Numeric Rating Scale

The Numeric Rating Scale (NRS) is a brief self-report measure that converts a person’s perceived intensity of a symptom, most commonly pain, into a numerical score, making subjective experiences easier to record and compare. In neuroscience and clinical research, respondents typically select a whole number on an anchored scale, such as 0 for no pain and 10 for the worst imaginable pain; the score reflects perception rather than a direct neural signal. Researchers and clinicians use NRS ratings to characterize symptom severity, track change over time, assess responses to treatment, and relate subjective experience to behavioral, physiological, or neuroimaging findings.
