Nurse-led Discharge

Nurse-led discharge is a structured process in which appropriately trained nurses coordinate a patient’s transition from hospital to home or another care setting, supporting safe, efficient continuity of care. It typically involves assessing clinical readiness against established criteria, reviewing medications and warning signs, providing self-care instructions, confirming follow-up arrangements, and communicating essential information to patients, families, and community providers. In medicine, nurse-led discharge can reduce delays, strengthen patient understanding, and promote adherence after hospitalization while allowing physicians to focus on patients requiring complex decisions. Its effectiveness depends on clear protocols, multidisciplinary communication, staff competency, and access to timely medical review when concerns arise.
