Extended Care Facilities

Extended care facilities are healthcare settings that provide ongoing medical, nursing, rehabilitative, or supportive care for people who do not require acute hospital treatment but cannot yet manage safely at home. Care typically follows an individualized plan, with clinical staff monitoring health status, administering treatments, assisting with daily activities, and coordinating rehabilitation or transition to community care. In clinical practice, these facilities may support recovery after illness or surgery, manage chronic conditions, and provide long-term care when functional limitations persist. Their structured services help maintain patient safety, promote independence where possible, and reduce avoidable hospital readmissions.
