulous anatomical landmarks without intraoperative ultrasound, this approach offers a precise and parenchyma-sparing option for dorsally located right-liver tumors, particularly in settings with limited advanced navigation resources.
