Hier beschrijven we protocollen voor het verkrijgen van beelden van goede kwaliteit met behulp van nieuwe, niet-invasieve beeldvormingsapparaten van reflectantie confocale microscopie (RCM) en gecombineerde RCM en optische coherentietomografie (OCT). We maken clinici ook vertrouwd met hun klinische toepassingen, zodat ze de technieken kunnen integreren in reguliere klinische workflows om de patiëntenzorg te verbeteren.
Huidkanker is een van de meest voorkomende vormen van kanker wereldwijd. De diagnose is gebaseerd op visuele inspectie en dermoscopie gevolgd door biopsie voor histopathologische bevestiging. Hoewel de gevoeligheid van dermoscopie hoog is, resulteert de lagere specificiteit in 70% -80% van de biopsieën die worden gediagnosticeerd als goedaardige laesies op histopathologie (valse positieven op dermoscopie).
Reflectantie confocale microscopie (RCM) en optische coherentietomografie (OCT) beeldvorming kunnen niet-invasief de diagnose van huidkanker begeleiden. RCM visualiseert cellulaire morfologie in en-face lagen. Het heeft de diagnostische specificiteit voor melanoom en gepigmenteerde keratinocytische huidkankers verdubbeld ten opzichte van dermoscopie, waardoor het aantal biopsieën van goedaardige laesies is gehalveerd. RCM verwierf factureringscodes in de VS en wordt nu geïntegreerd in klinieken.
Beperkingen zoals de ondiepe diepte (~ 200 μm) van beeldvorming, slecht contrast voor niet-gepigmenteerde huidlaesies en beeldvorming in en-face lagen resulteren echter in een relatief lagere specificiteit voor de detectie van niet-gepigmenteerd basaalcelcarcinoom (BCC’s) – oppervlakkige BCC’s die grenzen aan de basale cellaag en diepere infiltratieve BCC’s. OCT daarentegen mist cellulaire resolutie, maar beeldt weefsel in verticale vlakken af tot een diepte van ~ 1 mm, wat de detectie van zowel oppervlakkige als diepere subtypen van BCC’s mogelijk maakt. Beide technieken zijn dus in wezen complementair.
Een “multimodaal”, gecombineerd RCM-OCT-apparaat brengt tegelijkertijd huidlaesies in beeld in zowel en-face als verticale modi. Het is nuttig voor de diagnose en het beheer van BCC’s (niet-chirurgische behandeling voor oppervlakkige BCC’s versus chirurgische behandeling voor diepere laesies). Een duidelijke verbetering in specificiteit wordt verkregen voor het detecteren van kleine, niet-gepigmenteerde BCC’s ten opzichte van RCM alleen. RCM- en RCM-OCT-apparaten brengen een belangrijke paradigmaverschuiving teweeg in de diagnose en het beheer van huidkanker; het gebruik ervan is momenteel echter beperkt tot academische tertiaire zorgcentra en sommige privéklinieken. Dit artikel maakt clinici vertrouwd met deze apparaten en hun toepassingen en pakt translationele barrières aan in de routinematige klinische workflow.
Traditioneel is de diagnose van huidkanker afhankelijk van visuele inspectie van de laesie, gevolgd door een nadere blik op verdachte laesies met behulp van een vergrotende lens die een dermatoscoop wordt genoemd. Een dermatoscoop biedt ondergrondse informatie die de gevoeligheid en specificiteit verhoogt ten opzichte van die van visuele inspectie voor het diagnosticeren van huidkanker 1,2. Dermoscopie mist echter cellulaire details, wat vaak leidt tot een biopsie voor histopathologische bevestiging. De lage en variabele (67% tot 97%) specificiteit van dermoscopie3 resulteert in valse positieven en biopsieën die goedaardige laesies op pathologie blijken te vertonen. Een biopsie is niet alleen een invasieve procedure die bloedingen en pijn veroorzaakt4, maar is ook zeer ongewenst op cosmetisch gevoelige gebieden zoals het gezicht als gevolg van littekens.
