Renal Perfusion

Renal perfusion is the delivery of blood to the kidneys, a process essential for glomerular filtration, electrolyte balance, waste removal, and blood pressure regulation. It depends on renal vascular resistance and autoregulation, in which changes in afferent and efferent arteriole tone help maintain relatively stable renal blood flow and filtration across varying arterial pressures. In pharmacology, studying renal perfusion clarifies how vasoactive drugs, diuretics, nonsteroidal anti-inflammatory drugs, and renin-angiotensin system inhibitors influence kidney function. Measuring or modeling these effects supports therapeutic decisions, evaluation of drug-induced kidney injury, and understanding the relationship between hemodynamics and renal clearance.

The severe organ shortage has resulted in increased use of marginal kidney grafts for transplantation. This has triggered interest in alternative storage methods, since marginal grafts especially tolerate cold storage poorly. The technique of normothermic ex vivo kidney perfusion (NEVKP) represents a novel preservation method for kidney grafts prior to transplantation.
