Preparing and Administering Secondary Intermittent Intravenous Medications

Secondary IV piggyback medication setup is used to give small, controlled amounts of IV solution, usually 25 to 250 mL. These infusions run longer than IV push medications. That slower delivery can lower the risk of problems linked to rapid infusion, such as phlebitis and infiltration.

In secondary IV therapy, the medication tubing connects to the primary macrobore IV tubing, so the secondary line is tied to the main infusion. The secondary bag is usually hung higher than the primary bag. This setup creates more gravitational pressure on the secondary solution and lets it piggyback on top of the primary IV infusion.

When the secondary infusion runs, the primary infusion pauses until the full secondary volume has been delivered. Then the maintenance fluid returns to the IV line right away. This helps ensure that the medication is completely infused.

Secondary IV infusions may be used when the patient’s fluid status allows a temporary pause in maintenance fluid. They are also useful in patients with hypervolemia, which means excess fluid volume. The procedure can be done with an infusion pump or with gravity tubing without a pump.

The process includes collecting the intermittent secondary IV medication, priming the secondary tubing, preparing the patient, calculating the medication administration rate, and setting the secondary IV drip rate. Some antibiotic medications are only stable for a limited time in solution, so proper setup matters for safe delivery.