Om de patiëntenzorg te verbeteren door bestaande beperkingen te overwinnen, worden veel niet-invasieve, in vivo beeldvormingsapparatuur onderzocht 5,6,7,8,9,10,11,12,13,14,15,16,17,18 . RCM- en OCT-apparaten zijn de twee belangrijkste optische niet-invasieve apparaten die worden gebruikt voor het diagnosticeren van huidlaesies, met name huidkanker. RCM heeft current procedural terminology (CPT) factureringscodes in de VS verworven en wordt steeds vaker gebruikt in academische tertiaire zorgcentra en sommige privéklinieken 7,8,19. RCM-beelden laesies met een bijna-histologische (cellulaire) resolutie. Beelden bevinden zich echter in het en-face vlak (visualisatie van één laag huid tegelijk) en de diepte van de beeldvorming is beperkt tot ~ 200 μm, voldoende om alleen de oppervlakkige (papillaire) dermis te bereiken. RCM-beeldvorming is gebaseerd op het reflectiecontrast van verschillende structuren in de huid. Melanine geeft het hoogste contrast, waardoor gepigmenteerde laesies helder en gemakkelijker te diagnosticeren zijn. Zo heeft RCM in combinatie met dermoscopie de diagnose (sensitiviteit van 90% en specificiteit van 82%) aanzienlijk verbeterd ten opzichte van dermoscopie van gepigmenteerde laesies, waaronder melanoom20. Vanwege een gebrek aan melaninecontrast in roze laesies, vooral voor BCC’s, heeft RCM echter een lagere specificiteit (37,5% -75,5%)21. Een conventioneel OCT-apparaat, een ander veelgebruikt niet-invasief apparaat, beeldt laesies tot 1 mm diep in de huid af en visualiseert ze in een verticaal vlak (vergelijkbaar met histopathologie)9. OCT mist echter cellulaire resolutie. OCT wordt voornamelijk gebruikt voor het diagnosticeren van keratinocytische laesies, met name BCC’s, maar heeft nog steeds een lagere specificiteit9.
Om de bestaande beperkingen van deze apparaten te overwinnen, is dus een multimodaal RCM-OCT-apparaat gebouwd22. Dit apparaat bevat RCM en OCT in een enkele, handheld imaging probe, waardoor de gelijktijdige acquisitie van co-geregistreerde en-face RCM-beelden en verticale OCT-beelden van de laesie mogelijk is. OCT biedt architectonische details van de laesies en kan dieper (tot een diepte van ~ 1 mm) in de huid in beeld brengen. Het heeft ook een groter gezichtsveld (FOV) van ~ 2 mm22 in vergelijking met het handheld RCM-apparaat (~ 0,75 mm x 0,75 mm). RCM-beelden worden gebruikt om cellulaire details te geven van de laesie die op OCT is geïdentificeerd. Dit prototype is nog niet gecommercialiseerd en wordt gebruikt als onderzoeksapparaat in klinieken23,24,25.
Ondanks hun succes bij het verbeteren van de diagnose en het beheer van huidkanker (zoals ondersteund door de literatuur), worden deze apparaten nog niet veel gebruikt in klinieken. Dit is voornamelijk te wijten aan het gebrek aan experts die deze beelden kunnen lezen, maar is ook te wijten aan het gebrek aan opgeleide technici die beelden van diagnostische kwaliteit efficiënt (binnen een klinisch tijdsbestek) aan het bed kunnen verkrijgen8. In dit manuscript is het doel om het bewustzijn en de uiteindelijke adoptie van deze apparaten in klinieken te vergemakkelijken. Om dit doel te bereiken, maken we dermatologen, dermatopathologen en Mohs-chirurgen vertrouwd met afbeeldingen van normale huid- en huidkankers die zijn verkregen met de RCM- en RCM-OCT-apparaten. We zullen ook het nut van elk apparaat voor de diagnose van huidkanker beschrijven. Het belangrijkste is dat de focus van dit manuscript is om stapsgewijze begeleiding te bieden voor beeldacquisitie met behulp van deze apparaten, die zorgen voor afbeeldingen van goede kwaliteit voor klinisch gebruik.
In dit artikel hebben we protocollen beschreven voor beeldacquisitie met behulp van in vivo RCM- en RCM-OCT-apparaten. Momenteel zijn er twee in de handel verkrijgbare RCM-apparaten: een rcm-apparaat met brede sonde of op de arm gemonteerd RCM (WP-RCM) en een handheld RCM-APPARAAT (HH-RCM). Het is cruciaal om te begrijpen wanneer deze apparaten in klinische omgevingen moeten worden gebruikt. Kankertype en -locatie zijn de belangrijkste factoren die de selectie van het apparaat bepalen.
